Cardiovascular Revascularization Therapeutics 2005 Meeting

I attended CRT 2005 meeting held in Washington DC between March 28 and 31, 2005. This was my first time to join this meeting. This meeting is presented by Cardiovascular Research Institute of Washington Hospital Center in Washington DC, and its course chairman is Dr. Ron Waksman. The meeting structure is involving Duke Clinical Research Institute (Dr. Mitchell W. Krucoff), Brigham & Women's Hospital in Boston (Drs. Richard E. Kuntz and Jeffrey J. Popma), Lenox Hill Hospital in New York (Dr. Dary S. Roubin), The Cleveland Clinic (Dr. Patrick L. Whitlow), EuroPCR (Drs. Jean Marco and Patrick W. Serruys) as well as Drs. Stephen E. Epstein, Kenneth M. Kent or Augusto D. Pichard from Washington Hospital Center.
The meeting composes of scientific lectures, presentations, workshops as well as live demonstrations. I was invited as only one Japanese Faculty Member in Coronary Intervention.
I flied from Tokyo International Airport by taking NH 002 flight for Washington DC departing at 11:10 AM on March 28 Monday, 2005. After 11 and half hours’ flight, I arrived at Washington Dulles International Airport before 10:00 AM on the same day. After 30 minutes’ drive by a taxi, I could reach Hilton Washington Hotel, where the meeting was held.

Fallen in Love for a girl 40 years ago

I had got very tired, since I could not sleep on the airplane only for 2 hours and was very busy before I left Japan. I prepared for my presentation during the meeting and watched 2 movies on the airplane.
The first movie was a Japanese movie “Year One in the North”. The main actress was Ms. Sayuri YOSHINAGA. The story was on the people who were forced to move from the warm to the north area in Japan and had to struggle for life by themselves 100 years ago, when Japanese society had dramatically changed from SAMURAI (Japanese warriors) Era to more modern Emperor Era. The movie was not bad but a little bit long for me.
The next movie I saw was again a Japanese movie “Beyond the Green Hills”, which was produced in 1963. In early 60s, Japan used to be very poor as a defeated nation after the World War II. People wanted to have something interesting and making hopes for everybody. Then, this movie came up in front of people. Almost all of Japanese people went to the movie theaters to watch this movie. The theme song of this movie is still very popular in Japan. The main actress of this movie was also Ms. Sayuri YOSHINAGA. She was really pretty and beautiful in this movie. Although she at present is still beautiful, I have fallen in love for Sayuri YOSHINAGA in 1963.

After having a short lunch in the lobby restaurant of the hotel, I came down to the B2 hall to make my registration for the meeting. Dr. Waksman found and came to me. He told me, “We were really waiting for you coming. Thank you for joining.” Then, Dr. Pichard came to and welcomed me. He said me that they had installed a cardiac MRI machine within the catheter laboratory in Washington Hospital Center. After the registration, I walked in the speakers’ ready room and passed my presentation file to the guy in charge by using a USB memory stick.
A very interesting meeting “Workshop with the FDA: Regulatory Think-Tank Sessions” was held on the day from morning to evening. In the morning session, there were presentations and discussions on the recently FDA-approved medical devices used in PCI. My role was assigned in the afternoon session, which was “Global Perspectives on Harmonization of Pre- and Post-Market Device Evaluation” and started from 1:30 PM. Following the presentation by Dr. Hans Bonnier from Belgium, I did my presentation. Hans spoke how doctors in EU were related to the device approval processes in Europe. My presentation was about how the clinical trials for device approval should be, how much we physicians got ready for international trials, what we had done so far for the global harmonization and what we were planning to do in the context of “Global Harmonization by Doing”.
There were Dr. Waksman, Mr Koji IKEDA from PMDA Japan, Dr. Kazuhiro SASE from National Cardiovascular Center Japan and other famous people together with Drs. Bram D. Zuckerman and .William H. Maisel from FDA.
Now many people are working how we can achieve the global harmonization for regulatory approval process in the world. There is an international committee “Global Harmonization Task Force (GHTF)”. Each country has its own regulation, social customs, history, and so on, so that it might be very difficult to achieve the global harmonization. However, we are going ahead gradually by doing “Global Harmonization by Doing (GHD)”, which means that we start from what we can do now.
After finishing this Think-Tank meeting, I went to a Japanese Sushi restaurant “SUSHI TARO”.

I was sick in severe jet lag as always. I went to my bed around 11:00 PM but waked up completely at 2:00 AM. I knew that another role was assigned to me after I came to Washington DC. The 2nd role was the lecture on angioplasty for CTO. The title assigned to me was “Guidewire Selection for Different Subsets of Chronic Total Occlusions”, and it started from 7:00 AM. After finishing the preparation of my presentation, I came down to the 2nd basement floor by 6:40 AM. The registration counter was still vacant. I did my job of presentation just within the assigned time interval of 15 minutes. Next to me, Dr. Hans Bonnier did his presentation on SafeCross system. He finished his presentation just within 15 minutes. The last speaker was Dr. Koh Tian-Hai from Singapore National Heart Center. He is the chairman of Singapore Live Demonstration Course. The presentation duration assigned to him was also 15 minutes, but he continued his presentation for more than 20 minutes. Since I had to leave the hotel for the airport in the morning, I came back to my room to pack my luggage before he finished his presentation.
After checking out the hotel, I hurried to the Dulles International Airport to catch the returning flight to Tokyo of NH 001 departing at 11:20 AM. After flying for 13 and half hours, I arrived at Narita International Airport around 3:00 PM on March 30 Wednesday, 2005. I had to stay at the airport, since I had to take another flight to Beijing on the evening.
This time, my visit to US was short and just “1 night and 3 days” travel. However, it was very exciting and stimulating me.

