After 4 hours’ flight, I arrived at this new airport, which was constructed 2 years ago. After 2 hours’ transit time, I exchanged my flight to a China Southern Airline flight of CZ 3380 to ChangSha, HuangHua Airport, where I have not been yet in my long history of visiting China. One of the interesting foods during the dinner was one of the local foods in HuNang, spicy fermented bean curd. I guess many Japanese people cannot take it because of its strong smell and hot taste. He has 2 cars. He studied in US for 4 years before TiangMen Square Affair. There had been no stents yet. While he was studying the mechanism of restenosis after balloon angioplasty, he trained himself in clinical Interventional Cardiology. He could do angioplasty better than US physicians. He told me that he wondered whether he stayed in US as a physician or came back to China. He finally decided to come back to China. If he stayed in US and did medical practice, he would not be the top physician because of the language problem, although he can speak very fluent English from my standpoint of view. Thus, he came back to his home town ChangSha. Now, his family members are staying in US. </p> <h2> November 23 Wednesday, 2005 </h2> <p> I visited XiangYa Hospital, which was established by US people more than 100 years ago and has the 2nd oldest history among the hospitals in China. The outlook of the building is very historical. Now they are constructing a new big and tall hospital building. There was a monument of the writing by the Past Leader of China Mao ZeDong at the front gate of the hospital. The Chinese character of the 1st part of “XiangYa” is the same with the 1st character of my hospital name “ShoNan”. I asked the meaning of the character of “Xiang”. It was very interesting. “Xiang” means the south of the lake. The name of the province “HuNang” also means the south of the lake. Thus, “Xiang” is same as “HuNang”. There was a local meeting was held in a big auditorium within the hospital. The name of the meeting was “HuNang International Conference on Interventional Cardiology and Live Demonstration 2005”. Local Interventional Cardiologists in ChangSha participated in the meeting. I did one lecture and 3 cases there. After finishing 2 cases with Dr. Li and 1 lecture, we had lunch in the catheter laboratory. During the lunch, I heard that almost 600 cases of PCI were performed in this hospital. The final case had very severe angina and was transferred form the 2nd affiliated hospital of ZhungNang University. The patient had 2 vessels occluded completely. I guessed that the occlusion in the LAD was not chronic but that in the RCA was chronic. By taking the 7-French transfemoral approach, I crossed the lesion in LAD easily by using a floppy guidewire and placed a CYPHER stent there. For the CTO lesion in the RCA, I took a Miracle-3 guidewire. After the careful manipulation of the guidewire, I was successful to cross the lesion. Finally, I placed 3 CYPHER stents in the RCA. On the way to the airport, I could see a gate of dignified appearance. That was the gate of the National University of Science and Technology of Defense. I understood again that ChangSha was a really important city in China. After arriving at ShangHai HongQiao Domestic Airport, I went to a nice