From Ahmedabad to Tunis via Delhi and
Paris
I left Ahmedabad on October 6, 2004 by taking Jet Airways
9W 9706 flight departing at 20:30 for Delhi. At the Delhi
airport, I changed to an international airline of Air
France (AF 147) for Paris. It departed at 0:30 AM of October
7. The airplane arrived at the terminal 2A of Paris Charles
de Gaulle International Airport at 6:00 AM. I had to move
to the terminal 2F to take the same airline company flight
of AF 1984 for Tunis. I realized that the transit connection
in that airport was terrible. We had to take a connection
bus to move the terminals. However, I had to wait for
30 minutes to take the bus. I could reach the terminal
2F later than 7:10 AM. The flight for Tunis departed at
10:10 AM, although it was scheduled to depart at 8:30
AM. We were completely stacked in the airplane and waited
for the departure. There was no English announcement telling
why the departure was delayed. I asked a French gentleman
sitting next to me, “What is the problem?” He answered
me, “I am not sure. But, maybe the number of the people
sitting in the airplane is less than the passenger list.”
He was working the Professional Television Section in
Sony France. He told me that he was in charge of the sales
in whole French-speaking African countries. Since many
coastal African counties used to be under the rule of
France, they can speak French much more fluently than
English there.
All of the passengers were quite patient. Finally, the
airplane took off and went to the south over the city
of Paris. I could clearly see the Eiffel Tower as well
as the Gate of Triumph .

The 1st day in Tunisia
After 2 and half hours' flight, I arrived in Tunis Carthage
International Airport around 11:30 AM on October 7 Thursday,
2004. The airplane crossed over the Mediterranean Sea.
Before arriving at the airport, I could see a beautiful
reef and the ocean coast of Tunis. Although I had a cold
and terrible sore throat and was slightly febrile at that
time, I was excited, since this was the 1st time for me
to step in the Continent of Africa. The Republic of Tunisia
is a historical Islamic nation. The official language
is Arabic. However, most of people can speak French.

A young man with a signboard of “PACFIC” was waiting
in the arrival lobby. Since I was the 1st people coming
to the lobby, we waited for several persons arriving.
They were 3 sisters from Paris to join this meeting. It
took 1 and half hours from Tunis to Monastir, where the
meeting was held. The highway was nice. There were several
service stations. As in Japan, there were a snack corner,
convenience store, toilette and gas station. Monastir
is at 150 Km east to Tunis. In the car, the drive and
those 3 sisters were continuously talking each other in
French.
We passed through the downtown of Tunis and got off from
the highway at the city of Sousse. I could see the field
of Olive trees on the way. From Sousse to Monastir, it
took another 15 minutes. Both Sousse and Monastir are
the coastal cities facing the Mediterranean Ocean. Both
cities are old historical cities and now a kind of tourists'
area, where many beautiful resort hotels with beautiful
private beaches are built for the tourists from European
countries.
We arrived at the hotel “Emir Palace Hotel Monastir”,
where the convention was held.This hotel has also beautiful
private beach. It was already past 2:00 PM. I missed a
lunch. After getting in my room, I got very tired from
a cold and the unfamiliar environment. My condition was
very bad. I could not know what I had to do on that day.
I almost regretted to come to Tunisia while sparing my
busy schedule. I tried to connect my computer with Internet
by using a dial-up connection. But, it was failed. I asked
the front desk, “Do you have any idea to make an Internet
connection?” Her answer was quite simple, “It is impossible!”
I was totally alone. I felt the same feeling as I had,
when I first visited Beijing, China alone by myself to
demonstrate PCI in 1991. I kept staying in my room and
tried to take a sleep. After 8:00 PM, I went down to one
of the restaurant in the hotel, where the dinner was served
as buffet. I had a dinner alone by myself. The total price
there was 35,000 Dinars including one bottle of red wine.
The value of Dinar is similar to that of Korean Won. Namely,
one Japanese Yen is equal to 11 Dinars. After paying it
by using my credit card, I came back to my room with having
a loaf of bread and a bottle of wine. I drank several
glasses of red wine together pieces of bread. I turned
on my computer and played my favorite music titles by
using Window Media Player. Those are the theme music titles
of a Korean movie “Classic”, a 30 minutes' song by a Japanese
singer “Boro” titled “A woman born in Osaka” and a song
of “Inokashira Line” by Ms. Asami Chiuki. Those songs
touch me anytime. I also spent time to learn how to do
Win32 API programming. My computer is installed “Visual
Studio.NET 2003”. Thus, I can write C++, C# or Visual
Basic.NET programs and enjoy the programming anytime.
While reading a book and listening the music, I went to
sleep.

