Workshop on TRI in Saint Vincent's Catholic Medical Center (SVCMC)

I came back to New York again in order to demonstrate my techniques on transradial coronary intervention. As well known, it is quite difficult for Japanese physicians to conduct medical practice within U.S.A., since there is a strict regulation. Dr. John Coppola kindly did a lot of work to give me a temporal license to demonstrate TRI in SVCMC. However, the process this time might be easier than last year, because I already performed it on March 4 and 5 last year.
St. Vincent Hospital is famous for its “Comprehensive Cardiovascular Center”, where those famous doctors as Dr. Ambrose JA and Rentrop KP used to work. Their names are well known by all of the Interventional Cardiology doctors, Everybody can find a lot of literatures about the field of unstable angina and acute myocardial infarction written by those doctors in PubMed Website. The annual number of coronary angiography cases is about 5,000 including 2,500 cases of PCI in SVCMC. Surgical team is doing CABG in 250 patients every year. Thus, the ratio of PCI to CABG is 10 to 1 in SVCMC. The total bed number of SVCMC is less than 400. Thus, majority of diagnostic coronary and/or ad-hoc PCI patients come back to their home on the same day.

I left Japan by taking a direct flight to John F. Kennedy International Airport of New York by Japan Airline (JL 006) departing at 12:00 noon on March 2 Wednesday, 2005. During 12 and half hours' flight, I could sleep just only for 1 hour. I was continuously working in the airplane by using my Panasonic Laptop computer. About 3 hours before the arrival in New York, a stewardess came to me and asked, “Are you a doctor? We have one sick patient coming from some Asian country via Bangkok and Tokyo. She has burn in her face and upper body. She is complaining of pain. She cannot speak English. Could you kindly consult her?” Of course, it is my great pleasure to see any patient anywhere. I visited the patient's seat and consulted her. She looked scarred and painful. She had burn, which was not yet healed. I suggested her to drink water, injected a syringe of analgesia in her thigh, and then rubbed her back gently to lift the pain. After my consultation, she turned easy.

I arrived at the 5-stars hotel, Ritz Carlton New York Battery Park, which was arranged by Dr. John Coppola at 0:30 PM. I again visited a famous Japanese restaurant “Nobu”, which is one of the most popular places and difficult to reserve tables in New York. When I visited last year, I could not find any vacant tables even in daytime. However, I was very surprised to see a lot of vacant tables this year. I tested the taste of foods. Those raw fishes are flavored with somehow French and Japanese cuisine. It was really tasty. The cycles of popularity for restaurants are very quick in New York.
After getting short rest in my room, I visited SVCMC with Dr. Tejas Patel and meet Dr. Coppola. After small discussion, we went to a fine Indian Cuisine “TOMARIND”. We had Indian dinner together .

Dr. Tak-We Kwan came to the hotel to pick up us at 7:15 AM. He recently purchased a new car made by Porsche. We arrived at SVCMC at 7:30 AM. They had 29 cases of ad-hoc angioplasty on the day. I did one case in the morning. The patient was a male Indian patient. He had previous DES implantations in RCA and LAD. Dr. Bhambhani arranged this case for me, because the patient had a chronic total occlusion in the middle of LCX, which was found several years ago. I did the right radial artery puncture by using a 6-French introducer and took a 6-French XB3.5 guiding catheter from Cordis. I could cross the CTO lesion of 3.0 CM in length by using a Magic-FA guidewire with the assist of a 1.5-mm OTW Maverick balloon. However, the 1.5-mm balloon could not cross the lesion. Dr. Bhambhani assisted me. I removed it and placed a BMW guidewire in the proximal OM branch, which had a 90% lesion as well. By doing an anchor ballooning technique by inflating a 2.0-mm Maverick balloon in the OM branch, I could successfully passed the CTO by a 1.5-mm Rx Maverick balloon. After putting a long TAXUS stent, the distal LCX after CTO looked bigger. I was applauded by everybody there. After my case, Dr. Cezar S. Staniloae kindly introduced me a FoxHollow atherectomy catheter for peripheral arteries. He was bone in Rumania, finished his fellowship of Interventional Cardiology at Montreal Heart Institute in Canada, and moved to New York City. He is one of skillful operators in SVCMC and doing a lot of cases of peripheral angioplasties now. He showed me the angioplasty using a FoxHollow device in one patient with a chronic total occlusion in the right middle anterior tibial artery. The case was successful. It is a quite nice device for peripheral angioplasty. After watching several cases there, I went back to the hotel and took a rest.

