Introduction

After I transmitted 2 cases from my hospital to the Venue of CIT 2005 meeting in Beijing on Sunday April 3rd and worked for 1 day at my hospital on Monday, I flied to Beograd of Serbia and Montenegro on April 5 Tuesday, 2005. This trip was my 2nd trip to Beograd and a really tough one. I traveled with Dr. Doumae, one of my junior partner doctors. I took the flight to Zurich by Lufthansa Airline (LX 169) departing at 10:10 AM from Narita International Airport. After spending about 2 hours in Zurich International Airport, I changed the flight to Beograd by Yugoslavia Airline (JAT Airline: JU 331) and finally arrived at Beograd around 7:30 PM on April 5. Beograd is the Serbian notation of Belgrade in English.
Dr. Milan A. Nedeljkovic was waiting for us at the Airport and took us to our hotel by his car. On the way to the hotel, we could see a new basketball stadium, which was constructed after I visited last time in October 2003. It was already around 9:00 PM when we checked in the rooms at Hotel Hyatt Regency Beograd, which is the best hotel in Serbia and Montenegro and located near Hotel Intercontinental Beograd where we stayed last time.

We had dinner in the Serbian Food Restaurant within the hotel with several Serbian doctors. After having dinner, I went to the bed by 11:00 PM. This time, I visited Beograd to demonstrate complex angioplasty during the International Workshop held at Clinical Center of Serbia in Beograd between April 6 and 8. Actually, this workshop was held for my live demonstration and planned by Dr. Miodrag C. Ostojic, Director of Cardiology at Clinical Center of Serbia and Dr. Milan. All of Yugoslavian doctors were very much impressed by my previous live demonstrations in 2 cases with CTO lesions during the 14th Congress of Cardiology Society of Serbia and Montenegro held between October 12 and 15, 2003 in Beograd. Thus, they have planned to invite me again to learn how to do PCI for CTO lesions.

April 6 Wednesday, 2005

We went to the auditorium of Clinical Center of Serbia. It is the biggest general hospital in Serbia and Montenegro and actually the University Hospital of Faculty of Medicine at Beograd University. The hospital main building is huge. However, I was informed that the space above the 3rd floor was surprisingly empty, because they had no money to run it. The auditorium was full of more than 100 participants from all around Serbia and Montenegro, Bosnia and Bulgaria.
First, we reviewed cases together. When I met Dr. Ostojic and Dr. Milan in Orlando during the last ACC meeting, they took so many CDs of more than 20 patients. We reviewed those CDs in the booth of Siemens, where DICOM viewer machines were demonstrated. All of those CDs showed the cases planned for live demonstration during this International Workshop, and all of the lesions were chronic total occlusions. It seemed impossible to do all of those cases by myself during only 2 days live demonstrations. I told and declared them that I would be able to do only 4 cases with CTO lesions at most in one day during live demonstrations, so that the number of cases should be reduced.
However, they could not reduce the number of cases. They told me, “We had so many telephone calls asking the treatments by Dr. Saito from Japan. We could not reject it. Some patients are our friends, and some are very important people, and so on.”
After case review, I had to sign on the Certificate for Participation for all of the participants of more than 200 people.

They had 2 catheter laboratories for Interventional Cardiology. One room was very new and equipped with a Siemens’ flat panel single-plane machine, and another was with a Siemens’ II single-plane machine. They arranged 7 cases for each laboratory a day on Wednesday and Thursday for me. The hardware was satisfactory, and assistant doctors, nurses and technicians were well trained. Many young doctors were trained by Dr. Colombo in Milan. However, the supply of products was unfortunately not satisfactory. They had only limited shapes of 6 French guiding catheters but no 7 or 8 French ones. They had very few number of 1.5-mm balloons. They did not have any OTW balloons or microcatheters. The guidewires available were only ATW, Choice-PT or some Cross-it series. Of course, I took several guidewires for CTO with me from Japan. However, they arranged too many cases for my visit. I spent all of the guidewires taken from Japan.
While doing cases, they invited interviewers from several TV stations. I had to receive those interviews between the procedures. We finished 14 cases by 7:00 PM. Of course, I was not successful in some of these cases, but I was successful in many cases. After the procedures, the biggest pharmaceutical company in Serbia invited us to dinner at a Serbian Restaurant. Everybody had Beograd Steak.

