GuangXi Zhuang Autonomous Region

Flying by NH 923 flight departing at 9:50 AM on June 26 Monday 2006 from Narita Airport, I first moved to GuangZhou Airport. After spending almost 3 hours there, I changed my flight to CZ 3331 to Nanning in GuangXi Zhuang Autonomous Region. Before arriving Nanning Airport, I could see the scenery of the field characteristic in this area. Nanning is the Capital of this region, although the most famous city of the region is GuiLing. From the Nanning Airport, we moved directly to a Restaurant, where we had dinner together with doctors from People’s Hospital. I stayed at YongJiang Hotel. From my room I could see beautiful YongJiang River as well as YongJiang Bridge.

The People’s Hospital of GuangXi Zhuang Autonomous Region

I was invited to the TRI Live Demonstration Course held in the People’s Hospital next day. The hospital is a huge hospital. I guess this hospital must be the biggest one in China. After the opening ceremony in the auditorium, I moved to the catheter laboratory with Dr. Wang and Dr. Ling, where a Philips machine was installed. After discussing while watching the diagnostic angiograms, I started the first case.

The 1st case was not an easy case. However, I could finish that case very quickly within less than 30 minutes from the beginning of the radial puncture to the hemostasis. After finishing this case, everybody was surprised to know how quickly the cases could be done through the radial approach. We took a picture with the patient. During the lunch time, sweet and tasty fruit Lychee was served.

The final case was terrible. After crossing the CTO lesion in the proximal LAD and putting stent there, I saw the linear dissection extending to LMT from the stent implantation site. I placed another stent from LMT to LAD. However, after the kissing balloon dilatation in LMT-LAD and LMT-LCX, the flow to both LAD and LCX decreased. I could not understand what happened. There was no dissection visible. We placed IABP. Fortunately, the patient’s condition recovered after the insertion of IABP. After finishing 4 cases, I left the hospital and moved to Beijing by HU 7185 flight.

June 28 Wednesday 2006

I stayed at Grand Hyatt Hotel Beijing. I left the hotel to visit Beijing Friendship Hospital. By 8:30 AM, I arrived at the catheter laboratory and did several cases of TRI there. After finishing those cases, I saw Professor Jia SanQing to discuss about several issues. I came back to Japan flying by NH 906 flight.

2006年6月26日9:50 AM成田発のNH 923便で広州空港に向いました。そこで3時間ぐ らいの待ち時間の後、広西壮族自治区南寧(Nanning)に CZ 3331便で向いました。南寧空港に 到着する前には窓からこの地域に特徴的な風景が見えました。 南寧はこの地区の省都ですが、桂 林の方が有名かも知れません。空港からレストランに直行し、人民医院の先生方と夕食を摂りま した。 ホテルはYongJiangホテルでしたが、窓からはYongJiang河と大橋が見えました。

翌日、人民医院で開催されたTRIライブデモンストレーションに招かれました。この病院 はとても巨大で、もしかして中国国内最大の病院からも知れません。講堂での開会式の後、王先 生と林先生と一緒にカテ室に移動しました。カテ室にはPhilipsの機械がありました。まず、症 例を検討してから第一例目に入りました。

一例目はやさしい症例ではありませんでした。しかし、この症例を橈骨動脈穿刺から止血ま で30分以内にすばやく終えることができました。この症例を見て、参加者は皆、TRIによって 如何に素早く終えることができるかを知り驚いたことと思います。終えてから患者さんと一緒に 写真を撮りました。昼食時にはとても甘いライチが出てきました。

最後の症例は恐ろしい症例でした。LAD近位部のCTO病変をクロスしてから、ステント を入れました。しかし、逆行性解離が起こり左主幹部にも及びました。そこで、左主幹部にもス テントを入れ、LADとLCXに対してKBTを行いました。しかし、その後、LADとLCXへの 血流が低下しました。何が起こったのかさっぱりわかりません。大動脈内バルーン・パンピング を挿入しました。幸運にもIABP挿入後に患者さんの状態は好転しました。4例を終えてから、 HU 7185便で北京に向いました。

北京ではグランド・ハイアット・ホテルに宿泊しました。朝ホテルを出て、友誼病院を訪問 しました。8:30 AMまでには到着し、数例をTRIで治療しました。これらの症例を終えてから、 Jia先生と幾つかのことを話ししました。NH 906便で帰国しました。