We had dinner together in a nice seafood Cuisine “Blue Water Grill”, which is located in front of Union Square. As Appetizer, we ordered Seafood Platter. While we had dinner, Dr. Bhambhani made a telephone call to Dr. SK Sharma, who is also a very famous Interventional Cardiologist working in Mt. Sinai Hospital in Manhattan. He told me, “Dr. Sharma will come.” However, I could not believe it, because I did not think such a famous doctor would join us. But, Dr. Sharma really joined us. It was really a great honor for me to meet Dr. Sharma. I ordered Tuna Tataki as a main dish. It was good in taste but too much for me.

After the dinner, Dr. Bhambhani kindly took me to my hotel by driving his own car. He came to New York from Jaipur, India, which is called “Pink City”. He works hard and earns a lot of money. He is now doing the biggest number of angioplasty in SVCMC. He has his own clinic at Brooklyn area of New York City, from where he brings many patients to the catheter laboratories in SVCMC.

March 4 Friday, 2005

Again Dr. Kwan came to the hotel at 7:15 AM and took us to the hospital. The TRI workshop started from 8:00 AM with the opening speech by Dr. Coppola, which was followed by the diagnostic coronary angiography from the left radial approach for an old lady patient with previous bypass surgery conducted by Dr. Patel. Following this case, I showed 3 patients of TRI. One patient was for ad-hoc PCI for RCA, and the other 2 patients were elective PCI patient for CTO lesions. Again, I had to use an anchoring balloon technique for one patient with a CTO lesion in RCA. I was happy, since all of these cases were successfully finished without any complications. After having to short lunch in the hospital, I left the hospital and visited the office of Cardiology Research Foundation with Dr. Tejas Patel to meet Dr. Martin Leon. Although he was very busy to prepare for the ACC (the American College of Cardiology) meeting, he kindly took time for us. He introduced many things to us. I was amazed to know the annual budget of CRF this year reached 44 million US dollars, which is really compatible with 50 million US dollars for one-year budget for ACC.

After finishing the meeting with Dr. Leon, I had an interview from several analysts in a big security financial company in Wall Street. It was an amazing experience for me that I had some talk with people working in the different areas other than medical field.
Because it was in the Friday evening, the office area in the security financial company was vacant, when we finished the interview at 5:00 PM. One of the analysts in the company took me to a secret place, which has never been introduced in any guide books and kept confidential. It was a very nice Sushi restaurant in the downtown. There was no signboard of the restaurant so that everybody passed without identifying it. That was a real Japanese style high-grade Sushi restaurant. I dare not to disclose the name of the restaurant here in my sentence. After the fine Sushi restaurant, we moved to a real Japanese-style pub “Matsuri”, which was crowded by many New York people.

March 5 Saturday, 2005

It was clear in the morning. I could see the Stature of Liberty clearly from the window of my room . Although I could see a monument of the Japanese-style big ring in front of the hotel when I was in New York last year, the monument was replaced by a Moslem-style one. People told me that the monuments were replaced every 6 months.
Flying from Newark Airport in New Jersey by CO 192 flight to Orlando Airport in Florida, I arrived at Sheraton World Resort Tower Hotel near Orlando Convention Center to attend ACC meeting. I had an appointment to meet with doctors from Serbia and Dr. Kaneda at Hard Rock Cafe within Universal Studio City Walk at 8:00 PM. I could successfully meet with them. They showed me many difficult cases by using CDs, for which I would have to treat in April this year during my visit to Serbia. I came back to my hotel later than 1:00 AM next day.

I joined ACC meeting. There were several interesting presentations. The long-term results of TAXUS stent implantation were worse than CYPHER especially in diabetic patients, which were shown in both REALITY, and ISAR-DIABETES trials. The late stent thrombosis rate was significantly higher in patients with TAXUS stent (REALITY trial). It is really bad news for patients, physicians and company. Dr. Serruys presented the long-term results of ARTS-II study. The long-term survival was better in CYPHER stent patients of ARTS-II study compared to the CABG arm in ARTS-I trial.
TAXUS-V trial, which is a prospective randomized trial between TAXUS and bare-metal Express stents conducted for almost 1,200 patients within US, showed significantly better long-term results in TAXUS group without any increased risk of stent thrombosis.
The Endeavor-II trial was conducted for 1,200 patients in Europe and Asian countries, and compared the results after Endeavor and bare-metal Driver stents implantation. The Endeavor stent implantation resulted in the significantly better outcomes compared to Driver stent implantation. The incidence of late stent thrombosis was strikingly low.
During lunch time, I visited a famous restaurant “Emeril's Restaurant”. Chef Emeril is a very famous cooking chef in US TV programs, which is similar to “Iron Chef”.