April 7 Thursday, 2005

Before lunch time after doing the cases in the morning, I was invited to the office of Dean of the Faculty of Medicine in Beograd University. The building of Faculty of Medicine was at 7 minutes’ walk distance from the hospital. We crossed the garden of the University, where the cherry blossoms were in full bloom.
I was inaugurated as a visiting Professor of Medicine at Beograd University and delivered the inaugural lecture at the auditorium of the Faculty of Medicine. My lecture was on angioplasty for CTO lesions.

After delivering the lecture, we came back to the hospital and continued angioplasty for patients with chronic total occlusion. It was a really tough day.
Although the dinner party of the Workshop was arranged on the night, I politely rejected to join the dinner. I was really tired and wanted to be free from everything. We went to an Italian restaurant in the downtown of Beograd with Dr. Doumae and enjoyed the atmosphere of the city.

April 8 Friday, 2005

The program of the workshop was only for local people on Friday, so that we had free time on the day. Dr. Milan took us to city tour. In the city, there are still several buildings, which were bombed by NATO bombers in 1999. Those buildings were mainly of the Government like the buildings of the Ministry of Internal Affair or Yugoslavian Military Headquarter. Also Chinese Embassy building was erroneously bombed by a NATO aircraft. By these bombings continued for more than 2 weeks, many military as well as civilian people were killed. I think that they must preserve those bombed buildings forever to memorize the history.

Walking through the Military Museum, we arrived at the Kalemegdan Park. During the World War I, as much as 28% of whole population in Yugoslavia was killed, and 11% of people died during the World War II. The Kalemegdan Park used to be the Beograd Castle. The park is located on the hill, under which the River of Dunav and the River of Sava merge. Thus, Kalemegdan used to be the most important place strategically. The hill is as small as the square of 700 meters each side. However, more than 6 million people have been killed during 114 major battles there in the history. From the park, we can see the beautiful scenery of 2 major rivers.

In the city, there are old architectures. We also visited the Tomb of the Past Great President Tito, which is now preserved by the Yugoslavian Army. We could see many yellow buses, on whose body we can see the National flags of Japan and Serbia and Montenegro. Those buses were donated by Japanese Government.

We thought that all of our works during the workshop had been already finished. However, it was not true. Dr. Milan and Dr. Miodrag took us to the Headquarter of the biggest Newspaper Company in Yugoslavia “POLITIKA”. We had interviews there by the chief editor. While having the interviews, I honestly talked about my impression on Beograd. First of all, I was very surprised to know the high educational level of Serbian doctors. I think that the education is the most important part to develop the country. Second, I love the beautiful scenery and wonderful people in Serbia and Montenegro. Third, I knew some part of the history in Yugoslavia. Fourth, I knew the current situation of this country. After this interview, we finally got completely free from all of the duties.

Dr. Goran Milasinovic kindly invited the lunch in a very nice riverside restaurant. There are many nice restaurants along the riverbank of Sava River. We went to one of them. The food was very delicious in taste. We became relaxed first in this trip. After finishing the lunch, Dr. Goran took us to the airport by his own car.

On the freeway, I could see the traffic signboard showing “Zagreb and Beograd Airport”. We took the Alitalia flight (AZ 537) to Milan departing at 16:55. After spending 3 hours in Milan Airport, we took the returning flight to Japan (JL 418), and finally came back to Japan around 4:00 PM on April 9 Saturday, 2005.

A Sequel to the Story

This journey to Serbia was a really tough one for me. I almost finished my whole energy. It took more than 7 days for me to recover my spirit and physics completely. However, the e-mail from Dr. Milan, which was sent 3 weeks later, was really impressive. For what I am doing this kind of efforts in the world? I really believe that we can promote peace in the world through TRI and/or angioplasty procedure demonstrations. His e-mail was “We think that your performance during the Workshop was fabulous. You know that we had problems with Croatia in past ten years. However, your participation enabled us to make active cooperation with them. So they are still talking about quality of the work and about the organization of the workshop. Everything was done professionally especially because of you. I know that we prepared heavy cases for you, but we suffered terrible pressure to do all those cases. The whole Serbia expected miracle from you and honestly miracle happened. The papers and TV stations in Serbia are still talking about your visit. Popular medical journal will publish a text about you with the pictures from Orlando and the pictures from cath lab.”