October 8 Friday, 2004
I waked up around 6:00 AM. I examined the program of
the meeting. However, I found that my role would be started
from 4:00 PM of the day as moderators. I had my own simple
breakfast by red wine and bread, which I took back last
night from buffet. On the program, it was announced that
the meeting would start from 8:00 AM on the day by a live
demonstration. I went to the auditorium of the same hotel
at 8:00 AM. However, only few people were there in the
room. I sat there and waited for the start of the meeting.
PAFCIC means “Panafrican Course on Interventional Cardiology”.
This year meeting was the 5th meeting. PAFCIC is held
every year in Monastir during the first Friday and Saturday
immediately after “Ramadan” of Moslem people. The meeting
is supported by the Ministry of Health of Tunisia Republics
and held mainly by Professor GAMRA Habib in Fattouma Bourguiba
University Hospital of Monastir.
Professor Gamra and his wife look like movie stars. People
of Tunisia are all beautiful. Men look like Greek statures,
and women look like a French movie star, Sofia Lauren.
The live transmission of PTMC (Percutaneous Transvenous
Mitral Commissurotomy) done by Professor Gamra started
from 8:30 AM. Following this transmission, we had a live
satellite lecture on “Percutaneous Artificial Valve Replacement
on Congenital Heart Disease” by Dr. Philipp Bonhoeffer
from England. Because my condition was still not good,
I went back to my room and sleep. Thus, I skipped the
breakfast and lunch on the day.

I was waken up around 4:00 PM by a telephone call from
Professor Gamra telling, “We were very worrying about
your. You did not join the lunch today. How are you?”
I sincerely told about my bad condition.
I returned to the auditorium around 4:30 PM, where everybody
including Professor Jean Marco from Clinique Pasteur,
Toulouse, France as well as Professor Gamra waited for
me. I was one of the moderators during that session with
Jean. We had a live case transmission from Washington
Hospital Center, USA. The patient had severe congestive
heart failure with calcified critical narrowings in both
RCA and LAD. Dr. Kenneth Kent and Dr. August Pichard did
good job to fix these problems by doing rotablator and
DES implantation. Jean and I explained the strategy in
English and a lot of discussion with those American doctors,
although many people spoke in French. My sick mind gradually
became healthy, since I had a best friend and teacher,
Professor Jean Marco, sitting together with me.
Everybody met together in the lobby of the hotel around
9:00 PM to join the dinner, which was sponsored by a huge
French pharmaceutical company Sanofi. We took buses to
the city of Sousse. After getting off from the bus, we
walked together for 10 minutes to one of the best restaurants
in Tunisia, “Le Mediterranean”. Again, I was alone. I
tried to find where I took seat. Everybody came to the
restaurant together with friends or spouses. However,
I was totally alone. I was wondering. Suddenly, a light
came to me. Assistant Professor Addad Faouzi called me.
He took Professor Lafont Antoine and his wife together.
They were very kind to invite me to the same table.

Now, I can tell you why I came here to PAFCIC meeting.
The great President of TokSyuKai Medical Group, Dr. Torao
Tokuda, is very serious to open new hospitals in Asian,
West European or African countries in order to accomplish
his idea that the medical treatment should be equally
done for any people living in the world. He is inviting
many young doctors from those countries to Japan and our
group hospitals. Assistant Professor Adadd Faouzi was
one of them from Tunisia. He stayed in my hospital for
a week and came to the catheter laboratory in June this
year. While witching my difficult cases, he proposed me
to join PAFCIC meeting as one of the invited speaker.
I tried to negotiate my schedule and finally decided to
visit there after my pre-determined schedule in India.
Professor Lafont is a famous French doctor in Interventional
Cardiology. He is a Professor in Hospital Europeen Georges
Pompidou, University Paris 5. He likes to join a CCT meeting,
because he can learn a lot. Because we were very excited
with the talk between me, Dr. Adadd, him and his wife,
the time passed very quickly. When we waked up from the
talk, it was almost 0:30 AM. Dr. Adadd took us to the
hotel by driving his own car of Mercedes-Benz.

