Introduction - Back to Madrid

Spain is one of the countries, where transradial coronary intervention is popular. Professor Javier Goicolea is the leader in TRI in Spain. He has a maniac conference specified for transradial approach for past 3 years together with Professor Macaya. The 1st meeting was held 2 years ago. Dr Kiemeneij was invited to that meeting. The total number of attendants 2 years ago was only 20 people. Last year, the number increased to 50 people. However, it reached almost 300 people this year.

I flied from Tokyo to Madrid via Paris. The airplane arrived at the Madrid International Airport around 5:30 PM. I moved to the hotel Tryp Monte Real Madrid, which is located in the residential area of Madrid. The meeting was started from the evening of the day, and was held in an auditorium within the hotel. The meeting was commenced from 18:30.

The main topics of the day were so called debate sessions between transradial and transfemoral schools. Because the traffic to Madrid from the airport was so bad, I could reach the hotel around 8:00 PM. I did not join the meeting and hurried to the most famous seafood restaurant “Zalcain” , where we can have one of the most delicious seafood in Spain, Anguras.

Anguras is a dish of baby eel saute by olive oil with garlic on a hot pot. After enjoying Anguras and before coming back to the hotel, I visited a famous street called “Meson Street”, where are many traditional pubs. This area used to be a prison. There are many pubs including a pub famous for Spanish omelets and old prison. I visited one of them. It was Meson de la Guitarra .

I drank popular alcohol drink “Sangria” together with ham. Several visitors were playing guitars and singing songs. It was really nice.

Without having a breakfast, I moved to Hospital Clinico San Carlos, where I was supposed to do a live case. This hospital has the biggest number of cases of PCI in Spain. The chief director in Cardiology is the famous Professor Macaya. This hospital is a kind of Government University hospitals. There is an institute specific for Cardiology within the hospital. When I arrived at the hospital around 7:30 AM, it was still dark. The live cases were transmitted to the meeting venue via satellite transmission.

Professor Macaya arranged me a very difficult case. The patient was 50 something years old male patient with a big built. He used to be a famous football player. He had a previous stent implantation in the middle LAD, which was occluded several years ago. Several attempts were made to open this in-stent and before-stent occlusion but failed. The last angiography was done 1 year before. I was asked to open it. I bravely tried to open it through the left radial artery, since his wrist had no arterial pulsation because of his previous accidental injuries to his right hand. I used several kinds of stiff guidewires but failed to cross the lesion. I quit the case after 1 and half hours' of attempts.

The meeting venue of the day was a big auditorium within Hospital Universitario Puerta de Hierro, where the chairperson of this live demonstration, Professor Goicolea is working. I moved to the auditorium and did one lecture about PCI for Chronic Total Occlusion.

After finishing my lecture, I got free and went to another famous restaurant Asador Donostiarra. The restaurant is so famous that many famous football players playing in a football club “Real Madrid” come. The famous dish in the restaurant is beef roasted in a hot plate. The raw ham in the restaurant was so nice. In Spanish, ham is called “Jamon”.

From 8:00 PM of the day, I was invited from Professors Goicolea and Macaya to the dinner together with Professors Ives Louvard from Massy, France and Gerald Barbeau from Quebec City, Canada. The restaurant was surprisingly same one with that I visited the day before. It was Zalcain. Professor Macaya is so famous in Spain and ordered very nice red wine bottles. Its name is ALION. We had a very pleasant dinner together.

By leaving the dinner earlier, I went to one of the famous tablaos in Madrid “Torres de Bermejas”, which means a Red Tower. A piano music “Red Tower” is one of masterpieces composed by a famous Spanish composer, Isac Albeniz. The music was transformed to guitar music. I used to exercise hard to play this music by my guitar in my high school period .

I stayed long in Torres de Bermejas later than 1:00 AM. There was one beautiful lady in the small number of audiences at that time. I talked to her. She was from Argentina by herself. She really looked to enjoy Flamenco music. After saying “Good Night” to her, I moved to the Meson Street again, where still many people were walking around. I stopped in a popular Tablao “Tablao Arco de Cuchilleros”. This Tablao is not an expensive one. Many local young people were drinking and enjoying Flamenco music. One of the Flamenco dancers was very beautiful. After those professional dancers finished dancing, some of visitors came up to the stage and danced by themselves. It was a wonderful night. I came back to the hotel almost around 3:00 AM and found very nice gifts sent by Professor Goicolea. They were the packages of DVD and CD. Professor Goicolea remembered that I spent part of my high school days to learn playing Spanish guitar, and he gave me an instruction DVD for playing Flamenco guitar. I was really impressed.

After checking out the hotel around 10:30 AM, a car arranged by the convention took me to the airport. I came back to Japan via Paris and arrived at Narita airport around 1:00 PM on February 13 Sunday, and hurried to the Tokyo International Forum, where we had a starting meeting for the coming CCT 2005 meeting from 3:00 PM.
This visit in Madrid was very short one and spending just 2 nights. However, it was very fruitful for me. I really enjoyed it. Thank you for every body.