Several days later, I received a telephone call from the Secretary of the Ambassador of Serbia and Montenegro in Tokyo. The Ambassador Dr. Predrag Filipov kindly invited me privately to his office. He knew my visit in his nation through the news on the Website of POLITIKA. During 1 hour, we talked each other a lot about the backgrounds of us. He was born and grown up in Novisado, which is the 2nd biggest city in Serbia and Montenegro. He graduated from the medical school of Novisado University. He worked as a very good physician for many years. In order to rebuild the social structure in his nation, he came to the Government. He is a very kind person. I would like to keep in touch with him and trying to do my best for people in Serbia and Montenegro forever. .

はじめに 北京のCIT 2005学会会場に当院からライブ症例を4月3日日曜日に中継し、翌月曜日は病 院で働き、そして2005年4月5日火曜日にセルビア・モンテネグロのベオグラードに飛び 立ちました。今回は2回目のベオグラード行きであり、部下の堂前君を連れて行きました。 まず、午前10:10成田発のルフトハンザ航空でチューリッヒに飛び、そこで2時間のトラ ンジットの後で、ユーゴスラビア航空(JAT)に乗り換え、最終的にベオグラード空港に到着した のは5日の午後7:30でした。なお、ベオグラード(Beograd)というのは、英語ではベルグ ラード(Belgrade)となります。. Dr. Milan A. Ostojic, Director of Cardiology at Clinical Center of SerbiaとDr. Milanによって企画されました。2003年10 月12日から15日まで開催されたthe 14th Congress of Cardiology Society of Serbia and Montenegro において私によって行われた2例の慢性完全閉塞症例に対するPCIにユーゴ スラビアの全てのお医者さんが感銘を受けたのです。そこで、もっと慢性完全閉塞に対す るPCIを私から学ぶために企画されたのです。 2005年4月6日水曜日 我々はClinical Center of Serbiaの講堂に行きました。この病院はSerbia and Montenegro において最大の病院であり、Beograd University医学部付属病院です。建物は大きいので すが、3階より上は空っぽです。そこを運営する資金が無いのだそうです。 講堂にはSerbia and Montenegro, Bosnia そしてBulgariaから100名以上の参加者が集まっていました。. まず皆で症例検討をしました。私がこの前のACCの期間にOrlandoでDr. Ostojic とDr. Milan会った時に、彼らは20名以上の患者さんのCDをたくさん持参してきました。 Siemensのブースに行き、それを見ましたが、その患者さん全員が慢性完全閉塞病変をタ ーゲットとするものであり、その全てを今回のWorkshopで予定していたのです。私は彼 らに、一日最大で行える慢性完全閉塞症例は4例までである、と宣言しました。そして、 症例数を減らすべきだ、と提言しました。 しかし、彼らは症例数を減らすことができませんでした。彼らによれば、「日本から来る Dr. Saitoに治療して欲しい、という電話がたくさんかかってきた。それらを拒否できなか った。何人かは我々の友人だし、何人かはとても重要な人なので。」 症例検討が終わってから、200名以上の参加者の参加証にサインをせねばなりませんでした。 Clinical Center of Serbia Case Review Signing on the Certificates Doing a Procedure PCIで用いることのできるカテ室は2つありました。片方には最新型のSiemensフラット・ パネル(シングルプレーン)があり、別の部屋にはSiemensのII式シングルプレーンがあり ました。水曜日と木曜日のそれぞれに、それぞれ一部屋当たり7例の症例が用意されまし た。ハードは問題なく、助手、看護師そして技師さんたちも良く訓練されていました。若 い医師の多くはミラノでDr. Colomboの下でトレーニングを受けていました。しかし、問 題はサプライでした。ガイディング・カテーテルは全て6 Frenchであり、しかも形状も不 十分で7 Fr, 8 Frのものは一つもありませんでした。また、1.5mmのバルーンもほとんど 無く、OTWバルーンやマイクロカテーテルは皆無でありました。ガイドワイヤーについて は、あるのはATW、Choice-PTそしてCross-itが何本か、でした。もちろん、私はCTO 用ガイドワイヤーを日本から準備していきましたが、とても足らずに全て使い切ってしま いました。 症例の合間にはテレビ局のインタビューもありました。その日の午後7時までにこれらの 14例を終了しました。多くの症例はうまくいきましたが、何例かは不成功に終わりました。 夜はセルビア最大の製薬会社がディナーに招待してくれました。皆、ベオグラード・ステ ーキを食べました。.