It was a really busy day. I left the hotel at 5:30 AM and took a US 1044 flight from Orlando to Philadelphia Airport departing at 7:00 AM. Philadelphia is the capital of Pennsylvania State, which is located north-west to New York City. At Philadelphia Airport, I changed to a small jet airplane to Wilkes-Barre/Scranton Airport . Because Scranton is in the mountain area, the land was covered by snow. The airport is a small airport and located just middle of cities of Wilkes-Barre and Scranton. Dr. Samir Pancholy was waiting for me in the arrival hall. After 10 minutes' drive in Inter-States Freeway No. 81, we reached the city of Scranton. Scranton used to be famous for high-grade coal mine. There are many historical buildings in the city. I came to this city to visit Mercy Hospital, where Samir is practicing. Mercy Hospital is a Catholic private general hospital, has 350 beds, and does 750 PCIs and 100 CABGs every year. Mercy Hospital is competing with Community Hospital in Mercy. The distance between these 2 hospitals is only 3 Km. Everywhere in the hospital, I could see many posters showing the rating for Mercy Hospital conducted by a hospital grading authority. There were signboards welcoming my visit in front of the catheter laboratory area.

The chief of the catheter laboratory is Dr. Han Sun-Tak, who was from Gwangju in South Korea and senior to Dr. Jeong Myun-Ho. He started PCI since 1980 after attending Gruentzig's Live Demonstration Course. He started TRI since last November and is now doing TRI for most of cases. I assisted him to do TRI in 3 cases. After taking a picture in front of the Cardiology Center, we moved to a nice Japanese Restaurant “KATANA” in the neighboring city Wilkes-Barre, which was run by Japanese people. The Japanese-style dinner at KATANA was very nice but too much for me. After saying farewell to them around 10:00 PM, we moved to New York City by a car through Inter-State Freeway No. 80. The distance between Scranton and New York City is almost 250 Km. It took 3 hours. We crossed over the George Washington Bridge, which was constructed 70 years ago, to enter into Manhattan Island via New Jersey State. We finally arrived at Hilton Hotel Manhattan later than 1:00 AM next day. I flied back to Japan by taking JL 005 flight departing from John F. Kennedy International Airport at 12:15 PM on March 8, 2005 and arrived at evening of March 9.

セント・ビンセント・カトリック病院(SVCMC)でのTRIワークショップ

私のTRI手技を披露するために再びニューヨークに戻ってきました。皆様も良くご存知のように米国国内で日本人の医師が医療行為を行うことはとても難しいことです。Coppola先生は、私が合法的に医療行為を行うことが出来るように働いて下さいました。でも、昨年の3月4, 5日に既に行っているので、今回は昨年よりは簡単であったと思われます。
SVCMCは「包括的心臓病センター」の存在で有名です。ここにはインターベンションの分野ではとても有名なAmbrose先生や、Rentrop先生が働いていました。PubMedのウェブサイトで検索すれば急性心筋梗塞や不安定狭心症の分野でたくさんの論文を見つけることができます。SVCMCでは年間2,500例のPCIを含め、5,000例の冠動脈造影が行われています。冠動脈バイパス手術の症例数は250例です。ですから、SVCMCではPCIと冠動脈バイパス手術の比は10:1です。SVCMCではベッド数は400もありませんので、診断冠動脈造影あるいはAd-hoc PCIの症例のほとんどは日帰りです。

2005年3月2日昼の12:00に出発するJL 006ニューヨークJFK国際空港直行便に乗りました。12時間半の飛行の間私は1時間程度しか眠れませんでした。私は自分のラップトップで仕事をしました。ニューヨークに到着する3時間ぐらい前に一人のスチュワーデスが私の所に来て、「お医者さんですか?　アジアのある国からバンコク、東京経由の乗客が一人病気です。彼女は顔と体上半分に火傷があり、痛みを訴えています。彼女は英語を喋れないのですが、診て頂けますか?」と相談に来ました。勿論、どんな場面でも患者さんを診るのは私の勤めです。患者さんの席に行き診察しました。彼女は怯えていました。そして痛がっていました。彼女はまだ治癒していない火傷を負っていました。水を飲むように勧めてから、腿に鎮痛剤の注射をして、それから彼女の背中をさすりました。この後、この患者さんは落ち着かれました。