ShangHai Crab Restaurant and had ShangHai crabs as dinner. I guessed that almost 80% of customers there were Japanese. The traffic in Shanghai is not good especially in the morning as similar as in the other big cities in China. Shanghai is one of the most competitive cities for PCI, where TRI is still not popular. Dr. Xu and another director Dr. Liu want to introduce TRI to increase the volume of PCI in this hospital. They have had no experiences to do TRI. I did the radial artery puncture for the 1st case. From the next case, I stand by them doing the radial punctures and instructed how to do. It is a really nice area, where we can see and experience old Shanghai atmospheres. The area was crowded by many people. However, all of the people seemed to be not foreigners. I guess that still this area is not exposed to foreign tourists visiting Shanghai. There were a lady Vice Secretary of the Communistic Party Ms. Yang QingMin, the vice President Dr. Shen, Dr. Liu, Dr. Xu, other doctors, technicians and many nurses. The food was excellent. It was my first experience to have river shrimp. The President of YCC 2005 was Dr. Chen JunZhu, who is the Professor of Internal Medicine in Zhejiang University School of Medicine. He is the mentor for Dr. Linda Ye, who is now studying in my hospital. Probably, this was my 4th visit at Hangzhou. Before joining the Congress, I was invited to lunch in a very nice tea house beside the beautiful lake Xi Lake (West Lake), which is the symbol of this historical City Hangzhou. There I had delicious local foods. Boiled bamboo shoot was extraordinarily tasty. I had nice talk with a business man, who is the most successful in China among many companies coming from Japan. As the 1st lecture in during the Congress, I did a lecture on PCI for CTO lesions. Professor Shen in RuiJing Hospital of Shanghai Medical University kindly chaired the session. After finishing my lecture, I met the President of the Congress Professor Chen. I could understand at least one of the reasons why he was respected by all of the physicians in China. He is very calm, gentle, kindly and honest. The secretary lady physician is beautiful and looks intelligent. After I promised Professor Chen to visit his University Hospital next year, I returned to Shanghai by car. On the way back to Shanghai in the free way, I could see a lot of houses characteristic in this area, which have a lightning rod on the top of the pointed roof, since this area is famous for many lightning strikes. I came back to Japan on November 26 Saturday 2005 by taking a JL 620 flight from Shanghai Pudong International Airport to Osaka Kansai International Airport departing at 8:25 AM. After arriving in Osaka before noon, I joined the 30th Osaka Bay Area Conference to talk a lecture. After finishing my lecture, I came back to my home. Also during this journey to China, I could meet many people, who could enrich my experiences. 2005年11月22日木火曜日成田から広州白雲国際空港に飛びました。4時間の飛行の後この新しい2年前に建てられた空港に到着しました。2時間程のトランジットの後、長沙黄華空港行きの国内線に乗り換えました。長沙は私の長い中国訪問の中でも始めて訪れる都市です。長沙は湖南省の省都であり、共産党中国にとって歴史的にとても重要な都市です。この町であの毛沢東主席が育ったのです。</p> <center> </table> </center> <p> 5星ホテル華天ホテルにチェック・インしてから湘雅病院のお医者さん達と夕食を海鮮料理レストランで摂りました。副院長の唐先生、内科部長楊先生、PCIを行う李先生、浦先生達が参加されました。湖南省特産の発酵した辛い豆腐がありました。多分、ほとんどの日本人は、臭くて辛いために食べられないでしょうが、私は好きです。