October 9 Saturday, 2004
I was assigned to moderate the session with Professor
Patrick Serruys from Erasmus University, Rotterdam, Holland
from 8:00 AM. Skipping the breakfast, I went to the auditorium
by 8:00 AM. However, there was only Patrick sitting. He
was preparing his lecture presentation in his laptop computer.
We waited for people coming. We decided to start the meeting
at 8:30 AM.
Patrick first did an excellent lecture on the virtual
histology by using intravascular ultrasound examination.
After his lecture, we had a live case transmission by
Professor Gamra. The patient was a 40 years' lady, who
had a previous saphenous-vein bypass graft to LAD 10 years
ago. She was admitted due to unstable angina. Her coronary
angiography showed 2 discrete critical narrowing in the
saphenous vein graft. Professor Gamra did an excellent
treatment by placing stents in both lesions without any
distal protection. While I was sitting in the moderator
chair after this live case, I received a telephone call
from Professor Gamra. He told me, “If you please, could
you come to the hospital, since we have a patient with
chronic occlusion. Could you scrub with us?” It was really
a great pleasure for me to visit the catheter laboratory
and do cases there by myself. I quickly answered, “Oui!”
Several doctors took me to the hospital. The hospital
was in the center of the city of Monastir, and it took
about 15 minutes by car. The name of the hospital is Fattouma
Bourguiba University Hospital of Monastir. In this hospital,
they are doing about 450 PCIs every year. The price of
Cypher is also a serious problem here in Tunisia. It is
about 2,500 Euros. The price of BMS is 1,000 Euros. All
the cost for BMS is covered by the social insurance in
Tunisia. However, if the patient wanted to have DES, he
must pay 1,000 Euros by himself. Cypher was launched 2
years ago in Tunisia. Since that time, about 1,000 pieces
of Cypher were used in the country. In Monastir hospital,
they used 70 pieces of Cypher by now. This hospital is
one of the Government hospitals. They have 13 Government
hospitals and 10 private hospitals within the country.
The hospital is a typical Arabic building. I went up to
the 5th floor to meet with Professor Gamra. He showed
me the angiographies of the patient. The patient had 2
years history of effort angina. Thus, his RCA was supposed
to have an occlusion at least for 2 years. The occlusion
was in the middle of RCA with rich bridging collateral.
I declared to do PCI from the femoral approach by using
a 7 French guiding catheter without doing the contra-lateral
simultaneous bi-lateral angiograms.
They had one catheter laboratory, where a Philips' H-3000
X-ray machine was installed. Addad did the femoral artery
puncture. I took a 7 French JR4.0SH guiding catheter,
and waited for the live transmission. The transmission
suddenly started by Patrick's call from the auditorium.
After professor Gamra made a brief introduction of the
case history and me, I explained the strategy. First,
I attempted with a Magic-FA guidewire alone. However,
it was not successful, as I expected. Patrick made several
comments. Thus, we could have fruitful discussion between
the auditorium and the catheter laboratory. Then, I took
a Transit microcatheter, and replaced the guidewire to
a Conquest-Pro guidewire. It could successfully go through
the occlusion. I could hear a lot of applause from the
audience. Patrick told me, “Great!”
However, I could not pass a 1.5 mm balloon catheter through
the occlusion. Professor Gamra prepared a 1.25 mm balloon.
But, it was not successful. The patient's RCA had a relatively
big conus branch before the occlusion. While everybody
in the laboratory was wondering, I asked a BMW guidewire.
Nobody could understand what I would do. I passed this
guidewire into the conus branch. Then, I passed the previously-used
1.5 mm balloon catheter over this guidewire. The angle
of the conus branch from the main RCA was so acute that
I could not push the 1.5 mm balloon into the conus branch.
I inflated the 1.5 mm balloon in the proximal part of
RCA. While keeping the inflation, I tried to push a 1.25
mm balloon into the occlusion. Owing to the effect of
“Anchoring-balloon”, I could successfully push the 1.25
mm balloon into the occlusion. Everybody in the laboratory
as well as in the auditorium was surprised to see this
maneuver. After dilating the lesion by a 2.0 mm balloon,
I asked Professor Gamra whether I could use a Cypher stent
or not, because the vessel diameter seemed to be only
less than 2.5 mm in diameter. His answer was clear. It
was “If you would do it, you could use a Cypher stent.”
I placed a 2.5 x 33 mm Cypher stent there. Everybody was
excited to have this wonderful case during the live transmission.
On the way back to the hotel, Dr. Addad took me to the
short sightseeing in the city. The 1st President of Tunisian
Republics, Mr. Bourguiba Habib, was born in this city
Monastir. He was great. When he died, his private property
included only 1,000 Euros. He spent all of his own property
to the development of Tunisia. He is very famous for accepting
the right of women first in Islamic countries. In the
center of Monastir, he and his family are buried in the
tombs in a beautiful Mosque.