Coppola先生は5つ星のRitz-Carltonホテルを用意しておられました。そこには午後0:30頃に到着しました。私はまたニューヨークでも最も予約がとりにくいと言われている有名な日本食レストラン「Nobu」に行きました。昨年訪れた時には昼でも空席がありませんでしたが、今年は空席が目立ちましたので驚きました。味はフレンチと和風のコンビネーションです。味はとても良かったです。ニューヨークでは好みがとても早く変わるのです。
部屋で少し休んでから、Patel先生とSVCMCに行き、Coppola先生とお会いしました。少し打ち合わせてから、とても良いインド料理のレストラン「TOMARIND」にいきました。インド料理の夕食を摂りました。

朝7:15 AMにKwan先生がホテルに迎えに来て下さいました。彼は最近、新車のポルシェを買われました。7:30 AMにSVCMCに到着しました。この日は合計で29例のAd-hoc PCI症例がありました。午前中に1例私はしました。患者さんはインド系でRCAとLADにDESを植え込んでいました。Bhambhani先生がこの症例をアレンジされました。患者さんはLCX中部に少なくとも数年前からの慢性完全閉塞病変がありました。右橈骨動脈を穿刺し、6 FrのXB3.5を用いました。全長3 CMのCTOをMagic-FAを用いてクロスしましたが、1.5mmのOTW Maverickは通過しませんでした。Bhambhani先生が助手をされました。CTOの手前にOMがあり、しかもそこには90%の病変がありましたので、ここに2.0mm Maverickでアンカーをかけ、1.5mmのMaverickで病変を通過することができました。CTO部分には長いTAXUSステントを植え込みましたが、植え込み後にはCTOの末梢はとても大きく見えました。皆が私の手技を賞賛しました。この症例の後、Cezar Staniloae先生が優しく、私に末梢血管用のFoxHollowアテローム切除カテーテルを紹介して下さいました。Cezarはルーマニアで生まれ、カナダのモントリオール心臓センターでPCIのトレーニングを受けてからニューヨークに来ました。彼は腕の良い術者です。FoxHollowを用いて、膝下の中脛骨動脈完全閉塞を見事に治療されました。末梢血管に対してはとても良いディバイスです。それからしばらく症例をいくつか見てからホテルに戻りました。

この夜はユニオン広場前の「Blue Water Grill」というシーフード・レストランで皆と一緒に夕食を摂りました。前菜として、魚介類盛り合わせを頼みました。夕食の最中に、Bhambhani先生があのMt. Sinai病院の有名なSharma先生に電話されました。Sharma先生が来るよ、と彼は言いました。でも、あんな有名な先生が本当に来るかな?と、思いました。でも、Sharma先生は来られました。こんな有名な先生とお会いできて、とても光栄です。メインにはマグロのたたきを頼みました。味はとても良いのですが、量が多すぎました。

夕食後、Bhambhani先生がホテルまで車を運転して送って下さいました。彼はインドのJaipurからニューヨークに来ました。JaipurはPink Cityとも呼ばれます。彼はとても良く働き、収入も多いのです。現在、SVCMCでは最大の症例数を誇っています。彼はブルックリン地区に自分のクリニックを持ち、そこから患者さんをSVCMCに連れてきます。

この日もKwan先生がホテルに7:15 AMに迎えに来られました。Coppola先生の開会の言葉でTRI Workshopが8:00 AMから始まりました。それから、Patel先生が第一症例として、冠動脈バイパス手術後の患者さんに対する左橈骨動脈よりの診断カテーテルを披露されました。これに続いて、私がTRIを3例披露しました。一例目はAd-hocでRCAに対してPCIを行いました。残りの2例はCTOに対する待期的PCI症例でした。またもRCAに対するCTOに対してアンカーを用いなければなりませんでした。これらの症例に対して合併症無く、全例成功に終わり嬉しかったです。病院内で短い昼食を摂ってから、Patel先生とMartin Leon先生にお会いするために心臓病研究財団事務局を訪問しました。Leon先生はACCの準備でとても忙しかったのですが、我々に時間を割いて下さいました。色々なことを紹介して下さいました。この財団の年間予算は何と、44億円にもなり、これは今年のACCの年間予算50億円にも匹敵する額です。

