The beginning of a round-the-world trip

It was the beginning of the hard round-the-world trip in 7 days. I flew to Atlanta by the Delta Airline DL 056 flight departing at 15:30 on July 19th Wednesday 2006, where I exchanged my flight to DL 105 to San Paulo, Brazil. I arrive at Narita Airport more than 2 hours earlier than the departure time, since I had to pick up my flight tickets at the Delta Airline front desk. My flight tickets were arranged by the Congress and pre-paid from Brazil. That was why I almost missed to get Brazilian Visa. Normally, whenever I travel abroad, there are some tourism agencies. And, when we need Visa for the entry into some country, they notice us for the necessity to get it. However, it was only 7 days before my scheduled departure when I realized that Brazilian Visa would be necessary. It was already impossible to get Visa in Japan. Our side as well as the Congress Secretariat and President pushed Brazilian Embassy, and finally, I could get Brazilian Visa in Kuala Lumpur on July 14th Friday during M’sia Live 2006 just only in one day.

Actually, it was my first time to take Delta Airline for the international travels. I supposed that I could definitely use AC power as well as in-flight Internet connection in the business class. However, it was really surprising that the reality was totally different. I could use neither AC power nor Internet connection during my Delta flights for more than 12 hours. Seat TV program was also strange. There were several Japanese movie programs. However, all of the movie programs were not on-demand! I guess those aircrafts (Boeing-777) in Delta Airline are 10-years old.

Also, I was surprised to know that we had to go through the passport control when I arrived in Atlanta Airport. I could not understand why we had to go through the passport control. We were definitely transit passengers. I do not know any other countries where transit passengers have to go through the passport control. Anyway, it took time.

I had to wait for more than 4 hours for the flight connection on the schedule. Business class lounge was not bad. I could use wireless Internet connection there, although it was possible because I and an account for @hotspot.ne.jp. In the lounge, I prepared for my presentation for “TRY TRI”. Anyway, I prepared myself for the possible hard situations during the long flight. My clothing style was rough as always. I put short pants, T-shirts and sneakers on me.

Gate open for DL 105 flight was on time. However, after all of passengers sat on the seats, we had to wait for more than 1 hour. The announcement told, “There were some troubles in baggage loading.” Finally, the aircraft left the Atlanta Airport. Why I was flying for Brazil? It was because I was invited as an on of the International Faculty members by XII CONGRESS of SOLACI meeting held in Porto Alegre between July 19th and 23rd. SOLACI is “SOCIEDAD LATINOAMERICANA DE CARDIOLOGIA INTERVENCIONISTA: The Society of Latin American Interventional Cardiology”. It is held one a year in one of the South American countries. It is the biggest meeting for Interventional Cardiology in South Latin American countries. This was my 2nd time to be invited by SOLACI. The last time was in 1999 and held in San Tiago, Chile. At that time, before moving to San Tiago, I spent 2 nights and 3 days in Brazil and held an in-hospital live demonstration course for TRI in Sant Cruz Hospital, San Paulo. It was a small meeting. However, more than 50 Brazilian Interventional Cardiologists joined. I clearly remember that still now. Dr. Otsubo, Dr. Takimura and Dr. Arie were working there. Especially, Dr. Arie was actually the father for all of the Interventional Cardiologists in Brazil as well as Professor Sousa. After watching my procedure, Dr. Arie praised me for my skill. He told me, “I have never seen the quick-hand Interventional Cardiologist as you.” This praise to me by Dr. Arie is an important medal for me. Thus, this visit to Brazil was the 2nd time for me. I had forgotten that I needed Brazilian Visa at that time.

Because the departure of the Delta flight for San Paulo was delayed for more than 1 hour, I could reach the San Paulo GUARULHOS International Airport later than 7:20 AM on July 20th Thursday 2006. My next domestic flight to Porto Alegre was supposed to depart at 7:50 AM. Delta people at the arrival gate told me, “It is impossible to take the 7:50 AM flight. We already booked 2:30 PM flight to Porto Alegre for you. First, you have to pass the immigration and custom controls, and then you have to take a bus to the Domestic Airport.” I could not understand the situation. Anyway, I hurried up to pass through the passport and custom controls. However, there was incredibly long cue. It was already later than 8:00 AM, when I came out from those controls. I switched on my GSM cellular phone, and made several telephone calls to the related people including Mr. Ueno in TERUMO Latin America, who was also joining the Congress. Although TERUMO was not sponsoring me for the Congress, he kindly and quickly made a contact with a travel agency in San Paulo.