After visiting the Mosque, we went to the old castle,
where people were busy to prepare for the inauguration
ceremony for the new President. The new President will
be elected next week. However, it was already decided
that the current President Zine El Abidine Ben Ali would
be elected again. Next to the castle, we could see the
oldest Mosque in Monastir .
Monastir has a beautiful harbor. The ocean water was very
clean and beautiful.

We went back to the hotel and had lunch together with
the other attendants. Everybody praised my good job highly.
I was a little bit proud of my job. After having a lunch,
I came back to my room. It was already 3:00 PM. Since
the program showed that my next duty would start from
4:30 PM as a moderator and Presentator, I took a rest.
I came down to the auditorium again at 4:30 PM. However,
everything was delayed a lot behind the schedule. My presentation
started from 6:00 PM. Professor Gamra asked me to do the
lecture titled “Newest Technology on Angioplasty for CTO
lesions.” However, I changed the title to “Newest Techniques
of Angioplasty for CTO lesions.” I had several questions
from the audience in French. Professor Patrick Serruys
kindly translated them into English. He did the last lecture
next to me about the future of coronary stents. His presentation
was very interesting. Everything was finished at almost
7:00 PM. I learnt that everything went behind the schedule
in Tunisia.
Gala Dinner started from 9:30 PM in the same place. There
were folk music and dances. Ms Helen Van Beusekom, MD
from Rotterdam came to me. “I think you can speak only
in English. I also do too. They speak in French. Thus,
we sit together. Do you think it is better?” She was very
kind. We sat together in the most front seats. We could
enjoy the music together.

October 10 Sunday, 2004
A mini-bus was arranged to send us to the Tunis airport
around 10:00 AM. Professor Jean Marco, his wife, Dr. Iradi
Gandibackch from Madrid, Spain, Dr. Anne France Bourdeaux
from Paris and I took the same bus together. Dr. Gandibackch
is very famous for the carotid stenting. Everybody except
me continuously spoke each other in French. I had a short
sleep. Anyway, it was exciting for me to go together in
a small bus with those world-distinguished doctors.
I took a Tunisair flight (TU 790) for London Heathrow
Airport departing at 13:20. We flied over the city of
Tunis. Thus, I left this beautiful country Tunisia. Although
I almost regretted to come to Tunisia in the beginning,
I was finally very much satisfied with all of the experiences
I had in Tunisia. I met many people here. This trip to
Tunisia was very fruitful in my life.
The airplane arrived at Heathrow Airport after 3 hours'
flight. I could see the last supersonic airplane of Concorde
there. I change to the Japan Airline flight of JAL 402
there to Tokyo. Finally, I came back to Japan at 4:15
PM on October 11. Thus, my long hard journey started from
October 2 finally came to the end.

