Finally, I arrived at Pohang Airport, which looked very new. Pohang is one of the most important cities in South Korea, where the most important heavy industry company POSCO is established. POSCO Steal Company is now producing good-quality iron products.

After checking in the hotel, I moved to a sea-side seafood restaurant. Pohang is famous for fresh seafood. The most famous one is so-called “Water SASHIMI”. The fish is caught and cooked on the same day. My friend Dr. Cho KilHyn tried to find the best seafood restaurant in Pohang area for me. He introduced me U-Mok Raw Fish Restaurant. It is located at the end of the living area aside Pohang City. It took more than 30 minutes from my hotel to the restaurant by high-speed driving taxi. Dr. Cho was waiting for me with the master of the restaurant. The food was really fresh and very delicious.

It was the 2nd meeting with Dr. Cho for me after almost 1 and half years blank. I like him very much.
May 16 Monday, 2005
I went to SunLin Hospital by 8:30 AM, which is a private Catholic Hospital. Dr. Cho arranged me 7 cases. The normal arterial access site here is of course transradial approach. He is very skillful in doing TRI. His skill for TRI is very nice, and his personality is really worth for admiring. He already spent 2 years in Pohang City after he left from Busan Paik University Hospital. He is the only Cardiologist in this hospital. The technicians, nurses and a coordinator for catheterization and PCI are assisting his work very much.

Fortunately, he will have a young fellow doctor from the spring of 2006. He is doing his best to improve the situation in Interventional Cardiology in Pohang Area. I also would like to continue doing my best to support and keep deeply in touch with him.
After leaving SunLin Hospital, I took a car to Busan. The distance between Pohang and Busan is about 100 Km and it takes 2 to 2 and half hours by driving a car. I crossed the beautiful suspension bridge over the ocean “Gwang-alli Bridge”, and arrived at Hermes Hotel just in front of Gwang-alli Beach. The night view of the bridge is very beautiful.

After checking in the hotel, I went a famous seafood restaurant for “SeKoSi”, which name is “Chil-Sung Fresh Food Restaurant”. The price of one “SeKoSi” fish was almost 35 US$.

I visited Su-Yeong Han-Seo Hospital at 8:00 AM, where Dr. Kim Jong-Hyun and his cathlabo team members moved last autumn from Busan Baptist Hospital. I know him very much for many years. He is also one on the strong Radialists in South Korea.

He moved to this hospital with Dr. Lee, who is an electrophysiologist and an X-ray technician. He started coronary angiography and angioplasty since November 2004. His team has already done almost 2,000 cases for 6 months. He prepared me more than 20 cases on the day. His team worked very quickly. And we finished all of these cases by 6:00 PM. I love Dr. Kim and his team very much.

After finishing the cases, I flied to Seoul Kim’bo Airport from KimHae Busan Airport, and stayed at Grand Inter-Continental Hotel.

After waking up at 6:30 AM, I left the hotel around 7:15 AM. By a car, I moved to the city of Buntang, which is one of the satellite cities of Seoul and located southward by 30 Km. Buntang Seoul National University was opened 2 and half years ago. Dr. Dong-Ju Choi moved from GyeongSang University Hospital in ChinJu, and Dr. In-Ho Chae moved from Seoul National University Hospital.

I arrived at the hospital, which is a big beautiful building like a hotel and located on the hill. The entrance lobby is also like a hotel, and there are lobby gallery and mini-concert hall. After arriving at the hotel around 8:00 AM, I did a lecture on TRI for all of the Cardiology staffs in an auditorium.

All of the patients’ record were stored in a centralized Server Machine, and they do not use any paper file system no more in this hospital. They had 2 X-ray machines, and both are Philips’ machines: One with Image Intensifier, and another with FPD (Flat Panel Detector). The older machine with II (Image Intensifier) was mainly used for emergency cases, pediatric catheterization or electrophysiology. I used the newer machine with FPD. They arranged 4 cases for me. The 1st case was a male patient with not so difficult anatomy. Dr. Chae arranged this first case for me as a kind of “warming up”. I did this case successfully by TRI and quickly. The next case was interesting. He was 39-years old gentleman with triple vessel disease. He had double chronic total occlusions in the proximal LAD and mid LCX. The lesion in the proximal RCA was also a 99% narrowing. The LV function was normal. I could not understand how he could survive. I did this case from the right radial approach. First, I fixed the RCA lesion, and next tried to open the mid LCX occlusion. It was more difficult than my imagination. However, I could fortunately open the lesion. Finally, I opened the LAD occlusion. It was again more difficult than my expectation. Anyway, I was very happy to fix all of the problems with this young patient. Dr. Chae was very skillful and kind to assist me during this difficult procedure. Dr. Chae told me that he had at least 2 times chanes to go to US for study in the past. However, he cancelled it due to unavoidable reasons. However, I think he can speak very fluent English. Also, I think his skill , knowledge as well as personality are very much admirable. I am sure he will be one of the strong candidates for the leadership Interventional Cardiologists of next generation in South Korea.