はじめに - またまたマドリッド

スペインはTRIの盛んな国の一つです。Goicolea教授はスペインでのTRIのリーダーです。彼はMacaya教授と共に、TRIのライブデモンストレーションを過去3年に渡り開催してきました。最初の会は2年前に開催され、Kiemeneij先生が招かれました。その時の参加者はたった20名だったそうです。そして、昨年の会では参加者は50名となり、今年は何と300名に達しました。

私はパリ経由でマドリッドに入りました。マドリッド国際空港に到着したのは午後5:時30分頃でした。それから住宅街の中にあるホテル、Hotel Tryp Monte Real Madrid にまっすぐむかいました。ここは当日の会場も兼ねていて、会はその日の夕方6時30分から始まりました。

その日の主な内容は、TRIとTFIのディベートのようなものでした。空港からマドリッドまでの交通渋滞のためにホテルに着いたのは午後8時頃になっていました。私自身は会に参加せずに、有名なシーフード・レストランであるZalcainに向かいました。ここではスペイン料理の中でもおいしいAngurasを食べることが出来ます。

Angurusというのはうなぎの稚魚をにんにくとオリーブ油で熱い鍋で炒めたものです。Angurasを食べ終え、ホテルに戻る前に私は有名な居酒屋街であるメゾン・ストリートを訪れました。この一帯は昔監獄でした。そこにはたくさんの有名な居酒屋があり、スペイン・オムレツで有名なトルティーヤさんや、昔の監獄そのものの洞窟居酒屋などがあります。

私はお気に入りのギター居酒屋に入り、生ハムをおつまみにしておなじみのサングリアを飲みました。 何人かのお客さんはギターを弾き、唄を唄っていました。とても素敵な夜でした。

朝食を摂らずにライブデモンストレーション症例を行うサン・カルロス病院に向かいました。この病院はスペイン最大の症例数を誇る病院であり、有名なMacaya教授がおられます。病院は国立大学病院であり、中には心臓病研究所があります。病院に到着したのは朝の7:30ぐらいでしたが、まだ夜は明けていませんでした。ライブデモンストレーション症例は会場まで衛星を用いて伝送されていました。

Macaya教授は私にとても難しい症例を準備していました。患者さんは50歳台の体の大きな男の人で、昔は有名なサッカー選手だったそうです。以前LADの中ほどの病変にステントが植え込まれましたが、数年後にはそこが詰まってしまいました。数回にわたってそこに対するPCIが試みられましたが、何れも失敗に終わっていました。最後の冠動脈造影は1年前のものでした。これに対して、TRIで行うことにしましたが、右の橈骨動脈拍動は何らかの外傷のために触れませんでしたので、左橈骨動脈より行うことにしました。何本かのガイドワイヤーを試しましたが、病変を通過できませんでしたので、一時間半の後に諦めました。

この日の学会会場は会を主催されているGoicolea教授が働くPuerta de Hierro大学病院の大きな講堂でした。私は、その講堂に移動して、CTOに対するPCIの講演を行いました。

講演を終えて自由になったので、有名なレストランAsador Donostiarraに昼食を摂りに行きました。このレストランはとても有名で、レアル・マドリッドのサッカー選手などが訪れます。ここでの有名なメニューは牛肉の皿焼きです。ここでの生ハムもとても良かったです。

この日の夜8時からは、Goicolea教授とMacaya教授にLouvard先生と、Barbeau先生と共に夕食に招かれました。レストランは昨夜と同じZalcainでした。Macaya教授はとてもすばらしい赤ワインを注文されました。その名はALIONというものでした。とても楽しい夕食を過ごしました。

早めに夕食会を切り上げて、マドリッドで有名なタブラオ“Torres de Bermejas”に行きました。この名前の意味は「朱色の塔」というものであり、有名なアルベニスのピアノ曲スペイン舞曲の中にもあります。この曲はギターにも書き直されていて、高校生の頃、一生懸命練習した記憶があります。

ここには午前1時過ぎまでいました。その頃は聴衆も少なくなっていましたが、綺麗な女性を見つけました。彼女に話しかけると、彼女はアルゼンチンから来たそうです。とてもフラメンコを楽しんでいました。彼女にさよならを告げてから、再び居酒屋街に戻りました。まだ、たくさんの人々がいました。この中の大衆的なタブラオである、“Tablao Arco de Cuchilleros”に入りました。このタブラオは値段が安いのです。たくさんのマドリッドの若者が楽しんでいました。フラメンコ・ダンサーの一人はとても綺麗な人でした。ダンサー達が踊りを終えた後、お客さん達がステージに昇りダンスを楽しんでいました。とても素敵な夜てした。ホテルに戻ったのはかれこれ午前3時頃でした。部屋に戻ると、Goicolea教授からの素晴らしいギフトを見つけました。それはDVDとCDのパッケージでした。Goicolea教授は私が高校時代にギターを練習していたことを覚えておられました。そして、フラメンコの教則DVDを下さったのです。

ホテルを午前10時30分にチェック・アウトして、学会の用意した車で空港に向かいました。そして、パリ経由で成田空港に翌日の午後1時到着で戻りました。成田から東京国際フォーラムに急ぎました。そこでは今度のCCTのための企業説明会が午後3時から予定されていたのです。
マドリッドの今回の訪問はとても短く2泊のみでしたが、とても充実したものでした。皆さんにありがとう。