アーマダバードからデリーそしてパリ経由でチュニスへ
2004年10月6日20:30発のデリー行きJet Airways 9W 9706便に乗ってアーマダバードを飛び立ちました。デリーでパリ行きのAir
France AF 147便に乗り換えました。この飛行機は翌日の午前0時30分に飛び立ちました。パリ・シャルル・ド・ゴール国際空港に到着したのは午前6時でした。チュニス行きの同じAir
France AF 1984便に乗り換えるためにターミナル2Fまで移動せねばなりませんでした。この空港のコネクションは、まったくもってひどいものです。とにかくバスに乗らねばなりません。しかし、そのバスに乗るだけでも30分かかるのです。結局、ターミナル2Fに着いたのはもう午前7時10分になっていました。予定では午前8時30分発のフライトでしたが、結局10時10分になってチュニスに向けて飛び立ちました。我々は飛行機の中に缶詰でした。どうして出発が遅れているかについての英語でのアナウンスは全くありませんでした。隣に座っていたフランス人の男性に、「どうしたの?」と聞いてみました。彼は、「僕にも分からない。多分、搭乗人数と乗客リストが合わないのじゃないか。」と、答えました。彼は、Sonyフランスで業務用テレビジョン・システムの責任者としてアフリカのフランス語圏全部を受け持っているそうです。アフリカ沿岸諸国の多くはかつてフランス領でしたので、皆英語よりもフランス語のほうが得意です。
乗客皆はとても辛抱強く待ちました。結局、飛行機は飛び立ち、パリの上空を南へと横切りました。エッフェル塔や凱旋門が見えました。

チュニジア第一日目
2時間半の飛行の後、2004年10月7日午前11時30分に、チュニス・カルタゴ国際空港に到着しました。飛行機は地中海を横切ったのですが、着陸前には美しい環礁と、チュニスの海岸を見ることができました。その時、私は風邪をひき、とても喉が痛く、また少し熱もあったのですが、アフリカ大陸に初めて足を踏み入れるため興奮していました。チュニジア共和国は歴史のあるイスラム国家です。公用語はアラビア語ですが、多くの人がフランス語を喋れます。

PACFICの看板を掲げた若い人が到着ロビーで待っていました。私が一番に降りてきたので、他の人々が来るのを待たねばなりませんでした。私はパリからの三人の看護婦さんを待ち、一緒に会場まで向かいました。会場まではとても快適な高速道路で一時間半でした。途中にはサービス・エリアがあり、日本と同じように軽食コーナー、コンビニ、トイレそしてガソリン・スタンドなどがあります。モナスティアはチュニスから150Km東に位置します。車の中では三人の看護婦さんとドライバーがフランス語でお喋りしていました。
チュニス中心部を走り抜け、スースで高速道路を降りました。途中にはオリーブ畑がありました。スースからモナスティアまでは15分ぐらいでした。スースもモナスティアも地中海に面した都市です。両方とも歴史のある街であり、今では多くの旅行客が訪れます。そこには美しいリゾート・ホテルとプライベート・ビーチがあるのでヨーロッパからたくさんの旅行客が訪れます。
私たちは会場でもあるエミル・パレス・ホテルに到着しました。このホテルにも綺麗なプライベート・ビーチがあります。既に午後2時だったので昼食は抜かしました。部屋に入ると風邪と慣れない環境のために疲れがどっと出ました。とても辛い状態でした。その日何をすれば良いのかさっぱり分かりませんでした。忙しいスケジュールを割いてチュニジアに来たことを後悔しました。インターネットにダイアル・アップで接続しようとしましたがうまくいきません。フロントに、「うまくインターネットに継ぐ方法無い?」と聞いたところ、何と、答えは「不可能です。」の一言でした。
全く一人ぼっちでした。ちょうど1991年に初めて一人で北京をPCIするために訪れた時と同じ感じでした。部屋に篭り、眠ろうとしました。午後8時になって、下のレストランに降り、バイキングの夕食を摂りました。一人ぼっちで夕食を摂りました。赤ワイン一本と併せて35,000ディナールでした。ディナールは丁度、韓国ウォンと同じぐらいです。つまり、1円が11ディナールぐらいです。クレジット・カードでそれを支払ってから、パンとその赤ワインを持って部屋に戻りました。そして、パンをつまみに赤ワインを飲みました。コンピューターのスイッチを入れて、Window
Media Playerで、韓国映画「ラブ・レター」のテーマ・ソング、”ボロ”の「大阪で生まれた女」そして”あさみ ちゆき”の「井の頭線」を聞きました。とても良いのです。そして、Win32プログラミングの勉強をしました。私のコンピューターには”Visual
Studio.NET”が入っているので、C++、C#あるいはVisual Basic.NETで何時でもプログラム遊びが、出来ます。
本を読み、音楽を聴いている間に眠ってしまいました。

