Just one-night stay in Beijing

On October 17 Sunday, 2004, I flied to Beijing from Tokyo by taking an ANA flight. The seats were fully occupied by many elder Japanese tourists. In the history of Japan, Japanese people have developed our essential culture by learning from China through Korea for more than 1,000 years. Thus, many Japanese people, especially elder ones, have the special feeling on China and want to visit China at least once in their life.

I arrived at Beijing Capital Airport around 1:00 PM. Since I was asked to do a live case transmission for a CTO case through an e-mail from Professor Huo Yong 2 days before, I hurried directly to the 2nd affiliated hospital of Beijing University Medical School , where I have been several times before. The 1st visit in this hospital was in December 1991, which was my 1st trip to China as well as my 1st visit in the hospital in China. Although I went to the catheter laboratory early in the morning that time, the old machine of French company, CGR, was broken. The X-ray technicians in the hospital tried to fix the problems but failed. Professor Hu Dayi decided to send the patients already scheduled to Anzhen Hospital Cardiovascular Center. While they were preparing for the patientsﾕ transfer, I visited Tian-An-Men Square first, where many people were killed just 1 and half years ago. On the way to the hospital from the airport, I crossed the same road in front of Tian-An-Men Square. The scenery seemed to me unchanged for the past 13 years, although I got older

This time, I was invited by the 15th Great Wall International Congress of Cardiology, which is conducted by Professor Hu Dayi. This meeting is the oldest international Congress of Cardiology and including the live demonstrations for PCI, Valvuloplasty, EP or peripheral interventions. After my 1st visit in Beijing, I was continuously invited as one of the invited foreign faculty members every year since 1992.

Before starting my case, I was waiting in the conference room, where a big map was displayed. On this map, there were many red circles and stars, which indicated the cities. Professor Huo has been to these cities in order to teach PCI for the local physicians, or the local physicians came to the hospital to learn PCI from those cities. The case prepared for me was an about 60 years old male patient, whose RCA showed a chronic total occlusion in the middle segment with bridge collateral. I used the trans-femoral approach for this patient without contra-lateral angiograms. The initial attempt using a Magic-FA guidewire failed with the entry into the false lumen. I used the parallel guidewire technique by using a Conquest-Pro guidewire. This guidewire went into the distal true lumen. After the balloon dilatation with a 1.5-mm balloon, everything was fine. Finally, I placed a Cypher stent there. After finishing this case, I took a photo with my assistant Chinese young doctor and a head nurse in the laboratory, with whom I have been familiar.

I moved to China World Hotel to check in. The Great Wall Meeting was held within this hotel this year. Dr. Ferdinand Kiemeneij, who is one of my best friends as well as my teachers, came to Beijing this time with taking his elder son. In the hotel lobby, we had a chance to take a seat together with those famous doctors of Jia Sanqing, Hu Dayi, Wang Lei, Thach Ngyuen and Xubo.

New Building	Program of TRI Live Demonstration in Beijing

I waked up early in the morning. I arrived at Beijing Friendship Hospital before 7:30 AM. This one of the oldest hospitals in Beijing was now renewing the hospital buildings. They have already built up a new outpatient building. Total of 7 cases were prepared for me during the live case transmission in the morning. Actually, this live demonstration was held as “The 3rd Beijing International TransRadial Coronary Intervention Live Demonstration Course”, which was held by Professor Jia Sanqing. Among these 7 cases, I had 2 patients with unprotected left main bifurcation lesions, 2 patients with chronic total occlusions and others. I treated those cases all by TRI. We used only one catheter laboratory, where a GE flat panel machine is installed. Everything from the beginning to the finish of TRI was shown during the live transmission. I did the radial artery puncture by myself for these 7 cases from the beginning and showed how to make a hemostasis. Dr. Fu Xianghua from HuBei Medical University 2nd affiliated People's Hospital in ShiJiaZhuang of HuBei Province, Dr. Qiao Shubin from Fuwai Hospital as well as Jennifer and Dr. Wang from Beijing Friendship Hospital assisted me.

