I went to Institut Jantung Negara (IJN), which is National Heart Institute in Malay before noon. When first I visited IJN about 10 years ago, they had less than 300 cases of PCI every year. However, the case number has been growing rapidly with the growth of the economy in Malaysia. I heard that they surprisingly had 3,500 cases of PCI last year in IJN. Japan is going more and more behind every year.

The 1st case assigned to me was a gentleman who had in-stent restenosis within the previous bare metal stents placed in the left main artery to LAD. He had also a critical narrowing at the ostium of LCX. Dr. Wasan Udayachalerm from King Chulalongkorn Memorial Hospital, Bangkok, Thailand assisted me. I took a 7-Fr EBU 3.5 guiding catheter from the right radial approach. First, I placed an Endeavor 3.5x18mm drug-eluting stent from LMT to LCX. Then, I did kissing-balloon inflation from LMT to LAD and LCX. Finally, I placed another Endeavor stent in the proximal LAD (T-stenting). Finally, I placed the 3rd Endeavor stent in the distal LAD narrowing. I believe that this case was the 1st case, in which an Endeavor stent was placed distally through the stent strut of another Endeavor stent. Anyway, the case could be finished very quickly.

The 2nd case was very difficult one. He had double CTO lesions in the proximal LAD and the distal RCA. I could not imagine why this patient could survive. By taking the femoral approach, I placed 2 introducers from the right femoral artery. Dr. Asad Pathan from National Institute of Cardiovascular Disease, Karachi, Pakistan assisted me. I first attached the CTO lesion in the RCA. It was quite long and hard. However, I could open the CTO lesion and placed 2 CYPHER stents there. The vascular bed distal to the CTO lesion was strikingly huge. Then, I switched to LAD. However, the proximal end of the occlusion was not clear. After 30 minutes’ attempt, I stopped the procedure. It was already 6:00 PM. However, there were still many audiences watching my procedures in the ball room in Hilton Hotel.

My friend, respectable great Professor Seung-Jun Park was flying back to Seoul by taking the overnight flight on the day. He took 2 secretaries from Cardiovascular Research Foundation, Seoul. I invited him and his colleagues to one of my favorite places in KL to have dinner. That is a kind of street restaurants. The name of the restaurant is “Fatty Crab”. It is not a clean or deluxe place but a real local one. I was a little bit afraid whether Professor Park would like this kind of place or not. I was very happy to know that he and his colleagues liked the place. After finishing the foods, we shared a nice Durian fruit. It was really tasty. The dinner with Professor Park was really memorable and nice one.

We had a breakfast Symposium between 7:00 and 8:00 AM on the day. The symposium was for transradial coronary intervention titled “Try TRI”. The chair person was Professor Sim Kui Hian from Sarawak General Hospital, the Island of Borneo, Malaysia. The 1st Presentator was Professor Haizal Haron Kamar from National Malaysian University Hospital. The 2nd Presentator was Dr. Muhammad Munawar from National Cardiovascular Center of Indonesia, Halapan Kita. I was the last speaker. After finishing the presentation, I hurried to KLIA by taking KLIA Express and took the return flight of Malaysian Airline to Japan on the same day.

This trip to KL was also very fruitful for me. I would like to express for my great appreciation to all of the doctors related to this fantastic meeting.
クアラルンプール訪問記 - II

マレーシア・ライブ2005
私はマレーシア・ライブ2005から招聘外国人ファカルティーの一人として招かれました。 会はクアラルンプール(KL)のヒルトン・ホテルで2005年7月27日から30日まで開催されま した。私は、成田空港7月28日午前10時30分発のマレーシア航空MH 089で出発しました。 KL国際空港(KLIA)に到着したのは午後4時30分でした。数ヶ月前に、KLIAからKL中心部 を結ぶ高速鉄道が開通しました。この列車でKLの中心駅まで30分程度で到着しました。駅の 前にはツイン・ビルがあり、片方がメリディアン・ホテルそしてもう片方がヒルトン・ホテルで す。学会はそのヒルトン・ホテルの6階会場で開催されました。私たちもヒルトン・ホテルに 宿泊しました。部屋にチェック・インしてからRobaayah先生、Rosli先生、Koh先生、Santoso 先生、あるいはKao先生といった有名どころの先生方とお会いしました。その晩は、Endeaver 薬剤溶出性ステントのシンポが開催されました。

私に割り当てられた第一症例は、以前に左主幹部からLADに植え込まれたBMSの再狭窄 で、LCX入口部にもきつい狭窄がありました。バンコクのWasan先生が助手をして下さいまし た。右橈骨動脈より7FrのEBU 3.5を挿入し、まず3.5x18mmのEndeavor stentを植え込み、 その後、LMT-LAD, LMT-LCXにおいてKBTを行い、Endeavorのステント・ストラットを通 して、EndeavorステントをT-stentでLAD入口部に置き、LADの遠位部に3本目のEndeavor ステントを植え込みました。この症例は多分、Endeavorステントのストラット越しにEndeavor ステントを植え込んだ世界最初の症例だと思います。どちらにしても素早く終了しました。

2例目はとても難しい症例でした。患者さんはLADとRCAの2本にCTO病変を持ってい ました。どうしてこの患者さんが生き延びていたのか理解できません。この患者さんに対しては TFIで二本シースを挿入しました。カラチのAsad先生が助手をして下さいました。最初にRCA のCTOを狙いました。CTOは長くまた硬い病変でしたが、幸にも、この病変を空け、CYPHER を2つ植え込むことに成功しました。驚くべきことに非常に大きなRCAでした。LADのCTO にとりかかったのですが、断端がどこか分からず、30分ぐらいトライしてから諦めました。時 刻は既に6時でしたが、たくさんの聴衆が私の手技をヒルトン・ホテルの会場で見守っていま した。

私の尊敬する友人Park教授はその日の夜行便でソウルに帰られるご予定でした。彼は財団 から2人の助手をつれてこられていました。私は彼らを私がKLで大好きなレストランに夕食に 連れていきました。それはいわゆる道端のレストランで、その名を「太った蟹」と言います。と ても綺麗で豪華とは言えない、いわゆる庶民の店です。Park先生に気に入って頂けるかどうか 少し心配でしたが、Park先生も仲間も気に入って下さいましたので、ホットしました。食事を 終えてからは、あのドリアンを食べました。とてもすばらしいドリアンでした。この夕食はとて も思い出に残るものでした。

その日の朝7時から8時には朝食シンポジウムがありました。シンポの内容はTRIに関す るもので、タイトルは”Try TRI”でした。座長はSim先生で、最初の発表者はマレーシア国立大 学病院のHaizal先生、次にジャカルタのハラパン・キタのMuhammad先生、そして最後に私 でした。発表を終えて、KLIA急行に乗ってKLIAに急ぎました。そして、マレーシア航空機の 乗って、その日に日本に帰りました。

今回のKLへの旅行もまたとても有意義なものでした。このすばらしい学会に携わられたす べの先生方に感謝の意を表したいと思います。