After these 2 cases, we had lunch together in the “Sky Lounge” on the 19th floor. Then next case in the afternoon was a patient with a CTO lesion in the proximal LAD. Dr. Chae tried once several months ago but failed. I started with the double femoral punctures in order to take simultaneous bilateral coronary angiograms. I started from a Magic-FA guidewire, which went into the previously made false lumen. Then, I took a Conquest-Pro 12 guidewire, which passed into the diagonal branch distal to the occlusion. After dilating the occlusion by using a 1.5-mm balloon, I could successfully cross the wire into the distal true lumen and put a drug-eluting stent. I was quite happy for the result. The final case was supposed to have a total occlusion based on the findings of multi-slice CT scan. I did this case from the right radial approach. However, coronary angiograms showed completely normal coronary arteries. I was informed at the end of these 4 cases that I was the first foreign doctor invited to this catheter laboratory. I felt much honored to hear that. I left the hospital by 4:00 PM and returned to Seoul to have dinner with Dr. Gwon and his catheter laboratory members.
While having dinner, we talked about Korean and Japanese movie stars. Dr. Gwon told me that Japanese Director Hayao MIYAZAKI was very famous in Korea. The characters created by him such as "ToToRo" or "NAUSICA" is very famous among children in Korea.

After checking-out the hotel, I went to SamSung Medical Center around 8:30 AM. Dr. Gwon Hyeon-Cheol is doing a lot of cases in SamSung Medial Center. Dr. Lee is the director of Interventional Cardiology. Dr. Gwon’s skill is excellent. After doing several cases there, I left for Kim’bo Airport to take the returning flight to Haneda Airport departing at 2:30 PM on the day, and came back to Japan.

ポハン空港はとても新しい装いでした。ポハンは韓国の経済にとって、とても重要な都市であり、POSCO製鉄があり、 良質な鉄鋼を生産しています。

ホテルにチェック・インしてから、海沿いのレストランに急ぎました。ポハンはまた新鮮な魚介類でも有名ですが、 中でも水刺身というのが有名です。魚はその日に獲れたものを料理します。 私の友人であるDr. Cho KilHyn は 今回、私のために一番おいしいレストランを探して下さいました。それはウモック刺身という名前の食堂です。 ポハン市街地の外れにあり、神風タクシーでもホテルから30分かかりました。Cho先生と店のご主人が私の到着を 待っておられました。食事はとてもおいしく、新鮮でした。

朝の8:30にSunLin病院に向かいました。この病院はカトリック系の民間病院です。Cho先生は私のために7例の 症例を準備されていました。この病院でも経橈骨動脈的冠動脈インターベンションが標準です。Cho先生の 橈骨動脈穿刺はとてもうまく、また彼の人格もとてもすばらしいと思います。彼は釜山白大学病院からこのポハンに 移って既に2年間経過し、その間病院内たった一人の循環器科医師としてがんばってきました。技師さんや看護師さん、 そしてカテのコーディネーターはとても協力的で、彼一人で行うPCIを皆で助けています。

2006年の春からは喜ばしいことに、若手の医師が彼の下に来るということです。彼はこれまでポハン地域 のPCIの状況を改善するために努力してきました。私は、今後もより一層彼に協力し、ポハン地域のPCIの状況 を改善するために尽くしたいと思います。
SunLin病院を出て、車で釜山に向かいました。ポハンと釜山の間の距離は大体100 Kmぐらいであり、 車で2から2時間半かかります。とても綺麗な廣里(グワンガリ)つり橋を渡って、エルメス・ホテルに入りました。 このホテルはグワンガリ浜の目の前にあり、グワンガリ橋の美しい夜景を見ることができます。

水洋ハンソ病院を朝8時に訪れました。この病院に釜山バプチスト病院からDr. Kim Jong-Hyun先生と、 そのカテ室スタッフが2004年秋に移動しました。彼とはもう何年もの付き合いです。彼もまた韓国国内での 強力なTRI推進者です。

