My 1st official role during TCT 2005 was a lecture on PCI for CTO lesions between 16:55 and 17:10 in that evening, which was followed by another role for lecture in the session of TRI between 17:26 and 17:41 in the other room. All of the world-famous Radialists met together in the 2nd session including Dr. Kiemeneij, Dr. Louvard, Dr. Tiff Mann, etc.
Since I was very sleepy due to jet lag, I decided to have early dinner in one of the best Korean Restaurants in USA “Woo Lae Oak” from 18:30.

I am now in charge of JSIC Registry Database. I have been studying computer programming by myself for almost 30 years since 1978. I came in this world through the machine language programming of Z80 CPU by using MZ-80 personal computer made by Sharp Corporation to calculate automatically Sino-Atrial Conduction Time (SACT). After that, I started to read so many books on this field. My current strongest interest in this field is in C++ programming. Now, Japanese Society of Interventional Cardiology is setting up an Internet-based registry system for PCI. Dr. Tamai as the Chairperson of JSIC, Dr. Mitsudo as the Chairperson of the Japanese Association of Cardiovascular Catheter Therapeutics (JACCT), Dr. Kimura as the representative of JACCT and I met together from 7:30 AM in this morning to discuss how to negotiate the PCI Registry system each other. It was a fruitful meeting.

I had a personal meeting for Endeavor trial, since I am the principal investigator for Endeavor-Japan study after 9:00 AM. The most important presentation for me on the day was on the final results of Endeavor-III trial, which was the randomized comparison of CYPHER and Endeavor Drug-Eluting Stents done in US. The primary endpoint of the study was the equality in 8 months’ late lumen loss between 2 groups. However, the result could not show the equality. Late Lumen Loss was smaller in CYPHER group. In this context, the trial was failed. However, the secondary endpoints, which were clinical results such as TLR, binary restenosis rate, and so on, were not different between 2 groups. Also, it was amazing that any of the almost 1,000 patients, who received Endeavor stent during Endeavor-I, -II and III trials, did not have late stent thrombosis between 1 and 9 months after stent implantation. I believe that Endeavor stent is one of the most promising next-generation drug-eluting stents.
I was always thinking how young Cardiologists in Japan especially working in the community hospitals could start to write English articles. Writing English manuscripts are not only writing but also thinking in logical way. I am now one of the editorial board members for several English medical journals including “Catheterization and Cardiovascular Intervention”. I have trained myself to write English medical manuscripts for these 15 years. It was a difficult and hard way. Nobody taught me how to write English manuscripts. I would change the situation. So that, I am now planning to hold the teaching course of “How to write English manuscripts - Becoming an International Interventional Cardiologist”. For this purpose, I met several special people from Duke Clinical Research Institute of Duke University and discussed about my plan. Everybody was so excited about this revolutionary my idea. The meeting was great.

I was in severe jet lag. I waked up very early. I spent time in my room. I had a duty for the Asia-Pacific Case Review Session with Dr. Mitsudo, Dr. Tamai, and other Asia-Pacific doctors in the evening. After finishing this role, I went to one of the best Italian restaurants in Washington DC.

Early morning from 7:00 AM, we had an Investigator Meeting for SPIRIT-III Trial. I joined this meeting, since I am the principal investigator for this trial in Japan. I co-chaired this meeting together with Dr. Gregg Stone and Dr. Campbell Rodgers.
After this meeting, I had to hurry up to another room to participate in TCT Health and Human Services Town Hall Meeting, where Government officials from US-FDA, Japanese MHLW and PMDA as well as from EU met together with physicians or company people in order to talk each other about how new modalities could be introduced in clinical fields as soon as, as efficiently as, as safely as, and as scientifically good as possible. I was one of the discussants there. This meeting was the most important one for me during TCT 2005.
My last role for TCT 2005 was one of the Presentators for CTO lesions with Dr. Mitsudo, Dr. Tsuchikane an Dr. Tamai in the lunch time session. I talked about TRI for CTO lesions.

The dinner of the day was wonderful. Again, I went to the Korean Restaurant “Woo Lae Oak”. There, we had a pleasant dinner with young talented Japanese doctors studying in Cardiovascular Research Foundation in New York and with Dr. Masutani. While having dinner, we met with Dr. SJ Park, who flied to Washington DC from Seoul after transmitting live cases from Seoul. We drank much together. It was a wonderful dinner party.

I returned to Japan by flying on NH 001 flight and came back to Japan in the afternoon on October 21 Friday. It was a long journey for me.

