Introduction
We had the live demonstration course (Tokyo
Live Demonstration) and the scientific meeting of the Kanto-Koshinetsu
Regional Conference of the Japanese Society of Interventional
Cardiology in Tokyo on October 29 and 30, 2004. Since I
am the president of the Regional Conference, I stayed in
Tokyo from October 28 through 30. Everybody was happy to
have a lot of attendants in spite of the recent big earthquake
disaster in Niigata Area. However, some physicians, nurses
and technicians could not join the meeting, because the
disaster hit their hospitals as well as home. I would be
really worrying for them.
On November 2 Tuesday, 2004, I again flied to Bangkok by
the JL707 flight departing at 6:45 PM from Tokyo. The security
check in the Airport is quite tight in these days. Many
passengers are late for the departure time. As with the
other flights, our JL707 flight departed later than the
schedule. Finally, I arrived at Bangkok International Airport
around 0:30 AM on November 3. Since I had to take an early
morning flight, I stayed at Amari Airport Hotel, which is
located just in front of the Airport and at just 5 minutes'
walk distance.

I waked up at 4:45 AM and went to the Airport
by 5:15 AM to take the flight to Khon Khan by TG040 (Thai
Airline) departing at 6:30 AM. Khon Khan is the academic
city of the north-east area of Bangkok. There is a big University
named “Srinakarint Kohn Khan University”. The medical school
of this university has a big heart center named “Queen Sirikit
Heart Center”. I visited this hospital on
August 15 Friday, last year.
Last year, the catheter laboratory in Queen Sirikit Heart
Center was in a small poor-looking building. However, a
new heart center building was opened several months ago,
where they have 1 Siemens, 1 Philips and 1 Philips flat-panel
laboratories. I arrived at the catheter laboratory by 8:00
AM. Drs. Chaiyasit, Songsak and Wirote 10 minutes later
after my arrival. They prepared 8 cases including CTO for
me. My original return flight to Bangkok was scheduled to
depart at 20:30 from Khon Khan. However, since I could finish
all of these 8 cases through the radial approach by lunch
time, I changed the flight schedule to the earlier flight.
We had lunch all together in the conference room. I could
catch the return flight at 2:30 PM. I went to the downtown
Bangkok from the airport and stayed at Grand Hyatt Erawan
Bangkok Hotel as always.

I visited Chulalongkorn University Hospital
in the morning. Chulalongkorn University is the highest-rank
National University in Thailand. The hospital location is
not far from my hotel, and it takes less than 10 minutes
by car. Dr. Wasan Udayachalerm and Dr. Wacin Buddhari are
both my old friends in Thailand. They are working together
in this hospital. They have 2 catheter laboratories, and
one of them was equipped with the newest biplane flat-panel
machine from Siemens 6 months ago. They arranged 5 cases
for me. The 2nd case had a CTO lesion in the proximal LAD,
for which one of them tried to open but failed 2 months
ago. I did PCI for this patient through the femoral approach
by using the bilateral simultaneous coronary angiographies.
Fortunately, I could open the occlusion and placed a Cypher
stent there. Other cases were done from the radial approach.
Dr. Wasan and a young fellow doctor from Vietnam assisted
my procedures. This Vietnamese fellow doctor is very smart.
He can speak English fluently. He could not speak Thai when
he came to Bangkok first time 4 months ago. However, it
is surprising. When Thai doctors speak and ask to this fellow
in English, he replies in Thai now!

After having lunch in Chulalongkorn Hospital
together with them, I moved Ramkhamhaeng Heart Center, where
Dr. Wasan is also working as the director of Heart Center.
This hospital is a big private hospital. The hospital administrators
utilized my visit at this hospital. They called TV stations
and news papers to a conference room and set up an interview
for me and Dr. Wasan. The major topic for this interview
was of course how TRI could reduce the hospital stay and
load on the patients during coronary angioplasties. After
this interview, I moved to the catheter laboratory, where
a Siemens machine was installed. I did 2 cases from the
radial approach there . The head nurse in the laboratory
was the previous chief nurse in Chulalongkorn Hospital.
After she retired from the national hospital 2 years ago,
Dr. Wasan invited her to this hospital.
That day, I had dinner together with several famous Thai
doctors in Interventional Cardiology.

Police General Hospital is just in front
of my hotel. I could clearly see the building from my room.
This was my 3rd visit to this hospital. Thus, many nurses
in the catheter laboratory remember me very well. However,
since there were big changes in doctorsﾕ list in this hospital,
it was my first time to meet with Dr. Bundit Prommetet.
Actually, he is not only a doctor but also a high-rank policeman.
His title is “Colonel”. I did 5 cases there in the morning
through TRI. All of the staffs in the catheter laboratory
were very kind.

After having lunch in the hospital, I moved
to another big private hospital, Saint Louis Hospital, where
Dr. Boonsert Chatlaong is working as a chief Interventional
Cardiologist. We did one case with unprotected left main
disease through TRI. Although the procedure was much more
difficult than I expected, I could finish it successfully
by placing several TAXUS stents. The patient used to work
in a Japanese company. He could speak Japanese. We took
photography together finally.

