Introduction

After finishing my busy schedule during the 11th Kamakura Live Demonstration Course, which was held on December 10, 11 and 12, I was not free. I had to work very hard during the next week to do angioplasty and outpatient clinic. On December 16 and 17, I flied to Naha City of Okinawa Island to visit Tomishiro Central Hospital to do PCI. This visit was the 1st visit to Okinawa area after 4 to 5 years ago. Before that time I have been there frequently at least 3 times a year, since Dr. Sunao NAKAMURA was working in the hospitals in Okinawa Island and invited me frequently.
On December 19, Sunday, we had the 1st core-organizers' meeting for CCT 2005 (Complex Cardiovascular Therapeutics) meeting in a Chinese restaurant within “MARU” building in front of Tokyo Central Station. Drs. Hideo TAMAI, Osamu KATO, Nobuyuki SAKAI, Kaoru OKISHIGE, Ikuhide OKAWA, Atsushi INADA and me joined the meeting. Immediately after finishing the meeting, I took a Narita Express Train from Tokyo Station to Narita International Airport, and then I took an ANA flight for Beijing (NH955) departing at 17:30.
The airplane arrived at Beijing Capital Airport around 20:30. It was very cold in Beijing. The temperature on that day was around minus 5 - 8 degree Celsius. I directly went to the hotel Saint Regis, which is one of my favorite hotels in Beijing. At the lobby cafeteria, a Chinese doctor was waiting for me with his laptop computer to do case discussion. He was a doctor from Union Hospital. He showed me several cases by using CDs and his laptop computer. I rejected to do PCI for 2 cases among them, since I judged there were no clinical indications for PCI in those patients.
Since I was very busy during the preceding several weeks and very much tired, I went to my room after finishing the case discussion.

December 20 Monday, 2004

In the morning, I visited Union Hospital. Union Hospital is one of the oldest hospitals in Beijing. Union means “the co-operation between China and the United States of America”. The main hospital is located in the center of Beijing City and just close to the Wang-Fu-Chin Street, which is the most delicious and crowded street in China. Thus, many high-class people are admitted in the hospital. However, the X-ray machine in the main hospital is a very old one. Union Hospital purchased one of the Beijing Capital Government hospitals 1 year before. Now, it is called as West Hospital of Union Hospital. Because the X-ray machine is this hospital is not too old or bad, all of the patients in the main Union Hospital of candidates for angioplasty are transferred to this West Hospital. I visited this West Hospital. This was my 1st visit at Union Hospital. The building is a clean one. All of the staffs in the catheter laboratories are young and quite aggressive.They are doing a lot of cases through the radial approach. The radial puncture is done while the operator is sitting beside the patient's arm. I did 6 cases of TRI there. Although they had prepared more patients, I left the catheter laboratory by 1:00 PM, since I had to move to another hospital in the afternoon.

I visited PLA General Hospital in the afternoon, where the past leader of China Deng Xiaoping was admitted and finally expired there. The hospital building is huge and gorgeous. A monument with the sentence written by the current President of China Jiang Zemin is displayed in front of the main entry gate . The meaning of the sentence is “Build the highest-class PLA General Hospital”. PLA means Chinese People's Liberation Army, which is the regular Army in China. At the top of the main building, I could see another signboard written by the President Jian, which indicated “Chinese People's Liberation Army General Hospital”. I visited this hospital 5 years ago. This visit was my 2nd visit.
They have 3 catheter laboratories and all machines are Philips digital. One of them is specific for the carotid and abdominal angiographies. Interventional Cardiology group is mainly using a Philips single plane machine. Professor Gai Luyue is the chief of the group. I left the hospital at 6:00 PM after finishing several cases including 2 left main bifurcation lesions. All of the staffs were very kind.

I left the hotel at 7:15 AM for the airport to take CA1321 flight for Guangzhou. On the way to the airport, we dropped at the very local breakfast restaurant near the airport. The menu in the restaurant was quite simple. They have only hot and spicy clear soup and big meat dumpling. They are incredibly cheap but very delicious especially in the cold morning in December.
The airplane did not depart on time. I had to wait for the departure for more than 1 and half hours in the airport. Finally, I arrived at Guangzhou International Airport around noon. Guangzhou International Airport was moved from the center of the city and newly build about 1 year before. The new airport is beautiful and named “Baiyun Airport”, which means “White Clouds”.