Kim先生は同僚の電気生理専門のLee先生、そしてレントゲン技師さんと共に移動しました。 2004年11月からカテを開始したのですが、この6ヶ月で何と2,000例を行いました。この日彼は私に20例以上 用意して下さいました。このカテ室はとても動きが早く、夕方6時までにはこれらの症例全部を終了しました。 Kim先生と彼のチームもとても好きです。

症例を終えてから、釜山金海空港から金浦に向かう飛行機に乗り、ソウルの インターコンチネンタル・グランド・ホテルに宿泊しました。

朝6:30に起床し、ホテルを7:15AMに出発しました。車で盆唐(ブンタン)市に向かいました。 ブンタンはソウルの衛星都市の一つであり、ソウルの南30 Kmぐらいの所に位置します。ソウル国立大学ブンタン 病院は2年半前に開院しました。Dr. Dong-Ju Choiは清州(チンジュ)の慶山(キョンサン)大学病院から移動され、 Dr. In-Ho Chae 先生はソウル国立大学病院から移動されました。

病院はホテルのようで丘の上にあります。ロビーも綺麗で、ギャラリーやミニ・コンサートも開かれます。 病院には8:00 AMぐらいに到着し、まず講堂で循環器科スタッフ相手にTRIの講義を行いました。

この病院では完全電子カルテが採用されています。シネは2台ありますが、両方ともフィリップス製でした。 IIの古いマシンはもっぱら緊急用、小児あるいは電気生理に用いられています。この日私はフラット・パネルの 新しい機械を用いました。用意されていたのは4例でした。第一例目はそれほど難しい症例ではありませんでした。 Chae先生は私のウォーミング・アップのためにこの症例を準備されました。もちろんTRIでさっさと終わりました。 次の症例はとても興味深いものでした。患者さんは39歳の男性で、3枝病変でした。左冠動脈前下行枝近位部と 左冠動脈回旋枝の中ほどで慢性完全閉塞を呈していました。そして、右冠動脈近位部は99%狭窄でした。 どうしてこの患者さんがこの状態で生きておられるかが理解できませんでした。まず、右冠動脈病変を治しました。 そして、次に左冠動脈回旋枝の慢性完全閉塞を開けにかかりました。予想したよりは困難でしたが、 うまいこと開けることができました。最後に左冠動脈前下行枝の閉塞にとりかかりました。またしても予想よりも 困難でしたが、うまくいきました。この若い患者さんを救えた、ということは私にとってもとても嬉しいことです。 この間、Chae先生がとても上手に私のことをアシストして下さいました。Chae先生によれば、これまでに少なくとも2回の米国留学のチャンスがあったそうです。しかし、やむを得ぬ事情でキャンセルしたのだそうです。しかし、彼の英語はとても流暢です。そしてまた、彼の技術、知識そして人格も素晴らしいと思います。彼は間違いなく韓国の次代を担うInterventional Cardiologistsの一人でしょう。

この2例を終えてから、私達は19階にあるスカイ・ラウンジで昼食を摂りました。 午後の次の症例は左冠動脈前下行枝近位部慢性完全閉塞症例でした。 Chae先生が数ヶ月前に一回トライし、うまく行かなかった症例です。 経大腿動脈的冠動脈インターベンションで入り、左右同時冠動脈造影を行いました。 まずはMagic-FAから入りましたが、そのガイドワイヤーは以前に形成された偽腔に入りました。 次に、Conquest-Pro 12を用い、見事閉塞末梢の枝に貫通することができました。1.5 mmのバルーンで拡張し、 本幹にガイドワイヤーを選択しなおし、そして薬剤溶出性ステントを植え込みました。結果にはとても満足です。 最後の症例は、マルチ・スライスCTで左冠動脈前下行枝の閉塞が疑われた症例です。 右橈骨動脈よりまずカテを行いましたが、冠動脈は正常でした。 実は、私がこの病院のカテ室に招待された最初の外国人医師だという事でした。 その事を知ってとても誇りに思いました。病院を4時前に発ち、ソウルに戻りました。 そして、Gwon先生とカテ室メンバーと夕食を摂りました。
私達は、韓国や日本の映画俳優について話しました。Gwon先生によれば、日本の宮崎 駿 監督のアニメ・キャラクターである「トトロ」や「ナウシカ」のことは子供たちの間でとても有名だそうです。

ホテルをチェック・アウトしてから、サムソン病院に8:30 AMに到着しました。Dr. Gwon Hyeon-Cheolは、 この病院で数多くの症例を行われています。Lee先生がPCI部門の責任者です。Gwon先生の技術も とてもすばらしいものがあります。ここでは数例の手技を行い、それから金浦空港に向かいました。 そして、その日の2:30 PM発の羽田便で日本に帰国しました。