CRT 2005学会

今回私は、米国の首都ワシントン市で2005年3月28日から31日まで開催されたCRT 2005学会に参加しました。この学会に参加するのは初めてのことでした。この学会は、ワシントン市にあるワシントン病院センターの心臓血管研究機関によって開催されているもので、会長は同病院のRon Waksman先生です。主な参加母体としては、デューク大学臨床研究所、有名なボストンのBrigham & Women's病院、ニューヨークのLenox Hill病院、クリーブランド・クリニック、そしてヨーロッパが挙げられます。そこには、Krucoff先生、Kuntz先生、Popma先生、Roubin先生、Marco先生、Serruys先生なども加わっています。また、ワシントン病院からはKenneth Kent先生やPichard先生もメンバーです。
学会は、講演、発表、ワークショップそしてライブデモンストレーションからなっています。私は、冠動脈インターベンションに関する日本から唯一のFacultyとして招聘されました。
私は、成田空港発2005年3月28日午前11時10分発のワシントン行き直行便NH 002便に乗りました。11時間半の飛行の後、ダレス国際空港に着いたのは同日の午前10時前でした。タクシーで30分ぐらい走ってから、学会会場であるワシントン・ヒルトン・ホテルに到着しました。

40年前の映画なかなか良いです

飛行機の中では2時間しか眠らず、また、日本を出る前もとても忙しかったので、とても疲れました。飛行機の中では、自分の講演の準備をすると共に、2本の映画を見ました。
最初に見たのは、日本映画「北の零年」です。主演は吉永小百合であり、100年ぐらい前に暖かい土地を追われ、北海道の北の大地に移り住んだ人々の辛くもたくましい生活を描いたものです。時代は侍の時代から、明治天皇の時代に移り変わる時でした。この映画は悪くはなかったのですが、少し長すぎました。
次に見たのは、1963年に封切られた「青い山脈」です。60年代初めといえば、まだ第二次大戦の敗戦から日本は貧しい時であり、人々は何か希望を抱けるものを求めていました。この映画は皆の心を打ち、皆がこぞって映画館に行きました。皆、この映画の主題歌を知っています。主演はやはり吉永小百合です。彼女はとても可愛らしく綺麗でした。今でも吉永小百合は綺麗ですが、私は、1963年の吉永小百合に恋をしました。

ホテルのレストランで少し昼食を摂ってから、地下二階に降り、学会登録をしました。Waksman先生が私を見つけ近寄ってきました。「ようこそ、待っていました。来て下さって本当にありがとう」と、言われました。次に、Pichard先生が来ました。彼は、「ようこそ、ところで、ワシントン病院のカテ室には心臓MRIが入ったよ」と、僕に話しかけてきました。登録を済ませてから、発表の準備室に行き、USBメモリーを用いて、私の講演ファイルを係りの人に渡しました。
この日は朝から夕方まで、米国医薬品食品局(FDA)とのワークショップ:認可について考えるシンクタンクという面白いセッションが開催されていました。午前中のセッションでは最近FDAが認可したPCI分野のディバイスについての発表と討議が行われました。私の役割は午後の、「認可前後におけるディバイス評価に関する国際的基準の策定」というセッションの中での発表で、午後1時半からのものでした。私の発表は、Hans先生の、ヨーロッパでの現状についての発表に引き続き行われました。私は、日本では多くの医師が国際臨床試験を行うことに前向きであり、また、これまでに国際的統一基準のために何をしてきたか、そして具体的に何を予定しているか、について講演しました。
会場にはWaksman先生、日本のPMDAからの池田様、国立循環器病センターから佐瀬先生、米国FDAからZuckerman先生、Maisel先生そして、他にも有名な先生方がおられました。
世界で多くの人々が、認可プロセスを国際的に近づけるために努力しています。そして、GHTFという組織が既に作られています。それぞれの国には、それぞれの国内法があり、社会習慣も異なり、また歴史も異なります。従って、世界で統一的な基準を策定することはとても困難です。しかし、私達は、GHD「まずは今出来ることを行おう」という考えで一歩一歩前に進んでいます。
このシンクタンクを終えてから、寿司レストラン「寿司太郎」に行きました。

何時もの如くひどい時差ぼけでした。午後11時頃寝たにも関わらず、午前2時には起きてしまいました。ワシントンに着いてから、もう一つの役目を入れられていることを知りました。それは、CTOに対する講演でした。そして、講演タイトルも決められていて、それは「CTOそれぞれに対するガイドワイヤーの選択」というものでした。その講演の準備をして、朝の6時40分には地下二階の会場に降りました。登録受付はまだガランとしていました。私は、自分の講演を丁度15分間で終えました。次に、Hans Bonnier先生がSafeCrossの講演をしました。彼も、15分間で終了しました。次に、シンガポール心臓センターのKoh先生が講演しました。彼は、シンガポール・ライブデモンストレーションの会長です。彼の講演時間も15分間でしたが、20分経っても終了しませんでした。私はその日の朝に空港に向かわねばなりませんでしたので、途中で席を立ち、部屋に戻って荷物を纏めました。
ホテルをチェック・アウトしてから、ダレス国際空港に向かい、午前11時20分発のNH 001便東京行きに乗りました。13時間半の飛行の後、成田空港に到着したのは、2005年3月30日午後3時頃でした。私は、北京行きの飛行機に乗り継ぐために、空港に止まらねばなりませんでした。
今回、私のアメリカ訪問は短く、1泊3日の旅でした。それでもとても有意義なものでした。
