Introduction

I was invited from the 2nd World Congress of Interventional Cardiology held in Mumbai, India from February 24 to 27, 2005. The President of the meeting was Dr. BK Goyal and Executive President Dr Lekha Adik-Pathak. The name of the convention is “World Congress”. However, it is held by those doctors once every year only in Mumbai. Why it is “World Congress”? I asked several people. They answered me, “It is because they invite doctors from US, Europe, Asia as well as Japan.”

While India was British Colony, Mumbai use to be called Bombay. Bombay was the most important port for European countries, through where cotton, tea, spices and so on were transported to those countries. There are a lot of old magnificent colonial buildings, which were built during the colonial period within the city of Mumbai. I have been this beautiful city many times.

I flied from Narita New Tokyo International Airport by JL 717 flight to Bangkok around 0:30 noon of February 23 Wednesday, 2005. It was strongly windy on that day. The departure was delayed by 1 and half hours due to the strong wind. I was supposed to have enough transit time in Bangkok airport. However, because of this incredible delay in departure, I had to hurry up to take the CX 751 flight from Bangkok to Mumbai. This flight departed at 17:30 PM from Bangkok airport, and arrived at Mumbai International Airport around 8:00 PM after 4 hours flight.
The traffic in Mumbai is so bad that I arrived at the hotel later than 10:00 PM. I stayed at the Taj Mahal Palace and Tower Hotel, Mumbai, where the meeting was held. In front of the hotel, we can see a historical monument, Indian Gate. Inside of the hotel building is also impressive and beautiful. After having a bottle of Indian Beer, Kingfisher, I went to my bed, since it was already around 3:00 AM in Japan Standard Time.

I met my friend, Dr. Tejas Patel in the cafeteria within the hotel, and discussed something. Since the opening ceremony of the meeting was scheduled to commence from 11:30 AM, I came down to the venue by 11:15 AM. However, there was nobody in the auditorium.

The inauguration ceremony started before noon. I and Dr. Almagor from Israel were invited to the ceremony. After this ceremony, I was invited to do an inaugural lecture. The title of the lecture was “Angioplasty for Chronic Total Occlusion”.

In the afternoon, I had another lecture on transradial coronary intervention. While spending time within the hotel, I found an interesting signboard. On the signboard, I could read Japanese characters clearly. That was the sign for a Japanese restaurant within the hotel. I went to the restaurant to have dinner. It was the first and unforgettable experience to have Sushi there.

February 25 Friday, 2005

I waked up early and went to Bombay Hospital to do live cases. The X-ray machine there were Siemens machines. The 1st case for me had unstable angina with tight narrowings in mid RCA and non-protected left main coronary artery. He also had a chronic total occlusion at the ostium of LCX. He had not so enough money. Thus, I was requested to use bare metal stents. I did this case through the right radial approach and finished by placing 2 stents in RCA and LMT.

The next case had a chronic total occlusion in the proximal RCA. Several weeks ago, some Indian doctors touched the lesion but failed. I started from the femoral approach and did simultaneous bilateral coronary angiographies. However, although I did simultaneous bilateral angiographies, I could not see any artery distal to the occlusion, which was clearly seen in the previous angiograms. I guessed that the previous doctor totally destroyed the RCA. Thus, I quitted the procedure.

The telecast was done through a satellite transmission. TV staffs worked nicely. After finishing my procedure, I talked with Dr. Gary Mintz. He was complaining that the IVUS machine in the hospital was too old and not well maintained.

By leaving the hospital at 11:00 AM, I hurried up to the airport and took an Indian Airline flight AI 306 departing at 15:20 PM for Bangkok via Delhi airport. I arrived at Bangkok airport at 0:30 AM and changed the flight to a Thai Airline TG 648 for Fukuoka. Finally, I reached Fukuoka Airport at 8:00 AM of February 26 Saturday, and went to Kurashiki to join the Kurashiki Live Demonstration Course.

