After finishing several cases, I checked in Swisstel Beijing Hotel. I had dinner with Professor Gao Runlin and Dr. Xu Bo from Fuwai Hospital at a fine seafood restaurant. Before moving to the restaurant, I walked down to a subway station near the hotel. It was my first time to see the subway stations.
August 18th Friday 2006
I arrived at ChaoYang Red Cross Hospital around 8:30 AM. I did 7 or 8 CTO cases till 4:00 PM. Among them, several cases were performed through TRI. The pressure and EKG monitor was very compact and made in China.
I had dinner with doctors from Beijing Friendship Hospital. Unfortunately, I had no chance to see Professor Jia Sanqing this time.

August 19th Saturday 2006
I stayed in my room in the hotel and prepared for my lecture presentation during in the morning. The topic for my presentation was “PCI for diabetic patients” during the lunch time session in the International Heart Forum 2006. This meeting was held by Fuwai Cardiovascular Center at the Beijing International Convention Center (BICC) between August 18th and 20th. This year meeting was special, since it was also a 50-years’ Anniversary for Fuwai Hospital.
First, Professor Lu from Beijing Military General Hospital delivered a talk on the general management for diabetic patients. Second, I talked about which would be better between PCI and CABG for diabetic patients. Last, Professor Yang from Fuwai Hospital talked about future of PCI for diabetic patients. The auditorium was fully seated. Many people could not take seats and was attending by standing behind the room. It was a very successful lunchtime session.

The final case was terrible. After crossing the CTO lesion in the proximal LAD and
After finishing the session, I hurried to Fuwai Hospital to do cases. There were 2 cases arranged for my procedures. I finished the 1st case quickly, which was not transmitted. The 2nd case was the most difficult case during the live demonstrations in the course. The patient was 46-years old gentleman with more than 10 years’ evidence of a CTO lesion in his proximal RCA. He received a stent implantation 3 years ago in the middle of LAD. He came back to the hospital because of the recurrence of chest pain for several months. The diagnostic angiography showed the long-standing long CTO lesion in the proximal RCA and in-stent occlusion in LAD as well. Thus, from the definition, he had double CTO lesion in RCA and LAD. Dr. Xu Bo arranged this case for me.
The case was a really difficult one. I first passed the in-stent CTO lesion in the LAD and place a DES stent there. My assistant was Dr. Wu at Fuwai Hospital. He is still young but a very quick-hand physician. I like him. The CTO lesion in the RCA was really difficult. The guidewire went into a false lumen. However fortunately, my 3rd guidewire went though into the true lumen. It was a really educational case. I was very happy for this success.
I had dinner with Dr. Wang and Dr. Yang from ChaoYang Red Cross Hospital on that night. Flying by NH 956 flight, I returned to Japan on Sunday morning next day.

北京でのたくさんのCTO症例
2006年8月17日木曜日朝の便で北京に飛びました。飛行機は1時間ぐらい遅れ、北京空 港到着は2:00 PM過ぎになっていました。それから朝陽赤十字病院に直行しました。そこでは Wang先生がたくさんのCTO症例を準備していました。

数例を終えてから、Swisstelホテルにチェック・インしました。その夜は阜外病院のGao 教授とXu Boと一緒においしい海鮮料理レストランで食べました。そのホテルに行く前に、ホ テル近くの地下鉄駅に降りてみました。始めての経験でした。

8:30 AMには朝陽赤十字病院カテ室に到着しました。その日は4:00 PM頃までに7-8例の CTO症例を行いました。いくつかはTRIで行いました。とてもコンパクトな圧・心電図モニタ ーがカテ室の中にありましたが、それは中国製でした。
その日は北京友誼病院の皆と夕食を摂りました。残念ながら今回はJia先生にお会いするこ とは出来ませんでした。

その日は朝方ホテルの部屋で講演の準備をしました。講演タイトルは「糖尿病患者さんに 対するPCI」というものでした。これは、阜外病院が北京国際会議場(BICC)で8月18日から 20日まで開催した国際心臓フォーラム(IHF)という学会でのランチョンでのものでした。今年の IHF会議は阜外病院開院50年ということもあり、特別でした。
最初に北京軍総合病院のLu教授が糖尿病患者さんに対する一般的な管理について話をさ れ、次に私が糖尿病患者さんに対してはPCIと冠動脈バイパス手術のどちらが良いか、そして 最後に阜外のYang先生が糖尿病患者さんに対するPCIの未来について話をされました。会場 は満席で、何人もの人々が座ることができず、会場の後ろで立ち見でした。会としてとても成功 しました。

このセッションを終わってから、阜外病院で症例を行うために急ぎました。2例待っていま した。一例目をさっさとoff-lineで行いました。2例目はこの学会のライブデモンストレーショ ンで一番難しい症例でした。患者さんは46歳の男性で、少なくとも10年前から右冠動脈がCTO であることが分かっていました。そして、3年前にLADの中ほどにステントを植え込まれまし た。数ヶ月前から胸痛が再発し、診断カテの結果、右冠動脈のCTOでLADのステント部のCTO が見つかりました。ですから、定義から言っても、この患者さんは右冠動脈とLADの両者に CTO病変を二つ有していることになります。Xu Boが私にこの症例を割り付けました。
この症例は本当に難しい症例でした。最初にLADのCTOを開け、DESを植え込みました。 助手は阜外病院のWu先生がして下さいました。彼はまだ若いのですが、とても手が早い術者 です。僕は彼のことをとても気に入っています。右冠動脈のCTO病変は本当に難しいものでし た。ガイドワイヤーは偽腔に入りました。しかしながら、幸にも3本目のガイドワイヤーが真 腔を捉えました。この症例は本当に教育的症例でした。成功してとても嬉しいです。
この日の夕食は朝陽赤十字病院のWang先生とYang先生と一緒に摂りました。翌日日曜日 の朝の便で日本に帰国しました。