午前6時に起きて、学会プログラムの点検をしました。私の仕事は午後4時スタートでした。昨夜持ってかえったパンと赤ワインで簡単に朝食を済ませました。プログラムによれば学会は午前8時からライブデモンストレーションでスタートすることになっていました。学会場に午前8時に降りていきましたが、部屋にはほとんど人がいませんでした。座って、学会が始まるのを待ちました。
PAFCICというのは”Panafrican Course on Interventional Cardiology”の頭文字です。今年が第5回目です。PAFCICは毎年このモナスティアでラマダン明けの最初の金曜日と土曜日に開催されます。チュニジア保健省が講演し、モナスティアにあるファトゥマ・ブルギバ大学病院のGamra教授によって開催されています。
Gamra教授も奥様もまるで映画スターのようです。チュニジアの人々は皆とても綺麗です。男はギリシャ彫刻のようですし、女はかつてのフランスの映画スター、ソフィア・ローレンのようです。
Gamra教授が術者の経皮経静脈的僧房弁交連裂開術(PTMC)が8時半から始まりました。このライブ中継に引き続いて、イギリスからフィリップ先生によって先天性心疾患に対する経皮的弁置換術の講演がありました。まだ体調が悪かったので、部屋に戻って休みました。結局、まともに朝食も昼食もとりませんでした。

4時頃、Gamra教授の電話で起こされました。「昼食にも来なかったし、とても心配しているよ。」体調が思わしくないことを伝えました。
午後4時半に会場に戻ると、フランス・ツールースのMarco教授やGamra教授を初めとして皆が私の到着を待っていました。私はそのMarco教授と共にそのセッションのモデレーターでした。今度はアメリカのワシントン病院センターからのライブデモンストレーション中継でした。患者さんは心不全があり、RCAとLADに石灰化を伴った狭窄がありました。Ken先生とPichard先生がロータブレーターとDESを用いてうまいこと治療しました。皆はフランス語で話していましたが、Marco教授と私は英語で治療戦略について話し、アメリカの医者と多くの討議をしました。私の良き友であり、先生でもあるMarco教授と一緒に座ったため、徐々に痛んだ心は回復してきました。
フランスの大きな製薬会社であるサノフィ社主催の夕食会に出席するため、皆が午後9時にロビーに集合しました。バスでスースまで移動しました。バスを降りてから10分ぐらい歩いてチュニジアで一番良いレストラン「地中海」に行きました。またもや一人ぼっちでした。座り場所を探しました。皆、友達や奥さん連れで来ていました。でも私は完全に一人でしたが、突然光が差しました。Addad先生が私を呼びました。彼はLafont教授ご夫妻と一緒でした。

そもそも何で私がPAFCICに参加したのでしょう。徳洲会理事長の徳田虎雄先生はアジア、東ヨーロッパそしてアフリカの国々に本格的に病院を建設しようとされています。それは、その理念である、「生命だけは平等だ」というものに基づいてのものなのです。理事長はこれまでに多くの若い医師を日本にそして、私たちの病院に招いてこられました。Adadd先生もチュニジアから来られたメンバーの一人でした。彼は私たちの病院に6月の一週間ぐらいおられ、カテ室に来られていました。私の難しい症例を見ている内に、PAFCICに招待したいと言われました。そこで、私はスケジュールを調整して、この招待を受けるためにインドから直接入ることにしました。
Lafont教授はこの分野で非常に有名なフランスの医師です。彼はパリ第五大学付属病院である、ポンピドー病院の教授です。彼は、とてもたくさん学ぶものが多いのでCCTに参加するのが好きだと言われました。私、Adadd先生、Lafont教授ご夫妻との間で色々な話をしましたので、時間はあっという間に過ぎました。気づくともう午前0時30分でした。Adadd先生が彼のベンツでホテルまで送って下さいました。