By telephone, I was informed that I might take the 11:00 AM flight to Porto Alegre from the CONGONHAS Domestic Airport. I was waiting in the crowded lobby in the International Airport. Suddenly, my name was called by the announcement of the Airport Hall. I could not catch who were calling me. I spoke to a gentleman standing by me in English. Suddenly, he replied to me in very fluent Japanese. I talked him what happened on me. He kindly took me to the guy who called me. That Japanese speaking Portuguese gentleman was waiting for tourists group from Japan. If I did not meet him, I could not meet a guy who called me. Thanks a lot. The guy who called me was a Swiss Airline man. He was asked to take me to the car by a local travel agency, which was taking me to the Domestic Airport. Finally, I could reach the CONGONHAS Domestic Airport at 9:45 AM and check in the 11:00 AM flight. I heard that the distance between the 2 airports were more than 40 Km. Because Varig Brazil Airline Company became almost bankrupt, all of the airports in Brazil were heavily crowded and confused. I was lucky to take the 11:00 AM flight.

After 1 and half hours’ flight, I finally reached Porto Alegre. My lecture on TRI had been scheduled during the lunch-time session on the day. At the airport lobby, Mr. Ueno was waiting for me. We hurried up directly to the Fiergs Convention Center and arrived there before 1:00 PM on July 20th (1:00 AM on July 21st in JST). It was lucky for me that the program was running behind the schedule by half an hour. However, I was wearing short pants and rough shirt. I came in the auditorium. There were more than 200 people, and everybody was wearing the completely formal wears. I sat in the front line of the seats, and then I did my lecture on TRI. For the initial slide, I stressed that I had finally reached there 40 hours’ travel after I left my home. After my lecture, we had questions and answers. The session was wonderful. Again, I was lucky to carry out my duty. After my lecture, a nurse came to me. She was assisting me, while I did live cases 7 years ago in Santa Cruz Hospital. I was shocked to hear that Dr. Arie passed away in 2002 because of liver cancer.

After the session, we went to an Italian Restaurant with Dr. Gilberto Nunes, Dr. Fumiaki IKENO, Mr. Akamatsu and the President Sampei of TERUMO Latin America, and other TERUMO people.

After the lunch, I finally checked-in the Sheraton Hotel, which was prepared by the Congress for me. It was already 4:00 PM, which meant it was 4:00 AM on July 21 in JST. Thus, already 42 hours had passed after I left my home. I was really tired. I had short sleep. From 8:00 PM, we had dinner together in a Japanese Restaurant “SAKURA”. Because of my long flight and jet lag, I slept down in the end of the dinner. I talked to Dr. Ikeno about my idea to visit the famous hospitals like Dante Pazzanese Cardiology Institute to advertise TRI as well as Japanese approach for CTO lesions, where the first several cases of implantation of DES CPYPHER had been performed, and the famous Professors Dr. J. Eduardo Sousa as well as Dr. Alexandre Abizaid are working.

Dr. Ikeno graduated from Jichi Medical School. After his graduation, he spent several days in a local hospital “SAKUMA Hospital” in Gifu Prefecture. While he was working, he learnt English conversation from a Canadian lady teacher, who visited a junior high school of that region. After SAKUMA Hospital, he spent several years in TOYOHASHI Heart Center, and went abroad to study in Stanford University Hospital in US. He studied hard and fortunately got the right of permanent residency in US by drawing lots. He knows well Dr. Abizaid, who is one of the core members of SOLACI meeting. Thus, his name was put in the International Faculty List of XII SOLACI meeting. He is a quite active and very nice guy.

July 21st Friday 2006

I waked up at 4:00 AM and prepared for my lecture on PCI for CTO lesions. Leaving the hotel around 7:30 AM, I reached the FIERGS Convention Center. My lecture was scheduled from 8:00 AM. Because of the troubles in Video Projectors, the start was delayed. After finishing my presentation, I walked around the exhibition area. I found my name in the International Faculty List. It was my great honor that my name was put in the list together with those famous doctors as Drs. Eberhard Grube, Eulogiao Garcia, Giancarlo Biamino, Horst Sievert, Igor Palacios, Jean Marco, Jeffrey Moses, Marco Costa, Martin Leon, Patrick Whitlow, Ron Waksman, Stephen Ellis and others.