I finished these 7 cases by 11:45 AM and hurried up to the Airport to take a return flight to Japan departing at 2:50 PM

たった一泊の北京訪問

2004年10月17日日曜日に私は全日空便で東京から北京に入りました。座席は年配の日本人旅行客で満席でした。この1,000年間に渡り、日本は中国から朝鮮半島を通して数多くの文化を導入してきました。このためでしょうか、特に年配の日本人は生涯で少なくとも一度は中国を訪れたい、という特別な気持ちがあるようです。
北京首都国際空港に到着したのは午後1時ぐらいになっていました。この2日前に、Huo教授から、慢性完全閉塞の患者さんのライブデモンストレーションを行うように電子メールで依頼されていたので、北京大学医学部第二付属病院に直行しました。この病院にはこれまでも数回訪れたことがあります。最初の訪問は1991年の12月のことでした。実際、この時に初めて中国を訪れたのみならず、中国の病院を訪問したのです。その時、朝早くカテ室を訪問したのですが、フランス製CGRのレントゲン機械は故障していました。レントゲン技師がそれを直そうとしていましたが、結局失敗に終わりました。Hu教授は、患者さんを安貞病院心血管センターに移送することを決断しました。患者さんを移送する間、私は天安門広場を初めて訪れました。その天安門広場では、ほんの1年半前に多くの人々が死んだのです。空港から病院に行く途中で、この天安門広場を横切りました。その景色はこの13年間何も変わっていないように見えました。でも私はその間年をとりました。

今回の訪問は、Hu教授主催の第15回長城国際心臓病学会に招聘されたためです。この学会は中国で最も古い国際心臓病学会であり、PCI、弁形成、電気生理あるいは末梢血管インターベンションなどのライブデモンストレーションを含んだ学会です。北京を最初に訪問した翌年の1992年から、私は毎年この学会から招聘外国人医師として呼ばれています。

自分の症例を待つ間、カンファレンス室にいたのですが、そこには大きな地図が掛かっていました。この地図には赤い丸や星がたくさんありましたが、それらの都市は、Huo教授がPCI指導のために訪れたか、あるいはその土地から研修医を受け入れていることを示していました。私に用意されていた症例は60歳台の患者さんで、右冠動脈が完全閉塞を示しており、末梢はブリッジ副血行路のため良く造影されていました。大腿動脈からのアプローチをしましたが、対側造影は行いませんでした。最初、Magic-FAで入りましたが、偽腔に入ったので、Conquest-Proを用いてDouble guidewire techniqueを使い真腔を捉えました。1.5mmバルーンで拡張後は全てうまくいきました。最終的にCypherステントを植え込みました。この症例を終了した後、私をアシストしてくれた若い中国の先生、そしてカテ室婦長さんと一緒に写真を撮りました。彼女のことは以前から知っています。

中国大飯店に行きチェック・インしました。今年は、この学会はこのホテルで開催されました。キムニー先生は私の友人でありまた先生でもありますが、今回息子さんを連れて来られました。ホテルのロビーで皆で写真を撮ることができました。

New Building	Program of TRI Live Demonstration in Beijing

朝早く起床しました。そして、午前7時半には北京友誼病院のカテ室に到着しました。この北京の中でも古い病院は新しい建物を建造中でした。既に新しい外来棟が建っていました。この午前中に私のために7例の症例が用意されていました。実際、このライブデモンストレーションは第三回北京国際経橈骨動脈冠動脈インターベンションライブデモンストレーション学会の一環として行われました。この学会はJia先生により開催されました。7例の中には、2例の非保護左主幹部病変の患者さん、2例の慢性完全閉塞の患者さんが含まれていました。この全てを経橈骨動脈冠動脈インターベンションにより治療しました。カテ室はGE社製のフラット・パネル・マシンが備え付けられている一つだけでしたが、私はこのライブデモンストレーションの間、穿刺から止血まで全て行いました。Fu先生、Qiao繊維背などが私の助手をして下さいました。
何れにしても、私は経橈骨動脈冠動脈インターベンションの無限の可能性を中国の医師に示すことができました。全てを経橈骨動脈冠動脈インターベンションで行ったからこそ、4時間の間に何の合併症も無く、7例の困難な症例の治療が出来たのです。このライブデモンストレーションは中国の医師たちに大きな衝撃を与えました。この結果にとても満足しています。

7例を午前11時45分までに終了し、午後2時50分初の日本に帰る便に乗るために空港に急ぎました。