ロッテルダムのエラスムス大学セライス教授と一緒に午前8時から座長をやることになっていました。朝食を抜かして8時には会場に行きました。しかし、そこにはセライス教授しかおりませんでした。彼は自分のコンピューターで講演の準備をしていました。皆が集まるのを待ち、8時半から学会が始まりました。
セライス教授は冠動脈内超音波検査を用いた”仮想病理”のとても興味深い講演を行われました。この講演に引き続いてGamra教授によるライブデモンストレーションが行われました。患者さんは40歳台の女性で、10年前にLADに対して静脈グラフト・バイパスが行われています。今回、不安定狭心症で来院されました。冠動脈造影の結果、静脈グラフトに二ヶ所限局性の狭窄を認めました。Gamra教授は末梢保護無しにステントを二個植え込みうまいこと治療されました。このライブの後、座長席に座っているとGamra教授からの電話がありました。「もし良かったら、この後、慢性完全閉塞の患者さんがいるのでカテ室に来て、一緒にPCIをしない?」というものでした。もちろん、「はい」と答えました。
数名の医師が私を病院に連れていってくれました。病院はモナスティアの中心部にあり、車で15分かかりました。その病院はファトゥマ・ブルギバ大学病院という国立大学病院で、年間450例ぐらいのPCIをしています。チュニジアではCypherの価格は2,500ユーロぐらいし、これに対してBMSの価格は1,000ユーロ程度だそうです。チュニジアではBMSの場合には全て保険がカバーしますが、Cypherをいれた場合には、1,000ユーロを自分で支払わねばならないそうです。Cypherは2年前にチュニジアに入ってきましたが、これまでに国全体で1,000個のCypherが使われ、この病院では今まで70個植えたそうです。チュニジアには13の国立病院と10の個人病院があります。
病院は典型的なアラブ風の建物でした。五階まで上がりGamra教授と会い、そこで患者さんのシネを見ました。患者さんは狭心症歴2年間であり、RCAの中ほどで慢性完全閉塞を示し、ブリッジ副血行が発達していました。そこで、7
Frenchのガイディング・カテーテルを用いて対側造影無しで、経大腿動脈的冠動脈インターベンションを行うことに決めました。
カテ室は一つでしたが、PhilpsのH-3000が入っていました。Addad先生が動脈穿刺をされました。7
FrenchのJR 4.0SHを入れてライブ開始を待ちました。セライス教授の呼びかけで突然ライブが始まりました。Gamra先生が簡単に病歴と私を紹介したあと、私が戦略を話しました。最初にMagic-FAガイドワイヤー単独で入りましたが、案の定うまくいきませんでした。セライス教授がいくつか解説をされました。次に、私はTransitを用いて、Conquest-Proに変更しました。そして、うまいこと完全閉塞を越えました。会場からの拍手喝采が聞こえました。セライス教授も、「素晴らしい!」と言われました。
しかし、ここからが問題です。1.5mmバルーンでも病変を超えることが出来ませんでした。Gamra教授は1.25mmのバルーンを持ってこられましたが、それでも駄目でした。患者さんのRCAには比較的大きなConus枝が閉塞部の手前から出ていました。カテ室の皆が訝りましたが、私はBMWガイドワイヤーを要求しました。カテ室内の誰も私が何をしているのか理解できませんでした。私はそのBMWをConus枝に挿入し、それからさっき用いた1.5mmバルーンをこのガイドワイヤーに乗せました。Conus枝は鋭角的にRCAから分岐するため、このバルーンをConus枝に入れることはできませんでした。そこで、RCA近位部でそのバルーンを拡張しました。それを膨らましながら、1.25mmバルーンを閉塞部に押し付けました。”アンカー・バルーン”効果で、うまいこと1.25mmバルーンを閉塞部に挿入することが出来ました。カテ室内の誰もが、そして会場の皆が、この手技を見て驚きました。それから2.0mmバルーンで拡張した後、私はGamra教授に、血管径がどうみても2.5mm程度しかないので、Cypherを用いても良いか否かを聞きました。答えは明確でした。「もしも、そうしたいのならばCypherを使っても良いよ。」そこで、私は2.5
x 33mmのCypherを植え込みました。ライブデモンストレーションの最中にこのような症例があったことに皆が興奮していました。
ホテルに帰る途中で、Addad先生が短い市内観光に連れていって下さいました。チュニジア共和国初代大統領、ブルギバ・ハビブ氏はこのモナスティアで生まれました。彼は偉大でした。彼が死んだ時に残された遺産はたったの1,000ユーロ(日本円にして14万円程度)しか無かったそうです。彼は自分の財産のほとんどをチュニジア発展のために使ったのです。彼はイスラム国での女性の権利拡大に尽くしたことで有名です。モナスティア中心部には彼と彼の家族の墓がある美しいモスクがあります。