Most device companies were displaying in their own booths in the exhibition area. I found booths from TERUMO, NIPRO and “A” Company from Japan. It was very strange for me. Although there was one Japanese man standing without doing any work in the”A” booth, he had never greeted me. I guess that he was just working in US only by following the instructions from Japanese Headquarter. I am sure that he never did any good and productive work, although many other Japanese businessmen are developing new fields by themselves even in US. I also found a booth of Microport China. This company will definitely come in Brazil and Latin American countries.

After walking around the exhibition area, I came back to my room. It was interesting that there were the lanes only for bus, which were completely separated from the other lanes. After spending a few hours in my room, I left for the Clinical Hospital of the National University of Porto Alegre before 1:00 PM in order to do live case transmission. The hospital was huge. There was a bank in the entrance hall.

I did not know what kinds of case(s) I had to do for live transmission. In the catheter laboratory, I met Dr. Liliana Grinfeld from San Paulo and Dr. Marco Wainstein of this hospital. Dr. Grinfeld joined my live demonstration 7 years ago in Santa Cruz Hospital. They introduced me the case. The patient was a 50++ gentleman with silent inferior ischemia. The diagnostic angiography showed the CTO lesion of 2 CM in the middle part of RCA. There was a good collateral route through the left atrial branch from LCA. However, it was too much cork-screwed for the retrograde approach. I decided to take the bi-lateral femoral approach by using 7-French guiding catheters. The X-ray machine was a GE-digitalized machine. However, the storage device was not CD but cine films. We started the case from 1:30 PM, since the s transmission was scheduled to start from 2:00 PM. By using a combination of a 7-French JR4.0 guiding catheter, a Progreat-alpha microcatheter and a Miracle-3 guidewire, I could pass the CTO lesion in 5 minutes. We had to wait for the beginning of the transmission. However, the previous lecture speaker did very long talk, so that the start was delayed by 30 minutes. The chairperson for my live transmission session was Professor Eduardo Sousa. It was really my great honor that I could do a live case just in front of the great Professor Sousa. I could finish the case by placing 3 CYPHER stents successfully. We could hear a storm of applause.

I did not know that the relative of the patient was a nurse in the hospital. After the successful procedure, the family came to me. Dr. Grinfeld left for the airport after this procedure. The transmission was through microwave. The control room for the live transmission was quite simple. However, everything during the transmission was perfect. Dr. Marco proposed me to do another case under off line. I reversely proposed him to do by TRI. While waiting for the next case coming, we discussed about several matters. I heard that he studied Interventional Cardiology as a clinical fellow in Massachusetts General Hospital for 2 years 5 years ago. He is a very excellent Interventional Cardiologist

I quickly did the 2nd case through the right radial approach by using a 6 French guiding catheter. The patient was a lady patient. I put 2 BMS stents in her LCX lesion. Because the patient had only the Government Insurance, we could not use DES. All of the catheter laboratory staffs were happy for the successful results as well as transmission. After taking a memorial photo, I left the hospital.

We went to a Brazilian steak restaurant for the dinner with Dr. Ikeno. Dr. Ikeno was in the auditorium during my live transmission. He told me that Professor Sousa was deeply impressed by my live transmission. I was excited to think that I might have good relationship with famous Brazilian Interventional Cardiologists. During the dinner, we found beautiful Brazilian ladies. We drunk traditional Brazilian tea and high-alcohol spirits as well. We enjoyed the dinner party by almost mid night.

July 22nd Saturday 2006

I checked out the hotel at 7:30 AM and moved to the airport with Dr. Ikeno and Mr. Ueno. We took the flight to San Paulo, which arrived around 11:00 AM. Because the departure time of my next flight to Amsterdam was 6:35 PM, we had enough time in San Paulo. We decided to enjoy the short sightseeing. First, we visited the 9th Japan Festival, which was held in those 2 weeks. San Paulo has the largest Japanese community in the world outside Japan. Many Japanese big companies were sponsoring the festival. There were temporal street restaurants run by the group of people emigrating from each Prefecture in Japan.