I directly moved to ZhuJiang Hospital. This hospital used to belong to the 1st Military Medical University. Chinese Government has been working to restructure the Government organizations. Even Military Medical University is not exceptional from this Government policy. There were 4 Military Medical Universities in China. The 1st one was in Guangzhou, the 2nd in Shanghai, the 3rd in Chongqing, and the 4th in Xian. Honestly speaking, I am one of the Honorary Professors in the 1st and 4th Military Medical Universities. Now, I guess that ZhuJiang Hospital is now independent from the Military University. I remember that I visited this hospital almost 10 years ago with Professor Jia GuoRen from the 4th Military Medical University Xijing Hospital in Xian, when Dr. Liu YingFeng had moved from Xijing Hospital to this hospital several months before. When I first visited Xijing Hospital more than 10 years ago, the X-ray machine was very old and difficult to see the anatomy under the fluoroscopy. Professor Jia arranged many cases including left main cases for me in this 1st visit at Xijing Hospital. You have to understand that our weapon during PCI was only balloon catheters but not including any atherectomy devices or coronary stents. How it was dangerous to do POBA for those serious cases. Because I was not so brave at that time to reject the cases proposed by Chinese doctors, I accepted all of the cases proposed to me. I was very lucky to do all of the cases successfully.

When I visited ZhuJiang Hospital together with Professor Jia GuoRen, the X-ray machine was very bad. I rejected those dangerous cases. Dr. Liu blamed me. “Why you do not to do those cases? When you visited Xijing Hospital, you did very dangerous cases, while X-ray machine was very bad”. I answered to him, “It was 2 years back. Now, those dangerous procedures are now allowed.”
This time, several cases with CTO lesions, for which they had tried to open but failed, were prepared for me. I did several cases. Half of cases were successful, but the remaining was not successful.
After finishing these cases, I checked in the hotel DongFang Guest Hotel, where the TRI meeting was held. Immediately after putting the luggage in my room, I quickly moved to the Chinese restaurant near the hotel. We had dinner together with Dr. Chen Jiyan from GuangDong Provincial People's Hospital, Dr. Wu Chiung-Jen from Chung Gung Memorial Hospital, Kaohsiung, Taiwan, Dr. Liu YingFeng from ZhuJiang Hospital. Dr. Wu flied from Kaohsiung to GuangZhou via Hong Kong.

I came to GuangZhou to attend the TRI Workshop, which was held by the GuangDong Society of Interventional Cardiology and actually the 1st live demonstration course in GuangDong Province. My roles during the meeting included the operator of live demonstration in the morning and the lecturer for the basics of TRI in the early afternoon. I directly went to the ZhongShang University 1st affiliated Hospital in the morning, where Dr. Li Yi and Dr. Du ZhiMin were waiting for me. I arrived at the catheter laboratory by 8:00 AM. Everybody in the laboratory looked being strained to have the 1st live case transmission from the laboratory. The X-ray machine was a Philips old digital machine, which was the same machine I used when I first visited this laboratory 7 or 8 years ago. Because the transmission started from 9:30 AM, we had to wait for it. During the live case transmission, I did 5 cases through the right radial approach. After finishing the transmission successfully, we took a memorial photo together.
GuangZhou has still a taste of the old calm and beautiful city everywhere in the city. The backyard of the ZhongShang Hospital is typical for the taste. Between the hospital and the meeting venue, the transmission was sent by using an optical fiber network run by the China Telecom. The transmission was managed by a Japanese company Shoei without any remarkable troubles.

I hurried up to the meeting venue to do the lecture about the basics of TRI. Professor Chen JiYan was chairing the session. He is the president of the GuangDong Society of Interventional Cardiology. I did my lecture by using my new laptop computer, Panasonic's Let's Note CF-Y2. After finishing my lecture, I had to hurry to the airport to take a flight to Beijing departing at 15:00. Dr. Li Yi kindly came to me to say good bye.

Although the temperature in GuangZhou was above 20 degree Celsius, it was heavily snowing and the temperature was below minus 5 degree in Beijing. Because of the heavy snowing, the arrival was delayed by 1 hour. The Beijing Capital International Airport was all around covered by heavy snowing.
I went directly from the airport to a Chinese Seafood Restaurant “YunLong” Seafood Restaurant to have dinner with my friend Dr. Jia SanQing. We had good Shanghai Crabs. I was surprised to see the sign mark on the front armor of the crab, which is indicating the crab to be a genuine Shanghai crab. I heard that the sign mark is made by using a fine laser beam. I was very happy to see Dr. Jia. He kindly told my fortune by looking at my palm creases.