</p> <center> <table 楽しい夕食を終え、楊先生がご親切にもご自分で車を運転しホテルまで送って下さいました。彼は車を2台持っているそうです。彼は天安門事件の以前に米国に4年間留学したそうです。その頃はステントが無い時代でした。バルーン形成術後の再狭窄について研究する傍ら、臨床でもPCIをしたそうです。彼はアメリカ人の医師よりも上手にPCIをすることができました。中国に戻るか、アメリカに残ってPCIを続けるか迷ったそうです。でも、言葉の壁で、もしもアメリカの残ったならばトップ・レベルの医師とはなれない、と考え故郷の長沙に戻ることにしたそうです。現在彼のご家族はアメリカに住んでおられるそうです。</p> <h2>2005年11月23日水曜日</h2> <p> 100年以上前にアメリカ人によって建てられ、今では中国で2番目に古い病院である湘雅病院を訪問しました。病院の外観はとても歴史的なものです。今、新しいとても大きい高層の病院を建造中でした。病院の入り口には毛沢東主席が書いた記念碑がありました。湘雅の湘は私の病院名の湘南の湘と同じ文字です。湘という辞の意味を聞いたところ、面白い答えが返ってきました。何度も、湘という文字は、湖南という意味なのだそうです。してみると、湘南というのは湖の南の南という意味です。</p> 湘雅病院は中南大学付属第一病院でもあります。病院の大きな講堂を用いて地方会が開かれていました。その学会の名前は「湖南国際心血管インターベンション学会」というものでした。長沙のお医者さんが集まりました。私は、そこで一つの講演と3例の手技を行いました。浦先生によって紹介された後、皆で集合写真を撮影しました。</p> <center> <table border="3" </td> </table> </center> <p>レントゲン機械はPhilipsのsingle-plane Integrisでした。李先生と一緒に2例の手技を終え、講義を終えてからカテラボの中で昼食を摂りました。何でもこの病院では年間600例程の PCIが行われているそうです。最後の症例は重症でした。この患者さんは中南大学第二付属病院から移送されてきました。患者さんは二枝閉塞であり、LADの閉塞は比較的最近のもので、 RCA閉塞は慢性完全閉塞と考えました。7 Frの経大腿動脈的冠動脈インターベンションで入り、 LADに関してはフロッピーで通過し、CYPHERを植えました。RCAに対してはMiracle-3を用いて通過し、最終的に3個のCYPHERを植え込んで終了しました。皆、すばらしい結果に満足でした。</p> <center> <table border="3" bordercolor="#3333cc"> <tr> 　 <td> <p class="tbl"><a これらの症例を終え、上海に向かうために空港に向かいました。空港に向かう途中で威厳のある門を見ました。この門は、国家防衛科学技術大学の門でした。なるほど、長沙は中国にとってとても重要な都市なのだと分かりました。上海虹橋国内空港に着いてから上海蟹の有名な店に向かい、そこで夕食を摂りました。お客さんの8割は日本人だったと思います。そして、 Rendezvous Merryホテルに宿泊しました。</p> Shanghai Crabs</p> </td> </table> </center> <h2>2005年11月24日木曜日</h2> <p> 朝6時に起き、ホテルで朝食を摂ってから、厦旦大学付属上海第五人民病院に向かいました。中国の他の大都市同様に上海の圧の交通事情は良くありません。かれこれ1時間ぐらいのドライブの後、ようやく病院に到着しましたが、その一帯は上海の工業地帯です。</p> TRIが普及していない都市の一つです。徐先生および、もう一人のカテ室責任者である劉先生はこの病院のPCI症例数を増やすためにTRIを導入されようとしていました。でも、全くその経験は無かったのです。最初の症例は私が橈骨動脈の穿刺をしました。そして次の症例からは横に立って指導しました。とてもすばやく症例を終えたので予想よりも早く午後早くに終了しました。</p> <center> <table <p>徐先生は私に、車で30分ぐらいの所にある七寶街を訪れるように薦められました。そこは本当にすばらしい場所でした。昔の上海の風景がそのまま残っているのです。たくさんの人々で賑わっていましたが、外国人の姿はありませんでした。まだ、上海を訪れる観光客には知られていないのでしょう。そこで、腐豆腐を食べました。その後、病院に向けて戻り、近くの喫茶店でコンピューターで仕事しながら、病院の人たちとの夕食まで時間を潰しました。</p> 夕食会はとても恐ろしいものでした。病院の女性共産党副書記長楊様、副院長沈先生、劉先生、徐先生、他の先生方、技師さん、そして看護師さんたちが参加されました。食事はとてもおいしかったのですが、とりわけ川海老というのを初めて食べました。夕食会の最後には本当に酔っ払ってしまいました。</p> <center> <table border="3" bordercolor="#3333cc"> <tr> </center> <h2> 2005年11月25日金曜日 </h2> <p> この旅行の一つの目的は杭州市で開催された揚子江心臓病学会2005 (YCC 2005)に参加することでした。この学会がカバーしているのは、杭州、上海、そして南京です。YCC 2005の学会長は浙江大学医学部付属第一病院内か教授であられる陳　君柱教授でした。教授は現在当院で研修されている葉先生の恩師です。残念でしたがスケジュールが合わないため、ライブデモンストレーションを行うことはできませんでした。</p> <center> <table border="3" bordercolor="#3333cc"> <tr> 　 <td> <p class="tbl"><img <p>上海から杭州までは車で2時間半ぐらいかかりました。私にとって杭州は多分4回目の訪問でした。学会に参加する前に美しく歴史に満ちた杭州のシンボルである西湖のほとりにあるとても良いお茶処　「湖畔居」で昼食を摂りました。とてもおいしい郷土料理がありましたが、中でも筍の煮物はとてもおいしい味でした。ここで数ある日本から中国に進出した多くの企業の中でも最も成功した企業の方とお話する機会がありました。私も中国で彼のようになりたいものです。 <p>学会の登録デスクの前で私の古い友人である朱先生と買先生にお会いしました。私の慢性完全閉塞に対するPCIについての講演は学会の最初にアレンジされていました。上海医科大学瑞金病院　沈教授が座長をして下さいました。講義を終えてから、学会長であられる陳教授とお会いしました。どうして陳教授が中国の医師の中でも尊敬されているかが良く分かりました。それは、少なくとも、教授が穏やかで、紳士的で、親切でそして誠実だからです。秘書の女性医師はとても奇麗で聡明な方でした。陳教授に来年病院を訪問する約束をしてから上海に車で帰りました。上海の帰る高速道路の道すがらこの地方特有の家々を見ることができました。とんがり屋根とその先端に避雷針がとりつけてあります。この地域は雷が多いのです。翌日26日のJL 620 便上海浦東国際空港8:25 AM発の大阪関西国際空港行きの飛行機で帰国しました。大阪には正午前に到着し、第30回大阪ベイエリア・カンファレンスに講師として参加しました。講演を終え、帰宅しました。今回の中国訪問でも多くの人々とお会いできました。これらの人々によって私の経験は豊かになりました。皆さんありがとう。</p> </p> <center> <table border="3"