After the festival place, we moved to the building of the Brazilian Society of Japan Culture, where the 21st all Brazil Japanese Karaoke Championship was held. Just in front of the building, there is a famous Japanese Restaurant “KIDOAIRAKU”. We had lunch there. After the lunch, we visited the Museum of Japanese Immigrants in Brazil, which were in the 6th to 8th floors of the Japan Culture Society building. I was very impressed to know that the current Her Majesty the Empress Michiko had been Brazil already 3 times in order to appreciate the great pains that Japanese immigrants had taken. The year of 2008 will be the 100 anniversary for Japanese emigrants to Brazil.

Along the street between the Japan Culture Society and the Liverdade Subway Station, there are many Japanese restaurants, food shops or goods shops. It was my first time to see the Japanese characters on the signboard of the restaurant McDonald’s in the foreign countries outside Japan.

Before going to the airport, I visited Santa Cruz Hospital. We passed near the Dante Pazzanese Cardiology Institute. Already 7 years has passed after my last visit in Brazil. I swore to come back again soon to propagate TRI and Japanese techniques for CTO angioplasty in this country. By taking the highway memorial for Ayrton Senna, I reached the International Airport.

By taking KLM Airline KL 0798 flight departing at 18:35 from GUARULHOS International Airport, I flew to Hyderabad, India via Amsterdam Schiphol International Airport. This trip to Brazil was really hard one. However, it was tremendously wonderful and fruitful one. I will definitely come back many times to this lovely and promising country.

世界一周旅行の始まり

世界を7日間で一周する長い旅の始まりでした。2006年7月19日水曜日15:30発の Delta 航空DL 056便でアトランタに飛び立ちました。そこで、サンパウロ行きのDL 105便に 乗り換 えました。成田空港には出発の2時間以上前に到着しました。何しろ、学会がブラジルから 航 空券代を支払い、それを空港のDelta航空カウンターで受け取らねばならなかったからです。 通常は旅行代理店を通して航空券を購入しますが、今回は違います。従って、 ブラジル 入国にビ ザが必要なんてこと知りませんでしたので、それに気がついたのは出発予定の 7日前になって いました。もう、日本国内でビザを取得するのは不可能です。学会事務局や 学会長と共にブラジ ル大使館に申し入れし、ようやく7月14日金曜日一日でマレーシア・ライブ の最中にクアラル ンプールのブラジル大使館でビザを取得することが出来ました。

実際今回がDelta航空の国際線に乗るのは初めてでした。ビジネス・クラスの座席ではAC 電源もインターネットも使えるだろうと思っていたのですが、驚くべきことにそうではありませ んでした。12時間以上のDelta航空の飛行機の中でAC電源もインターネットも使えないので す。また、座席のテレビもとんでもないのです。幾つかの日本語プログラムがあるのですが、何 とオン・ディマンドでないのです。きっと、Deltaの飛行機(ボーイング777)は10年ぐらい古い のでしょう。

またまた驚くべきことに、アトランタ空港でのトランジットではいちいち米国の入出国をせ ねばなりませんでした。どうして出入国手続きをせねばならないか理解できません。だって、完 璧にトランジットなのですよ。トランジットなのに出入国を強いる国なんて他に知りません。と ても時間がかかりました。

予定の次のフライトまで4時間以上待たねばなりませんでした。ビジネスクラス・ラウンジ はそんなに悪くありませんでした。そこではワイヤレス・インターネット接続が使えました。も っとも、私が:@hotspot.ne.jpのアカウントを所持していたから可能なのですが。そこで、自分 の講義資料「TRY TRI」の準備をしました。まあ、こんなこともあろうかと、予め準備してい ました。とてもラフな格好で、短パン、Tシャツそしてズックといういでたちでした。