はじめに

2005年2月24日から27日までインドのムンバイで開催された第2回世界インターベンション学会から招聘されました。この学会の会長はGoyal先生で、執行会長がDr Lekha Adik-Pathak でした。学会は「世界」と銘打っていますが、この先生方が2年に1回ムンバイで開催しているのです。どうして「世界」と言うのでしょう。皆の答えは、アメリカ、ヨーロッパ、アジアそして日本から医者を招聘しているからだそうです。

インドがイギリスの植民地だった頃、ムンバイはボンベイと呼ばれていました。ボンベイはヨーロッパの国々にとってとても重要な港であり、この港から綿花、紅茶、香辛料等が運び出されました。ムンバイの街中には、その当時建てられた荘厳な植民地時代の建物がたくさんあります。私はこの美しい街に何回も来たことがあります。

2005年2月23日水曜日発のバンコク行きJL 717便で昼の0:30頃に成田を飛び立ちました。その日はとても風の強い日でした。この強い風のために出発は1時間半遅れました。ですから、バンコク空港では乗り継ぎに十分な時間があった筈でしたが、ムンバイ行きのCX 751に乗り継ぐのには急がねばなりませんでした。この便はバンコク空港を17:30にと飛び立ちました。そして、4時間の飛行の後、ムンバイ空港に到着したのは、夜の8時になっていました。
ムンバイの交通渋滞はひどく、ホテルに到着したのは午後10時を回っていました。私は、学会場でもあるTaj Mahalホテルに泊まりました。ホテルの向かいには歴史的建造物であるインド門があります。ホテルは建物の中も荘厳で綺麗です。インド産のKingfisherビールを飲んでからベッドに向かいました。もう日本時刻で翌日の午前3時になっていました。

友人のPatel先生とホテルのカフェで落ちあい、色々と話をしました。学会の開会式が午前11:30から予定されていましたので、11:15には会場に行ったのですが、誰もいませんでした。

開会式典は正午を回ってから始まりました。私とイスラエルのAlmagor先生が式典に呼ばれました。この式典が終わってから私が開会記念講演を行いました。タイトルは「慢性完全閉塞に対するPCI」です。

午後には、経橈骨動脈冠動脈インターベンションについての講演を行いました。ホテルの中で時間をつぶしていると、面白い看板が目につきました。それは、はっきりと日本語がかかれていました。ホテルの中の日本食レストランの看板でした。夕食はここで摂りました。これがインドで初めての寿司を食べた経験です。

早く起き、ボンベイ病院にライブデモンストレーション症例をするために向かいました。レントゲン設備はシーメンスでした。一例目は、不安定狭心症の患者さんで、右冠動脈にきつい狭窄があり、保護されていない左主幹部にもきつい狭窄がありました。そして、LCXは起始部で慢性完全閉塞でした。患者さんはあまりお金を持っていないので、Bare Metal Stentを使うように言われました。この症例に対しては右のTRIで2個のステントを右冠動脈と左主幹部に植え込んで終了しました。

次の症例は右冠動脈近位部の慢性完全閉塞でした。数週間前にインドの先生が行い、不成功に終わりました。経大腿動脈的冠動脈インターベンションで行い、左右同時冠動脈造影を行いました。しかし、同時造影をしても、慢性完全閉塞の末梢が全く映りませんでした。以前のPCIでぐちゃぐちゃになってしまったのだと思います。結局、この症例はキャンセルしました。

中継は衛星を用いて行われました。テレビ・スタッフは良く働きました。手技を終えてから、Mintz先生と話をしました。彼は病院に置いてあるIVUS機械がとても古いし、メンテがされていない、とぶつぶつ言っていました。

病院を午前11時に出発し、空港に急ぎ、15:20発のデリー経由バンコク行きインド航空AI 306便に乗りました。バンコクには午前0:30に到着し、そこでタイ航空TG 648便に乗り換えて福岡空港に到着したのは2月26日土曜日の朝8:00でした。そこから倉敷ライブデモンストレーションに参加するために倉敷に急ぎました。
