The 6-times Visit in Moscow

My 1st visit in Moscow was on June 4th 2002. This time, I flied to Frankfurt Airport by NH 209 flight departing at 11:35 AM on June 07 Wednesday 2006 from Narita Airport. After 1 and half hours’ transit time, I transferred to Lufthansa LH 3188 flight to Moscow. After 3 hours’ flight, I finally reached Moscow Sheremt International Airport around 11:00 PM on the same day.
As always, I moved to Cafe Pushkin to have late dinner. This restaurant was crowded with many Russian rich people. After I had Black Caviar and Beef Storganov, I checked in the Sheraton Palace Hotel Moscow around 2:00 AM on Thursday.

Good Performance in Moscow

As similar to the past several years, I came to Moscow as one of the invited faculty members to the 7th Moscow International Course on Endovascular Surgery of Congenital and Acquired Heart Diseases, Coronary and Vascular Pathology, which is organized by Professor B.G. Alekyan of Bakoulev National Cardiovascular Center. It was colder than the previous years. Since the economy in Moscow was very nice, there were many buildings under construction. I left the hotel at noon for Bakoulev Cardiovascular Center. It takes more than 30 minutes by car, and we cross the Triumphal Arch and Triumphal Memorial Park, which are the symbol for the triumph against the German attack during the World War II.

First, I saw Professor Alekyan and Dr. Yves Louvard in the Professor’s room. We talked about several matters. Many European famous doctors were also invited as the faculty. Everybody was very familiar with me, especially after the great activity for the difficult CTO case performed by myself from Rotterdam during the last EuroPCR meeting. After delivering my lecture on PCI for CTO lesions from 2:00 PM, I moved up to the 9th floor, where the catheter laboratories are installed. Professor Alekyan prepared for 2 CTO cases. The 1st patient was a 60++ years old gentleman and had a long CTO lesion in the very tortuous RCA. The previous attempt was failed. This time, I brought 7 French short guiding catheters. However, I did not bring any microcatheters. I could not find any good retrograde routs for this patient. The antegrade approach seemed very difficult due to the heavy tortuosity of the RCA. After several attempts, I gave up this case. Anyway, it is very important that I finished the case without any complications. Angioplasty for CTO lesions are not performed in an emergency situation. The patient has the right to receive other kinds of treatment. If we would induce any severe complications and take away the chance of the alternative treatment strategy from the patient, it would be the most prohibited scenario. I hear very often in Japan that some cases with CTO lesions result in severe complications after long and failed procedures. I also hear that some of them are lost. As a doctor, I would never do such a mal practice.

The 2nd case was a 40++ years old gentleman with severe angina pectoris. He has 2 CTO lesions both in proximal LAD and RCA. Previous twice attempts in June and November 2005 were failed. Dr. Staferov Anton assisted me. I first tried to open the CTO lesion in LAD. After 5 minutes’ attempt, I could cross the lesion and finally placed 2 DES stents. Actually, this case was the 1st case of TAXUS stent implantation in Russia. Then, I moved to the RCA. By using a Miracle 3 guidewire, I could cross the proximal half of the CTO lesion. The guidewire passed through the proximal part into a big Conus branch. While keeping the Miracle 3 guidewire in the Conus branch, I took a Miracle 12 guidewire. The distal half of the CTO lesion was very hard. I took a Conquest-Pro 12 guidewire as well. Although I had no microcatheters or OTW balloons, I did the double guidewire technique. Finally, the Miracle 12 guidewire passed the hard distal half of the CTO lesion.

However, I could not cross the lesion by using a 1.5-mm balloon. There were a lot of excited questions and comments from the auditorium. I did the Anchoring-Balloon technique by placing a 2.5-mm balloon inflated in the Conus branch. Finally, I could cross a 1.5-mm balloon through the lesion. The final result after placing 2 CYPHER stents was beautiful. Thus, I could treat this young patient with sever effort angina due to double CTO lesions both in LAD and RCA in 1 hour. The case was of course the best case during the meeting.