DL 105便のゲートは定刻通り開きました。でも、全員が乗り込んだ後も1時間以上出発 し ませんでした。アナウンスで「荷物の収容に問題が発生しました」と、言っていました。ようや く 飛行機はアトランタ空港を飛び立ちました。どうしてブラジルまで行ったのかってですか? 　 それは第12回SOLACI学会から招聘されたからです。今回この学会は7月18日から23日ま で Porto Alegreという街で開催されました。 SOLACIとは” SOCIEDAD LATINOAMERICANA DE CARDIOLOGIA INTERVENCIONISTA: The Society of Latin American Interventional Cardiology”のことで、年1回南アメリカの国で 開催されます。何れにしてもこの学会は南アメ リカ・ラテンアメリカ地区で最大のインターベンションの 学会です。私にとってSOLACIから 招聘されるのは今回で2回目です。前回は1999年で、 チリのサンチャゴで開催されました。そ の時はブラジルで2泊3日過ごし、サンパウロのサンタクルス病院 でTRIの院内ライブを開催 しました。会は小さなものでしたが、50人以上のブラジルの インターベンションの先生方が集 まりました。その時のことは今でも鮮明に覚えています。 Dr. Otsubo, Dr. TakimuraそしてDr Arieがおられました。特に有江先生はSousa先生と並んで 事実上ブラジルのインターベンショ ン医師全ての父でもあります。有江先生は私の手技を 見てから私に、「君のように手の早い術者 は今まで見たことが無い。」と、賞賛して下さいました。 有江先生のこの言葉は僕にとっての勲 章です。従って、私のブラジル訪問も今回で2回目でした。 この時にブラジルのビザが必要だ ったなんてことはすっかり忘れていました。

サンパウロ行きのDelta便出発が一時間以上遅れたために、サンパウロのGUARULHOS 国際空港に到着したのは既に2006年7月20日木曜日の7:20 AMを回っていました。私の次の Porto Alegre行きの便は7:50 AM出発の予定でした。到着ゲートのDeltaの職員は私に、「もう 7:50 AMの便に乗るのは不可能だ。Porto Alegre行きの2:30 PMの便を予約したから、まず入 国審査してから、バスで国内線空港まで移動しないと駄目です」と、言いました。何が何だかさ っぱり分かりませんでした。何れにしても、入国審査と税関審査に急ぎました。しかし、ものす ごく長い行列が出来ていました。8:00 AMになってやっと税関を通過することができました。 自分のGSM:携帯電話のスウィッチを入れ、テルモ・ラテンアメリカから学会に参加されている 上野氏はじめ幾人かと電話でコンタクトを取りました。今回のSOLACI出張に関して、テルモ は全く僕のスポンサーでありませんでしたが、親切にもサンパウロの旅行代理店とすぐに連絡を 取ってくれました。

電話で、CONGONHAS国内空港から出発する11:00 AM発のPorto Alegre行きの便に乗れ るかも知れないと、伝えられました。私は混雑した国際空港のロビーで待っていました。突然、 館内アナウンスで私の名前が呼ばれました。誰が呼び出したのか聞き取れませんでした。私の横 に立っていた方に英語で話しかけました。突然彼は流暢な日本語で答えてきました。この方に、 何がどうなっているかを説明しました。この方は親切にも、私を呼び出した人の所に連れていっ てくれました。この日本語を話せるポルトガルの方は日本からの団体旅行の到着を待っておられ たのでした。もしも、彼かおられなかったならば、私を呼び出した人と会うことはできなかった でしょう。その人は何とスイス航空の人でした。彼が旅行代理店から私を国内空港に向う車まで 連れていくように頼まれたのでした。ついに、私はCONGONHAS国内空港に9:45 AMに到着 し、11:00 AMの便にチェックインすることでできました。何でも後から聞くと、この二つの空 港は40 Km以上も離れているのだそうです。ヴァリグ・ブラジル航空が破産寸前のために、現 在ブラジルの空港はとても混雑し、また無茶苦茶になっているのだそうです。何しろ11:00 AM の便に乗れたのはとてもラッキーでした。

1時間半の飛行の後、無事Porto Alegreに到着しました。TRIの私のレクチャーはその日 のランチョン・セッションで予定されていました。空港ロビーでは上野氏が待っておられました。 まっすぐFiergsコンベンション・センターに急ぎ、7月20日の1:00 PM前に到着しました。 これは日本時間で7月21日の1:00 AMということになります。ラッキーなことに、会の進行が 30分ぐらい遅れていました。でも問題は、自分の格好が短パンにポロシャツといういでたちで した。会場に入りましたが、そこには200名以上の聴衆がおられ、皆完璧な礼服姿でした。私 は最前列に座ってから自分の講演を行いました。最初のスライドで、「自宅を出発してから40 時間かかり、やっと今ここに到着しました」ということを強調しました。講演の後、討議があり ました。このセッションはとても素晴らしいものでした。本当に幸運にも私のデューティーを完 遂することができました。私の講演の後、一人の看護婦さんが私の所に来られました。彼女は私 が7年前にサンタクルス病院でライブデモンストレーションを行った時についてくれた方でし た。有江先生が2002年に肝臓癌のために亡くなられた、と聞きショックでした。