Leon先生と別れてから、ウォール街にある有名な投資会社の上級アナリストからインタビューを受けました。医学と違う分野の人々と話をする機会が持てて、とても面白かったです。
この日は金曜日でしたので、インタビューが終わった午後5時には投資会社のオフィスにはほとんど人が残っていませんでした。この会社の人が私をとても良い寿司屋さんに連れていって下さいました。この店は秘密の場所で、どんなガイドブックにも紹介されていません。店には看板も無いので、皆知らずに通り過ぎます。敢えてこの店の名前はここに書かないことにします。この寿司屋さんの後、私たちは、本当の日本風居酒屋「祭り」に行きましたが、ニューヨーカーで込み合っていました。

朝はとても晴れていました。部屋の窓からは自由の女神がはっきりと見えました。昨年来た時にはホテルの前に日本の寺の鐘のモニュメントがありましたが、今回はイスラム風のものに換えられていました。何でも6ヶ月毎に取り替えられるそうです。
ニュージャージーのニューアーク空港からオーランド空港に飛びました。そして、ACCの学会場にあるシェラトン・ホテルに泊まりました。この夜は8時からHard Rock Cafeセルビアの先生方と金田先生と会う約束がありました。彼らは、今度4月にセルビアを訪問した時に行う予定のとても難しい症例のCDを見せました。部屋に帰ったのは翌日の1:00 AMになっていました。

ACCに参加しました。とても興味深い発表がありました。REALITYでもISAR-DIABETESでも特に糖尿病がある患者さんに対してはTAXUSよりもCYPHERの方が、長期成績が良いことが示されました。また、REALITY試験では、TAXUSは慢性期ステント血栓症の発症がCYPHERよりも有意に多いことも示されました。これは患者さんにとって、医師にとって、そして会社にとってとても悪いニュースです。Serruys先生はARTS-II試験の結果を発表されました。ARTS-Iの冠動脈バイパス手術群よりもCYPHER植え込み群の方が、良い長期長期成績であることが示されました。
アメリカで1,200名の患者さんに対して行われたBMSとTAXUSの比較試験であるTAXUS-Vの結果も発表されました。ここではTAXUSの方がBMSよりも有意に良い長期成績であり、ステント血栓症の危険も増加しないことが示されました。
アジア、ヨーロッパで行われたEndeavor-II試験では、EndeavorとBMSのDriverが1,200名の患者さんで比較されました。Driverに対してEndeavorの長期成績は良く、またステント血栓症の発症はとても少ないことが示されました。
昼食は有名なEmerilで食べました。ここは、アメリカのテレビで料理の鉄人のような番組で有名なシェフが持っている店です。

この日はとても忙しい日でした。ホテルを5:30 AMに出て、オーランドからフィラデルフィア行き7:00 AM発のUA 1044便に乗りました。フィラデルフィアはペンシルバニア州の州都で、ニューヨークの西北に位置します。フィラデルフィア空港でスクラントン行きの小さなジェットに乗り換えました。スクラントンは山ですので、雪に覆われていました。空港はとても小さく、二つの町の中間に位置します。Pancholy先生が空港に迎えに来て下さいました。大陸横断道路81号線を10分ぐらい走るとスクラントンの街に到着しました。スクラントンは昔、良質な石炭の産地として有名な処でした。街には歴史的建造物があります。Pancholy先生が働くMercy病院はカトリック系の民間病院であり、350ベッドを有し、年間750例のPCI、100例のCABGを行っています。Mercy病院は地域の自治体病院と激しく競争しています。病院のそこら中に格付け機関が行った病院比べの結果が張り出されていました。カテ室の入り口には私の訪問を歓迎する看板がありました。

カテ室の主任はハン先生でした。彼は韓国光州出身であり、丁先生の先輩です。彼は1980年からPCIを行っており、去年の11月からTRIを始め、現在では大多数の症例に対してTRIで治療しています。私は3例のTRIをアシストしました。心臓病センター前で写真を撮ってから、隣町にある日本食レストラン「刀」に皆で行きました。日本人がやっているレストランでした。和風の夕食はとてもおいしかったのですが、量が多すぎました。10:00 PM頃に別れを告げ、80号線でニューヨークまで車で移動しました。距離は大体250 Kmであり、3時間かかりました。ニュージャージー州を越えてニューヨークのマンハッタン島に入るには70年前に造られたジョージ・ワシントン橋を渡ります。ヒルトン・ホテルに着いたのは翌日の1:00 AMでした。3月8日 12:15 PM発のJL 005便に乗って日本に帰り、3月9日夕方に日本に到着しました。