I visited a Japanese Restaurant “Kamakura”. The symbol of Kamakura City “Sitting Great Buddha” was displayed in the entrance. Since the faculty dinner was held in the Russian Restaurant “Godunov” and I was of course invited, I had to move. Since the traffic in Moscow was so bad, I took the subway train. At the 2nd stop, I had to change the train. Finally, I could reach the “Bolshoi Theater Station”, from where we can reach Godunov on foot. Bolshoi Theater was under large-scale renovation. After saying “Hello” to everybody, I left Godunov and visited a Georgian Restaurant. There I enjoyed live Georgian music and nice food. Two beautiful young Russian ladies were enjoying dinner at the table next to us. The Georgian singer was also beautiful. I was happy to meet with these beautiful ladies in Moscow.

June 09 Friday 2006

I left the hotel around 11:00 AM. After short sightseeing around the Red Square, I had lunch in one of the best Russian Food Restaurant “Red Square”. Again I had Beef Storganov and Borshch soup. Because the traffic in Moscow in Friday afternoon is terrible, it took almost 2 hours from the restaurant to the airport.

I came back to Japan by taking LH 3185 flight between Moscow and Frankfurt and NT 210 flight from Frankfurt to Narita. Finally, I reached Tokyo around 3:30 PM on June 10 Saturday 2006. The main differences which I could see this time compared to the previous trips to Moscow are:

  1. A lot of buildings under construction.
  2. Many Japanese businessmen flying from Frankfurt to Moscow.
  3. More improved Internet connection.
  4. Increased price for restaurant food.

On the contrary, I could see nothing changed:

  1. Very friendly and comfortable atmospheres of people in Bakoulev Center.
  2. Good taste of Russian food.
  3. Unfriendly passport control in Moscow Airport.
  4. Insufficient preparation for guiding catheters, microcatheters or OTW balloons in the catheter laboratory.
  5. Too long flight between Tokyo and Moscow by using Frankfurt route.

Next time, I will fly to Moscow by using direct flights. It is better to save time.

6回目のモスクワ訪問

私が最初にモスクワを訪問したのは2002年6月4日でした。今回、私は2006年6月7日 水曜日11:45 AM成田発のNH 209便に乗ってフランクフルトに飛びました。そこで1時間半の トランジットの後で、モスクワ行きLH 3188便に乗り換えました。3時間の飛行の後、モスク ワ・シュレムト空港に着いたのはもう11:00 PMになっていました。
いつものように、そのままカフェ・プーシキンに向いました。ロシアの裕福な人々で込み合って いました。ここでキャビアとビーフ・ストルガノフを食べてからシェラトン・ホテルにチェック・ インしたのはもう翌日木曜日の2:00 AMでした。

モスクワでの活躍

過去数年と同様に、私は、バコレフ心臓病センターのAlekyan教授によって開催されてい る学会のファカルティーとして招聘されモスクワに来ました。今回は第7回です。いつもより 天気は寒く感じました。モスクワの経済はとても良いので、街にはたくさんの建設中のビルがあ りました。昼頃にホテルを出て、バコレフ心臓病センターに向いました。車で30分はかかるの ですが、途中に第二次大戦の時にドイツ軍を撃退した記念の凱旋門や戦勝記念公園を通ります。

まず、Alekyan先生の教授室でAlekyan先生やYves Louvard先生にお会いして色々な話を しました。たくさんのヨーロッパ中の有名な先生方が今回も招聘されていましたが、あの EuroPCRで私がロッテルダムで行った難しい慢性完全閉塞の症例の後ですから、皆私のことを 良く知っていました。2:00 PMから慢性完全閉塞に対するPCIの講義をした後、カテ室のある 9階に移動しました。Alekyan教授は私に慢性完全閉塞を2例用意されていました。一例目は 60歳台の男の方で、蛇行の強い右冠動脈の慢性完全閉塞症例です。これまでにもPCIが試みら れましたが不成功でした。今回私は7 Frの短いガイディング・カテーテルを持参しましたが、 マイクロカテは持って行きませんでした。この患者さんでは良い逆行性ルートを見つけることが できませんでした。順行性ルートは蛇行が厳しいので困難に思えました。しばらくトライしてか らこの症例は中止しました。何れにしても、何らの合併症無く終了したことが重要です。慢性完 全閉塞に対するPCIは緊急で行われるものではありません。患者さんは他の治療法を受ける権 利を持ちます。もしも、重大な合併症を引き起こし、その結果他の治療法を受ける機会を失って しまうことになるとすれば、それは最も許されざる医療行為でしょう。日本国内で、とても長い 手技の挙句に合併症を起こして失敗に終わる慢性完全閉塞症例の話を時々聞きます。しかも、そ のような症例の中には命を落とす方もあるそうです。私は医師として、このような悪い医療をす るつもりは全くありません。