このモスクの後、古い城を訪れましたが、そこでは来週の大統領選セレモニーの準備中でした。でも、現大統領のエル・アビディン氏が再選されることが決まっているそうです。城の隣にはモナスティア最古のモスクがありました。
モナスティアには美しい港があります。海の水はとても透明で奇麗でした。

ホテルに戻り、他の参加者と一緒に昼ごはんを食べました。皆に私の仕事振りを称えられましたので、少しばかし自慢です。昼食後、自分の部屋に戻りましたが既に午後3時でした。私の次の仕事はプログラム上、午後4時半からだったので少し休みました。
午後4時半には会場に降りて行きました。しかし、全てが大幅に遅れていました。結局、私の講演が始まったのは午後6時からでした。Gamra教授からは、「慢性完全閉塞病変に対する最新のテクノロジー」というタイトルが与えられましたが、私はタイトルを「慢性完全閉塞病変に対する最新のテクニック」に変えました。聴衆からいくつもフランス語で質問がありましたが、セライス教授が英語に翻訳して下さいました。彼はまた、今後の冠動脈ステントの方向についての講演を最後に行われましたが、とても興味深いものでした。全てが終わったのが午後7時でした。私はチュニジアでは全てが時間遅れで進む、ということを学びました。
最後の夕食会は同じ場所で午後9時半から行われました。そこでは民族音楽と民族舞踊が披露されました。ロッテルダムのヘレン先生が私の所に来て、「あなたも英語しか喋れないのではない? 皆、フランス語喋っている。だから私と一緒に座らない? その方が良いでしょ?」と話しかけてきました。彼女はとても親切で、私たちは一緒に最前列で座りました。そして一緒に音楽を楽しみました。

午前10時頃、小さなバスに乗ってチュニス空港に送られました。マルコ教授ご夫妻、スペインのガンディバック先生、パリのブルドー先生とご一緒しました。ガンディバック先生は頚動脈ステントでとても有名です。私以外は皆お互いにフランス語で喋り続けでした。何れにしても、これらの世界的に有名な先生方と一緒のバスに乗れて興奮しました。
午後1時20分初のチュニジア航空ロンドン・ヒースロー空港行きの便(TU 790)に乗りましが、飛行機はチュニスの街の上を飛びました。こうして美しい国チュニジアを発ちました。最初はチュニジアに来たことを後悔しましたが、最後にはチュニジアで得たたくさんの経験に満足しました。ここでたくさんの人々に会いました。このチュニジア旅行は私の人生でとても有意義でした。
飛行機は3時間の飛行の後、ヒースロー空港に到着しましたが、そこには超音速旅客機コンコルドの最後の飛行機がとまっていました。私は東京行きのJAL
402便に乗り換えて、結局、10月11日の午後4時15分に日本に帰ってきました。こうして10月2日に始まった私の長くて過酷な旅がついに終わりとなりました。