このセッションの後、イタリアン・レストランでDr. Gilbero Nunes、池野　文昭先生、テ ルモ・ラテンアメリカの赤松氏、そしてその社長の三瓶氏、そして他のテルモの人々とともに昼 食を摂りました。

昼食を終えてからようやく学会が私に用意したシェラトン・ホテルにチェックインしました。 既に4:00 PMであり、日本時刻で7月21日の4:00 AMですので、既に自宅を出てから42時間 が経過したことになります。とても疲れていましたので短い睡眠を取りました。夕食は日本料理 店「櫻」で摂りました。長い飛行機と時差ぼけのため、夕食の最後には眠ってしまいました。池 野先生に、最初にCYPHERステント植え込みが行われた所でもあり、またあの有名なEduardo Sousa先生や、Alexandre Abizaid先生がおられるあの有名なDante Pazzanese心臓センター を訪問してTRIやCTOに対する日本のテクニックを宣伝できたらばいいね、と話をしました。

池野先生は自治医大を卒業し、その後、岐阜県の田舎にある佐久間病院で数年間働きました。 その間に、近くの中学校に教えに来られていたカナダ人の女性教師から英会話の勉強を受けたそ うです。佐久間病院の後、豊橋ハートセンターで数年研修し、それから米国スタンフォード大学 病院に留学されました。とても熱心に研究され、運良く抽選で米国永住権を取得されました。彼 はSOLACIの主要メンバーでもあるAbizaid先生を良くご存知であり、その縁で今回のSOLACI のInternational Facultyの一人に選ばれました。彼はとても熱心で良い方です。

4:00 AMに起床しCTOに対するPCIの講演の準備をしました。ホテルを7:30 AMに出発 し、Fiergsコンベンション・センターに行きました。私の講演は8:00 AMから予定されていま した。ビデオ・プロジェクターのトラブルで開始が少し遅れました。自分の発表を終えてから、 展示場を回りました。自分の名前をInternational Faculty Listの中から見つけました。Drs. Eberhard Grube, Eulogiao Garcia, Giancarlo Biamino, Horst Sievert, Igor Palacios, Jean Marco, Jeffrey Moses, Marco Costa, Martin Leon, Patrick Whitlow, Ron Waksman, Stephen Ellisやその他の有名な先生方と共にこのリストに自分の名前がある、というのはとても光栄な ことでした。

ほとんどのメーカーが展示場にブースを設けていました。日本からもテルモ、ニプロ、そし て”A”社が展示していました。とても変です。”A”社のブースには何もすることなく立っている一 人の日本人がいました。しかし、彼は決して私に挨拶もしませんでした。きっとこの人は米国 でただ日本本社からの指示に従うだけの仕事しかしていないのでしょう。きっと、これまで何も すばらしい生産的な仕事はしてこなかったのでしょう。米国在住の日本人ビジネスマンは皆、新 たな仕事を自分で見つけて開拓しているのにね。また、中国のマイクロポートも展示していまし た。この会社もブラジルやラテンアメリカの国々に確実に進出して来るでしょう。

展示場を回ってからホテルの部屋に戻りました。バスのレーンが他のレーンから完全に分離 されているのはとてもユニークでした。部屋で数時間ゆっくりしてから1:00 PM前にPorto Alegre国立大学付属病院でのライブデモンストレーションのために出かけました。病院は巨体 でしたし、玄関ホールには銀行までありました。

どんな症例を治療するのか知りませんでした。カテ室ではサンパウロのDr. Liliana Grinfeldとこの病院のDr. Marco Wainsteinが待っていました。Grinfeld先生は、7年前のSanta Cruzにも聴衆として見学に来られたそうです。彼らから症例を見せて頂きました。患者さんは 50歳台の男性で、下壁に無症候性虚血が認められる方でした。診断カテの結果、右冠動脈半ば に2 CMのCTO病変があり、左冠動脈から左房枝を介して良好な副血行を認めました。ただ、 この副血行路は蛇行がひどく逆行性アプローチには向きませんでした。両側大腿動脈穿刺により 7 Frのガイディング・カテーテルを用いることに決めました。レントゲン機械はGEのデジタ ルでしたが、シネフィルムでした。中継が2:00 PMからの予定でしたので、1:30 PM頃から開 始しました。7Fr JR4.0ガイディング・カテーテル、Progreat-alphaマイクロカテそしてMiracle 3を用いて5分以内にCTOを通過しました。そこで、中継開始を待たねばなりませんでした。 しかし、その前の講演がとても長く、中継開始が30分遅れました。ライブデモンストレーショ ン会場での座長はあのSousa先生でした。Sousa先生の前でライブを行えるなんてとても光栄 でした。CYPHERを3つ植え込んで成功裏に終了しました。会場からの割れんばかりの拍手が 聞こえました。