二例目は40歳代のきつい狭心症患者さんでした。彼は左冠動脈前下行枝と右冠動脈の両方 に慢性完全閉塞病変があり、2005年6月と11月の二回に渡ってPCIが試みられましたが不成 功でした。Dr Antonが私の助手をして下さいました。まず左冠動脈前下行枝の慢性完全閉塞か ら入りました。5分ぐらいで病変をクロスすることができ、そこに薬剤溶出性ステントを2個植 え込みました。実際、この症例がロシアで最初のTAXUS植え込み症例となったのです。ついで、 右冠動脈に移りました。Miracle 3を用いて、右冠動脈慢性完全閉塞の手前半分をクロスするこ とに成功しました。ガイドワイヤーは大きなConus枝に入りました。そこにこのガイドワイヤ ーを残したまま、Miracle 12を用いました。慢性完全閉塞の残り半分は非常に硬く、 Conquest-Pro 12も取り出しました。マイクロカテーテルもOTWバルーンもありませんでした が、Double Guidewireテクニックを用いました。ついにMiracle 12が慢性完全閉塞を通過しま した。

しかし、1.5-mmバルーンは病変を通過できませんでした。会場からはたくさんの質問や意 見がもたらされました。私は、2.5-mmバルーンをConus枝に置いてAnchoring-Balloonテク ニックを用いました。これによって1.5-mmバルーンが病変を通過し、最終的にCYPHERを2 個植え込んで終了しました。かくして、私はこの若い左前下行枝と右冠動脈に2ヶ所の慢性完 全閉塞病変を有する重症狭心症患者さんを1時間以内で治療しました。この症例はもちろん、 会期中のベスト症例でした。

日本食レストラン「鎌倉」に行きました。鎌倉市のシンボル「鎌倉大仏」が入り口に掲げら れていました。その夜はファカルティー・ディナーがロシア料理レストラン「ゴドゥノフ」で開 催され、もちろん私も招待されていまし。モスクワの交通渋滞はひどいので地下鉄を使って移動 することにしました。二つ目の駅で路線を乗り換え、ボリショイ劇場駅に着きました。そこから 「ゴドゥノフ」までは歩いてすぐです。ボリショイ劇場は大規模改装中でした。皆さん方に挨拶 してから、グルジア料理のレストランに移動しました。ここでは生演奏とおいしいグルジア料理 を楽しめます。私たちのテーブルの隣に若い綺麗なロシア女性二人組みが食事していました。グ ルジアの歌手もとても綺麗でした。モスクワで綺麗な女性に会えて幸せです。

ホテルを11:00 AMに出ました。赤の広場周辺を少し観光した後、最上級ロシア料理レスト ラン「赤の広場」に行きました。ここでもビーフ・ストルガノフとボルシチ・スープを頼みまし た。モスクワの金曜日午後の交通渋滞はひどいので、空港まで2時間かかりました。

モスクワ-フランクフルト間はLH 3185便、そしてフランクフルト-成田間はNH 210便に 乗って成田に戻りました。最終的に到着は2006年6月10日土曜日の3:30 PM頃になっていま した。今回の旅で感じた違いは以下の通りです。

  1. たくさんのビルが建設中である
  2. フランクフルトからモスクワにたくさんの日本人ビジネスマンが飛んでいる
  3. インターネット接続状況が改善している
  4. レストランの値段が高くなっている

一方で何も変わらなかったのは

  1. いつもとかわらずとても親切で快適なバコレフ心臓病センターの人々
  2. とてもおいしいロシア料理
  3. モスクワ空港のぶっきらぼうの入国管理官
  4. カテ室にあるガイディング・カテーテル、マイクロカテーテルあるいはOTWバルーンが不 十分
  5. 何しろ、東京からフランクフルト経由でモスクワに入るのは長すぎる

次回は、モスクワとの直行便を使うつもりです。時間を節約しましょう。