By taking the return flight departing at
10:15 PM, I came back to Japan in the morning on November
6 Saturday. From Narita Airport, I moved to the city of
Kamakura by car. I could reach at Kamakura City by 8:00
AM.
The day was Saturday. However, since many patients in my
hospital were waiting for my return, I did several PCI cases
in the morning of the day in my hospital.
タイ訪問記-III
湘南鎌倉総合病院 心臓センター循環器科

はじめに
2004年10月29日と30日には日本心血管インターベンション学会関東甲信越地方会および東京ライブデモンストレーションがありました。私は支部長をしていますので、28日から東京に入りました。新潟地方で大きな地震という大災害があったにも関わらず数多くの方々が集まられたので、皆とても喜びました。しかし、何人かの医師、看護師あるいは技師さん達は病院や自宅が地震で損害を受けたため参加することができませんでした。とても気がかりです。
2004年11月2日木曜日、私は午後6時45分東京発のJL707便に乗り、再びバンコクに飛び立ちました。最近は空港の保安検査が厳重です。多くの乗客が時間通りにゲートに着くことができません。他の便と同様にこのJL707便も遅れて出発し、バンコク国際空港に到着したのは翌日の午前0時半になっていました。翌朝の便は早いので、空港から歩いて5分、目の前のAmari空港ホテルに泊まりました。

午前4時45分に起床し、6時30分発のコン・ケーン行きタイ航空TG040に乗るために5時15分には空港に行きました。コン・ケーンはバンコクの北東に位置する学術都市です。そこにはSrinakarint
Kohn Khan Universityという大きな大学があります。この大学医学部付属病院には女王Sirikit心臓病センターが併設されていました。この病院には昨年の8月15日に訪れました。
昨年、この心臓病センターのカテ室は小さく貧相な建物にありました。しかし、数ヶ月前に新しいビルに移転し、そこにはシーメンスの機械一台、フィリップスの機械一台、そしてそのフラット・パネルが一台ありました。カテラボには朝8時前には着きましたが、先生方は未だ見えておりませんでした。私に慢性完全閉塞を含めて8例の症例が準備されていました。初めの予定では、午後8時半の飛行機でバンコクに戻ることになっていましたが、この8例を昼までに全て終了してしまったので、便を変更することができました。カンファレンス室で昼食を皆で食べました。午後2時半の飛行機でバンコクに戻り、いつものようにErawanグラン・ハイアット・ホテルに泊まりました。

朝、チュラロンコン大学病院を訪れました。チュラロンコン大学はタイの国立大学の中の最高とされている大学です。病院はホテルから遠くなく、車で10分もかかりません。Wasan先生とWacin先生は共に私の友人ですが、この病院で一緒に働いています。病院にはカテ室が2つありますが、その中の一室には最新型のシーメンス社製フラット・パネル・バイプレーン装置が半年前に備え付けられました。ここでは5症例準備されていました。2番目の症例はLAD近位部の慢性完全閉塞症例でしたが、2ヶ月前に試みたけれど不成功に終わった症例でした。この症例に対しては経大腿動脈的冠動脈インターベンションで対側造影を併用しながら行いました。幸にも、うまく通し、最終的にCypherステントを植え込みました。他の症例に対しては経橈骨動脈冠動脈インターベンションで行いましたが、Wasan先生とベトナムからの若い研修医が助手をして下さいました。このベトナムの研修医はとても優秀で、4ヶ月前に初めてこの病院に来た時には、タイ語を喋ることができなかったのに、今では驚くべきことに、タイの先生方が英語で話しかけたり、質問したりすると、タイ語で返事するのだそうです。

チュラロンコン病院で昼食を食べた後、ラムカムハン心臓センターに移動しました。Wasan先生はその病院の循環器部長も兼任されています。病院はとても大きな民間病院です。病院上層部は私の訪問を利用しました。彼らはテレビ局や新聞社を呼んでいました。そして、私とWasan先生に対するインタビューを開催したのです。インタビューの主要な点はもちろん、経橈骨動脈冠動脈インターベンションを行うことによって、如何に入院期間を短縮することができ、しかも患者さんに対して経皮的冠動脈インターベンションに伴う負荷を軽減することができるか、といすものです。インタビューを終わってからカテ室に行きましたが、そこにはシーメンスの機械がありました。ここでは橈骨動脈を用いて2例の症例を行いました。カテ室の師長さんは以前チュラロンコン病院で師長さんをしていました。国立病院を2年前に退職したあと、Wasan先生がこの病院に呼んだのだそうです。
この日は、タイの有名な先生方と夕食を食べました。

警察総合病院はホテルの目の前にあります。部屋からもはっきりと建物が見えます。この病院を訪問するのは3度目でした。従って、カテ室の看護婦さんの多くが私のことを知っていました。でも、この病院では医師の大きな移動があったため、Bundit先生にお会いするのはこれが初めてでした。実際には、Bundit先生は単なる医師ではなく、役職の高いレベル警察官なのです。彼の肩書きは“大将”です。私はここで午前中に5例のTRIを行いました。カテ室のスタッフは皆、とても親切でした。

病院で昼食を食べた後、別の民間病院、聖ルイス病院(聖類思醫院)に移動しました。ここのチーフはBoonsert先生です。我々は非保護左主幹部病変の患者さんに対してTRIを行いました。私が最初に想像していたよりもはるかに困難でしたが、最終的にTAXUSステントを植え込んで終了しました。この患者さんは日本企業で働いていたことがあり、日本語を話すことができました。最後に一緒に写真を撮りました。

夜の10時15分発の便で日本に帰りました。そして着いたのは6日の早朝でした。私は成田空港から車で鎌倉に移動し、鎌倉市には朝8時には着くことができました。
その日は土曜日でしたが、私の病院の患者さんたちが待っておられたので、湘南鎌倉総合病院で朝の内に数例の経皮的冠動脈インターベンションを行いました。 |