The outside was snowing again. I left the hotel at 7:15 AM and moved to the FuWai Cardiovascular Center. On the way to FuWai Hospital, I passed in front of Beijing Hotel and TianAn Men Square. Those areas are covered by snow. The backyard of FuWai Hospital was also covered by snow. I guess this hospital is now ranked within the top 5 biggest Cardiology Institutions in Asia. While I was doing cases this time in FuWai Hospital, I did the 2,600th PCI case of the year of 2004 in that hospital. That means more than 2,600 cases of PCI are done within one year in that hospital. Also, the surgical department has done more than 1,400 cases of coronary bypass surgery in the year of 2004. These numbers are amazing, since they had only less than 100 cases of PCI a year in 1991 when I visited first time. When I arrived at the catheter laboratory at 8:00 AM, there was already Professor Gao RunLin. In the laboratory, they have 4 X-ray machines: One is a biplane digitalized Philips BH-3000 machine, which is mainly for Electrophysiology, one single-plane Philips flat panel machine, 2 Siemens single-plane flat panel machines. They have more than 20 cases of PCI every Thursday. I did 5 cases of them. One of those cases had a CTO lesion in the proximal LAD. I tried but failed to open it. The patient will be sent to the surgery. Transradial approach is now quite popular in FuWai Hospital. Almost half of cases are now done through the radial approach. I discussed with Professor Gao as well as my friend Xubo about the coming CIT (China Interventional Therapeutics) 2005 meeting. I am sure that CIT meeting will be one of the biggest meetings for Interventional Cardiology in Asia. I had to move to ChaoYang Red Cross Hospital around 1:00 PM after having simple lunch.

In ChaoYang Red Cross Hospital, they were waiting for my arrival. They prepared 9 cases of PCI for me. I did all of these cases definitely only through the right radial approach. All of the cases were successful. I and ChaoYang Hospital have a long history of friendship. Thus, all of the staffs are very familiar for me including the chief catheter laboratory nurse as well as Dr. Wang LeFeng. From 7:00 PM on the night, all staffs of the catheter laboratory, fellows and doctors from ChaoYang Hospital had a Christmas party in the North Korean Restaurant near the hospital. It was a real North Korean Restaurant run by the North Korean Government. I joined the party. Beautiful ladies from North Korea showed songs and dancing.

After checking out the hotel, I visited again FuWai Hospital at 8:00 AM. They had many cases. I did several cases including CTO lesions. Around 11:30 AM, I left the hospital for the airport. On the way to the airport, I dropped in a local SiChuan restaurant in NiuRenJie Street. NiuRenJie means “Ladies' Streets”, where we can visit many boutiques for ladies as well as restaurants. Sauteed Hot Cabbage is one of my favorite foods in this restaurant. Even if you have too much food and drinking with 2 or 3 friends together, you will never spend 20 US dollars. I came back to Japan by taking a NH906 flight to Tokyo departing at 14:50.

はじめに

12月10, 11, 12日と開催した第11回鎌倉ライブデモンストレーションのためとても忙しかったのですが、それが終わった後も、PCIや外来診察のためにずっと忙しい状態が続きました。12月16, 17日には、沖縄県那覇市にある豊見城中央病院にPCIを行うために訪れました。この沖縄訪問は、それこそ4,5年ぶりの訪問でした。昔は、中村 淳先生が沖縄で働いていて、良く私を招いてくれましたので、年に少なくとも3回は沖縄に行っていました。
12月19日日曜日には、CCT 2005のための最初の打ち合わせが東京駅前の丸ビルの中にある中華料理レストランで行われました。玉井 秀男先生、加藤 修先生、坂井 信幸先生、沖重先生、大川先生、稲田先生と私がこの会に参加しました。打ち合わせが終わってすぐに成田エクスプレスに乗り、東京から成田空港に向かいました。そして、17:30発の全日空便NH955に乗って北京に向かいました。
飛行機は北京空港に20:30に到着しました。北京は非常に寒く、気温は氷点下5から8度でした。そのまま大好きなセント・レジス・ホテルに直行しました。ホテル・ロビーの喫茶店では、ラップトップ・コンピューターを持ったお医者さんが私を待っていました。彼は協和病院のお医者さんで、CDを使って何例か私に症例紹介をしました。私は、その中の2例に対してPCIを行うことを拒否しました。その理由は、PCIの適応が無い(必要が無い)、と判断したからです。ここ数週間とても忙しく、とても疲れていましたので、この症例検討を終えてすぐに部屋に行き寝ました。