とても長い米国旅行
米国首都ワシントンで2005年10月16日から20日まで、世界最大のライブデモンストレ ーションの一つであるTCT 2005に参加しました。私はこの会では、延吉正清先生に次いで、 日本で二番目にFacultyとなりました。成田から2005年10月16日日曜日午前11:10発の全日 空NH 002便に乗って行きました。同じ飛行機に何人かの顔見知りのお医者さん達が乗られて いました。12時間かけてワシントン・ダレス国際空港に着いたのは、同じ日の午前10時頃でし た。今回のホテルは学会が用意したルネッサンス・ホテルであり、ワシントン国際会議場の目の 前にあります。チェック・インしてから、中華街で点心を昼食に摂りました。日曜日でしたが、 たくさんの中国レストランが開いていました。
昼食の後、別のホテルに移動し、Kiemeneij先生、Louvard先生達と、TRIのシミュレー ターについての打ち合わせを2時間ぐらい行いました。

この学会での最初の仕事は、4:55 PMから17:10 PMまで、CTOに対するPCIの講演を行 うことでした。引き続き、5:26 PMから5:41 PMには、TRIのセッションでの講演がありまし た。TRIのセッションには、Kiemeneij先生、Louvard先生、Tiff Mann先生初め、有名な先生 方が集まっていました。
時差ぼけでとても眠かったので、この日は早めの夕食を米国で一番良い韓国料理店である、 “Woo Lae Oak”で6:30 PMから摂ることに決めました。

私は日本心血管インターベンション学会PCIデータベースの責任者です。自分自身、1978 年からもうかれこれ30年間、コンピューターの独学をしてきました。最初に始めたのは、シャ ープのパーソナル・コンピューターMZ-80に搭載されていたZ80 CPUの機械語のプログラムで した。それをプログラムして、洞房電導時間(SACT)を自動的計算するようにしました。この後、 たくさんの本を読んで勉強しました。現在最も興味ある分野はC++プログラムです。現在、日 本心血管インターベンション学会ではインターネット上に、PCIのデータベースを立ち上げよ うとしています。JSIC理事長である玉井先生、JACCT理事長である光藤先生、そして、JACCT の代表としての木村先生の4人で、7:30 AMより今後PCIデータベースをどのようにうまく纏 めるか、について話し合いました。とても有意義な話し合いでした。

私は、Endeavor-Japan研究の責任医師ですので、この日の9:00 AMから内輪の会に参加し ました。この日行われたもっとも重大な試験結果の発表は、Endeavor-III試験についてのもの でした。この試験は米国で行われ、CYPHERとEndeavorの比較をしたものです。一時エンド ポイントは8ヶ月目のLLLでした。残念ながらこの2群間での同等性は却下されました。しか し、二次エンドポイントである臨床結果、例えばTLRや再狭窄率では差は認められませんでし た。また、素晴らしいことに、Endeavor-I, II, IIIの3つの試験でEndeavorステントを植え込 んだ1,000名ぐらいの患者さんの誰一人として、植え込み後1から9ヶ月の間の後期ステント 血栓症を起こしませんでした。間違いなく、Endeavorステントは次世代DESの中の一つでし ょう。
何時も、どのようにして若い日本の循環器医師、とりわけ市中病院で働いている医師が英語 論文を書けるようにできるか、考えています。英語で論文を書く、ということは、単に英語で書 く、というだけでなく、論理的に考える、ということなのです。私は、今、いくつかの英文医学 論文誌の編集委員会委員になっています。その中の一つはCCIです。自分自身、この15年間、 どのように英文論文を書くか、独学してきました。それは、とても辛く困難な道でした。誰一人、 どのようにすれば良いか、を教えてくれませんでした。この状況を何とかせねばなりません。で すから、今考えているのは、「どのように英語論文を書くか - 君も国際的なInterventional Cardiologistになれる」というような勉強会を開催することです。このために、Duke臨床研究 所のメンバーと会いました。そして、私のプランについて検討しました。皆、この新しいプラン に興奮しました。とてもすばらしい話し合いでした。

とてもひどい時差ぼけでした。とても早く起きてしまいました。そして部屋で時間が流れ ました。夕方にはAsia-Pacific Case Review Sessionで光藤先生、玉井先生達と、役目がありま した。この仕事を終えて、この日はイタリア料理を食べました。

朝早く7:00 AMからSPIRIT-III試験のInvestigator Meetingがありました。私はこの試験 の日本側PIですので参加せねばなりませんでした。Stone先生、Rodgers先生と共に座長をし ました。
この会の後、タウンホール・ミーティングに参加するために急ぎました。この会には、米国 FDA、日本厚生労働省、PMDAそしてEUからも政府関係者が参加しました。勿論、医師そし て企業の人々もです。そして、新たに治療器具を、いかに早く、安全に、効率的に、そして科学 的に正しく臨床現場に導入するか、について討議しました。この会は、私にとってTCT 2005 において一番重要な会でした。私は、この中でDiscussantとして参加しました。
TCT 2005での最後の仕事は、光藤先生、玉井先生、土金先生と共に、ランチョン・セミナ ーでCTOの話をすることでした。私は、CTOに対するTRIについて話をしました。

この日の夕食はとても素晴らしいものでした。再び、韓国料理“Woo Lae Oak”に行きました。 そこで、現在New YorkのCRFで研究されている日本の若い優秀な先生方達、そして舛谷先生 と楽しい夕食を摂りました。その内、SJ Park先生が、ソウルからのライブデモンストレーショ ン中継を終えられて、このワシントンに飛んで来られました。そして、一緒に楽しい食事とお酒 を楽しみました。とてもすばらしい夕食でした。

NH 001便に乗り、日本には10月21日金曜日午後に到着しました。とても長い旅でした。
