TRI in Kolkata
Leaving the Crown Plaza Hotel in Delhi
at 7:45 AM on October 4 Monday, 2004, I moved to the Delhi
Airport to take the Jet Airways flight (9w 0922) departing
at 9:45 AM for Kolkata. The previous name of Kolkata was
“Calcutta”, which used to be the Capital of India during
the rule of United Kingdom. Kolkata is again a big city.
The city is surrounded by the fertile land. Its population
is 10 million in the city, and another 15 million people
are living around the city. Both in the morning and evening,
the traffic becomes so bad due to the cars coming from
and back to home. After arriving at the Kolkata Airport,
I directly moved to Anandalok Hospital, which is a private
hospital. The building does not seem to be a hospital
but a hotel. There 2 patients were waiting for my arrival.
The catheter laboratory was equipped with a Phillips'
digital machine (H-5000). The 1st case had a discrete
lesion in the proximal LAD at the bifurcation with a diagonal
branch. I did it from the right radial approach by using
an EBU 3.5 guiding catheter and placed a Vision stent
there. The patient and his family were very grateful to
my procedure. The next patient had a long lesion in RCA.
I again did the right radial approach and put a bare metal
stent. Again, the patient and his family expressed deep
appreciation to me. The 3rd case was one with the diagnostic
coronary angiography. I showed the transradial CAG. The
patient had normal result. We had a lot of discussions
about left main stenting, CTO and TRI with Drs Jain and
Roy.

I checked in Taj Hotel Kolkata in the
afternoon. I skipped the lunch. From 7:45 PM, we had a
round table discussion with several doctors of Interventional
Cardiology in Kolkata. Dr. Taun Praharaj in B.M. Birla
Heart Research Centre is doing a lot of cases. The famous
Mother Teresa stayed in Kolkata. Few people know about
it. She had effort angina and received angioplasties.
The 1st PCI was done in 1993 at La Holla Hospital in San
Diego, USA by using POBA. She had restenosis. Dr. Praharaj
himself performed repeated POBA and subsequently stent
implantation. The last PCI was done in 1996 in Kolkata.
Dr. Kahali is the senior doctor to Dr. Praharaj. I did
a lecture about TRI and PCI for CTO lesions. The discussion
was finished around 10:00 PM.

Next morning, I waked up at 4:00 AM and
left the hotel at 4:30 AM for Kolkata Airport . I took
the Jet Airways flight (9w 0202) departing at 6:05 AM
for Mumbai, where I changed the flight to 9w 0480 for
Ahmedabad.

はじめに
2004年10月4日午前7時45分にデリー・クラウン・プラザ・ホテルを出発し、カルカッタ行きの9時45分デリー空港発Jet
Airways便に乗りました。カルカッタの綴りは、以前は“Calcutta”でしたが、現在は“Kolkata”です。かつて大英帝国の統治時代にはここは首都でしたが、現在はデリーが首都です。街は肥沃な土地に囲まれています。人口は一千万人ですが、周辺には一千五百万人が住んでいます。朝や夕方の通勤時には行き帰りの自動車で大変な交通渋滞になります。カルカッタ空港に到着してから直接Anandalok病院に向かいました。この病院は私立病院です。建物は病院というよりも、ホテルのように見えます。カテ室にはPhillipsのH-5000が入っていました。最初の症例はLADの分岐部に限局性狭窄がありました。右橈骨動脈よりEBU
3.5を用いてVisionステントを植え込みました。患者さんと、ご家族はとても感謝されました。二例目は右の瀰漫性病変でしたが、やはりTRIでBMSを植え込みました。またもや患者さん達はとても感謝されました。三例目は橈骨動脈からの診断カテーテルでしたが、結果的には何もありませんでした。Jain先生とRoy先生と左主幹部ステント植え込み、CTOそしてTRIに関して色々な話をしました。

午後にTajホテルにチェック・インしました。昼食は抜かしました。夜の7時45分からラウンド・テーブル・ディスカッションをカルカッタのお医者さん達と行いました。B.M.
Birla Heart Research CentreのTaun Praharaj先生はたくさんPCIを行っておられます。有名なマザー・テレサはこのカルカッタにいて活動していたのですが、彼女が狭心症に患い、PCIを受けていたことはあまり知られていません。最初のPCIはPOBAでアメリカ・サンディエゴにあるラ・ホヤ病院で行われました。その後数回PCIが再狭窄のために行われ、最後のPCIは1996年にステント植え込みが行われました。このPraharaj先生その人がカルカッタで治療したのです。Kahali先生はPraharaj先生の先輩です。私は、TRIとCTOについての講義をしました。この会は夜の10時まで続きました。

翌朝は午前4時に起き、4時30分にはホテルを出発してカルカッタ空港に向かいました。6時5分発9w
0202便でムンバイに向かい、そこで飛行機を9w 0480便に乗り換えてアーマダバードに向かいました。