私は午前中、協和病院を訪れました。協和病院は北京で最も古い病院の一つです。協和というのは、「米国と中国共同の」という意味です。本院は北京市の中心にあり、中国最大の繁華街である王府井通りの近くにあります。そんな訳で、この病院には、お偉いさんが入院します。しかし、本院のレントゲン機械はとても古いものです。1年前協和病院は北京首都政府から病院を一つ買収しました。その病院が現在、協和病院西分院と呼ばれるものです。この病院のレントゲン機械はそれ程古いものではなく、悪くもないので、本院にいるPCI適応患者さんは皆、この分院に転送されます。私は、この分院を訪れました。これが協和病院を訪れた最初でした。建物は奇麗です。カテ室スタッフは皆若く、とても積極的です。彼らは経橈骨動脈的冠動脈インターベンションをたくさんやっています。橈骨動脈穿刺は座って行います。この病院ではTRIを6例しました。もっとたくさんの患者さんがいましたが、午後は他の病院に行かねばなりませんでしたので、午後1時までにはカテ室を去らねばなりませんでした。

午後は人民解放軍総合病院を訪れました。この病院は、トウ小平が入院し、最後に息を引き取った所です。病院は巨大で、しかも豪華です。江沢民主席直筆の文章が書かれたモニュメントが入り口の前にあります。その文章というのは、「人民解放軍総合病院を立派な病院にしよう」といものです。病院の上にはこれも江沢民が書いた、「人民解放軍総合病院」の文字が掲げられています。私はこの病院には5年前に一度訪問したことがあり、今回は2回目でした。
病院にはカテ室が3つあり、全部がPhilips社製でした。この内の一つは、脳血管や腹部血管造影専用でした。PCIは主として、Philipsのシングル・プレーン・マシンを使っています。Gai Luyue先生がグループのトップです。2例の左主幹部病変を含めて数例のPCIを行いました。皆とても親切でした。

広州行きのCA1321便に乗るためにホテルを7:15 AMに出ました。空港に向かう途中で、空港近くの庶民的な店に寄り、朝食をとりました。この店のメニューはとても簡単で、辛いスープと、肉饅頭しかありません。値段はとても安いのですが、12月の寒い朝にはとてもおいしく感じます。
飛行機は定刻通りには出発しませんでした。空港で1時間半以上待つことになりました。結局、広州の空港に着いたのは昼頃になっていました。広州国際空港は1年ぐらい前に待ちの中心から移転し、新しく建設されました。新しい空港は奇麗であり、名前は「白雲空港」と言います。

私はそのまま朱江病院に向かいました。この病院はかつて第一軍医大学に所属していました。中国政府は政府組織のリストラを行っていて、この方針は軍医大学にも適用されます。中国には4つの軍医大学があり、第一は広州、第二は上海、第三は重慶、そして第四が西安にあります。正直に申し上げますと、私は第一と第四軍医大学の名誉教授です。現在は、朱江病院は軍医大学から独立しているようです。西安にある、第四軍医大学西京病院のJia先生と共に、この病院に10年ぐらい前に訪問したことがあります。丁度その数ヶ月前にLiu先生が西京病院から移動したところでした。10年以上前に西京病院を最初に訪問した時には、レントゲン機械は非常に古く、透視も全く見えませんでした。Jia先生はたくさんの症例を私に用意していました。その中には左主幹部病変もありました。当時は、ステントも方向性冠動脈粥腫切除術もありませんでした。全てをバルーンのみで治療せねばならなかったのです。このような症例に対してバルーンだけ、というのは非常に危険です。当時、私は用意された症例を断る程には勇敢でありませんでしたので、これらの症例の全てを行いました。幸にも全例うまくいきました。

私が、Jia先生と共に朱江病院を訪問した時、レントゲン機械は非常に悪いものでした。私は危険な症例を行うことを拒否しました。Liu先生は、「どうしてやらない。西京病院の時には、危険な症例も悪いレントゲン機械で行ったのに」と私を責めました。私は、彼に「それは、2年前のことです。今は、そのような危険な手技は許されません」と、答えました。
今回、彼らはPCIを試みたが不成功に終わったCTOの症例何例か用意していました。半分の症例では成功しましたが、半分では不成功に終わりました。
症例を終えてからホテル「東方賓館」にチェック・インしました。そこがTRIの会の会場です。部屋に荷物を入れすぐに、近くの中華料理の店に行きました。そして、広東人民病院のChen先生、台湾高雄の長庚記念病院のWu先生、朱江病院のLiu先生と共に夕食をとりました。Wu先生は、香港経由で広州に入られました。