患者さんの親戚が病院ナースだったなんて知りませんでした。成功裏に終了してから、家族 の方々が私の所に来られました。Grienfeld先生は空港に向かわれました。中継はマイクロウェ ーブで行われていました。中継のコントロール・ルームはとてもシンプルでしたが何の支障もあ りませんでした。Marco先生はオフラインでもう一例やることを提案しましたので、それだっ たらばTRIでやる、と答えました。次の患者さんが入って来るまで、色々なことを話ました。 何でも彼はマサチューセッツ総合病院で冠動脈インターベンションの臨床フェローとして5年 前に2年間研修したそうです。彼はとても優秀なインターベンション医師です。

次の症例は右橈骨動脈より6 Frのガイディング・カテーテルを用いてとてもすばやく終了 しました。患者さんは女性でしたが、回旋枝に2個のBMSを植え込みました。患者さんの保険 は政府の保険だったのでDESは使えませんでした。症例が皆うまくいき、また中継もうまくい ったのでカテ室の皆が喜んでいました。記念写真を撮ってから病院を去りました。

夕食は池野先生も含め、ブラジル・ステーキの店に行きました。池野先生は私のライブデモ ンストレーションの時に会場におられたそうです。彼が言われるには、Sousa先生はとても感激 されていたそうです。これを機会にこの有名なブラジルの冠動脈インターベンションの先生と関 係が持てるかと思うと興奮しました。夕食では綺麗なブラジル女性と一緒でした。伝統的なブラ ジル茶と高濃度のアルコール酒を飲みました。真夜中まで楽しみました。

ホテルを7:30 AMにチェックアウトして池野先生、上野氏と共に空港に向いました。サン パウロ行きの飛行機に乗り、11:00 AM頃に到着しました。私の次のアムステルダム飛行機は 6:35 PM出発予定でしたので、ちょっとした市内観光には十分な時間がありました。まず、第9 回日本祭りの会場を訪れました。丁度この2週間が会期でした。サンパウロには海外で最大の 日本人街があります。たくさんの日本の大きな企業が協賛し、在ブラジル各県県人会が屋台を出 していました。

ここを出てから、在ブラジル日本文化協会ビルに向いました。ここでは第21回全ブラジル 歌謡選手権が開催されていました。このビルの向かいに日本食レストラン「喜怒哀楽」がありま す。ここで昼食を摂ってから、協会ビル6階から8階にある日本移民歴史博物館に行きました。 今の皇后様美智子妃がこれまでに日本人移民を慰労するために3回もブラジルを訪れてきたこ とを知り、感激しました。2年後の2008年はブラジルに日本から移民が始まって丁度100周年 になります。

日本文化協会から地下鉄のリベルダージ駅までの通り沿いにはたくさんの日本料理屋、食品 店や雑貨店があります。何とここではマクドナルドの看板までカタカナで書いてありますが、こ んなこと外国で見たことありません。

空港に行く前にサンタクルス病院に寄りました。また、Dante Pazzanese心臓病センターの 横を通りました。もうあれから7年間が経ちました。TRIや日本のCTOテクニックを広めるた めに、またすぐにブラジルに舞い戻ることを誓いました。アイルトン・セナの名前のついた高速 道路を経て空港に到着しました。

GUARULHOS国際空港18:35発のKL 0798便に乗り、アムステルダム・スキポール空港 経由でインドのハイダラバードに向いました。今回のブラジル旅行は本当にとても過酷でした。 しかし、考えられないくらい素晴らしく、また実り多い旅でもありました。きっとこれから、こ の愛すべき、そして将来性に富んだ国に何回でも戻ってくるでしょう。