広州に来たのは、TRIワークショップに参加するためです。この会は、広東インターベンション学会が主催したもので、広東省で初めてのライブデモンストレーションでした。私は、午前中にライブデモンストレーションの術者、午後一番で講演というスケジュールでした。朝、中山大学付属第一病院に行きました。Li先生と、Du先生の二人が私を待っていました。私は8時に着きましたが、カテ室の皆は初めてのライブデモンストレーションとあってとても緊張していました。レントゲン機械は古いPhilipsのデジタルでしたが、7,8年前にこの病院を訪問した時と同じ機械でした。中継は9:30から始まりましたので、待たねばなりませんでした。ライブデモンストレーション中継の間に、私は右橈骨動脈アプローチにより5例の症例を行いました。ライブデモンストレーション放映を成功裏に終えてから、記念写真を皆と撮りました。
広州には街中にまだ、古く静かで美しい雰囲気の場所があります。中山病院の裏庭もそのような場所の一つです。病院と会場の間の中継は、China Telecomの所有する光ファイバーを用いて行われました。この中継は日本のショーエイが行いましたが、特にトラブル無く行われました。

TRIの基礎についての講演をするために会場に急ぎました。Chen先生が座長をされていましたが、彼が広東インターベンション学会の会長です。私は、自分の新しいラップトップPanasonic Let's Note CF-Y2を用いて講演をしました。講演を終えてすぐに15:00発の北京行きの飛行機に乗るために空港に急がねばなりませんでした。Li先生がさよならを言うために来られました。

広州の気温は20度以上ありましたが、北京は氷点下5度以下で、雪が降っていました。雪がひどかったので、到着が1時間遅れました。北京首都国際空港はすっかり雪に覆われていました。直接、中華海鮮料理の「雲龍海鮮」に向かいました。そこで友人のJia先生と夕食をとるためです。おいしい上海蟹がありました。何と甲羅に、本当の上海蟹という印がつけてありました。何でもレーザーで彫るのだそうです。Jia先生にお会いしてとても嬉しく思いました。Jia先生は私の手相を見てくださいました。

外は雪が降っていました。阜外病院にいくために7:15 AMにホテルを出ました。阜外病院に行く途中、北京ホテルの前や天安門広場を通過しましたが、雪で覆われていました。阜外病院の裏庭も雪で覆われていました。阜外病院はアジアで5指に入る心臓病センターとなっていると思います。今回私がPCIを行った症例の中には今年の2,600例目のPCIというのがありました。ですから、2004年一年間に阜外病院では2,600例以上のPCIが行われたことになります。また、冠動脈バイパス手術も1,400例以上行われたそうです。私が1991年に最初に訪れた時には、年間PCIが100例にも満たなかったことを考えればとても驚くべき数字です。カテ室に8:00 AMに着いた時には、Gao先生は既にカテ室におられました。カテ室には4つのレントゲン機械があり、電気生理専用のPhilips BH-3000 1台、Philipsのフラット・パネル1台、Siemensのフラット・パネル2台です。毎週木曜日には20例以上のPCIが行われます。その中の5例のPCIを行いました。1例はLAD近位部のCTO症例でありましたが、不成功に終わりました。患者さんはバイパス手術に回るでしょう。阜外病院でもTRIは盛んです。現在では半数の症例がTRIで行われています。来るべきCIT 2005学会の打ち合わせをGao先生とXuboと一緒に行いました。間違いなくこの学会はアジア最大のものの一つとなるでしょう。簡単な昼食をとった後1:00 PM、朝陽赤十字病院に移動しました。

朝陽赤十字病院では私の到着を待っていました。9例のPCIが用意されていました。私は、その全例を右橈骨動脈アプローチで行い、すべてうまく行きました。私はこの朝陽赤十字病院に長い間関わってきました。ですから、スタッフの皆を良く知っていますが、この中にはカテ室のチーフ・ナースもWang先生もいます。夜の7時からは、朝陽病院近くの北朝鮮レストランでクリスマス・パーティーが行われました。このレストランは北朝鮮政府が開いている本物です。北朝鮮の綺麗な女性が唄を歌い、また踊りを披露しました。

ホテルをチェック・アウトしてから8:00 AMに阜外病院を再び訪問しました。たくさんの症例がありました。私はCTOを含めて数例行いました。11:30 AM頃に病院を発ち、空港に向かいました。空港の途中で、「女人街」にある四川料理の店に入りました。「女人街」には女性のためのブティックやレストランがあります。このレストランの辛いキャベツ炒めが私の好物です。ここでは、数名の友人と一緒に、飲み食い放題しても、20ドルにもなりません。14:50発のNH906便に乗って日本に帰国しました。
