I had no duties on the day. I lazily spent time in the morning in my room. For the lunch, I went to a BAR near the hotel, where I drank beer and wine with Spanish snacks. I like these Spanish snacks including marinated Anchovies, Spanish Omelets, Spanish Salad and so on. Of course, we cannot forget Jamon (Spanish raw ham). Since I was so tired and in severe jet lag, I came back to my room and had sleep after the lunch.

For the dinner, I had an appointment with Dr. Fina Mauri. Since she had busy schedules by 9:00 PM, we went to Siena Verde, which is one of my favorite Garcian seafood restaurants. I was visiting this restaurant almost every time when I was in Madrid. Our choice of foods included Bread pasted with Tomato and Garlic, White Asparagus, Green Asparagus, Jamon Ibelico, Garcian Octopus and Angulus. It was very interesting for me to check the price of Angulus. It was 70 Euros in November 2005 and 95 Euros in January 2006. The price in September 2006 was again 95 Euros. All of the foods were so delicious. I cannot imagine how they cook octopus so soft. During and after the late dinner, we drank not so expensive Sicilian Red Wine and Liquor. We enjoyed the dinner very much.

September 27th Wednesday 2006
I went to the Hospital Gregorio Mararion, which is a big University Hospital, where Professor Eulogio Garcia is working. The case assigned to me during the live transmission was a relatively young gentleman with double vessel disease in LAD and RCA. The narrowing in LAD was very tight and almost totally occluded. I started with the right radial puncture. After crossing the LAD lesion by a floppy guidewire, I could not cross it by any balloon. I did an anchoring balloon technique. However, it did not work. Professor Garcia was standing beside me as an assistant. He suggested me to take a rotablator. However, the problem was how to exchange the PCI guidewire to a rotablator guidewire. Anyway, we had no other way to do. I put a microcatheter as far as I could, pulled out the PCI guidewire, and then tried to put a rotablator wire. It was a really difficult process. However, I could fortunately replace the guidewire with a rotablator wire. I took a 1.25-mm rotablator burr. After the rotablator, I could put a balloon and a DES stent there. The lesion in the proximal RCA was also tight. It was again difficult to put a balloon there. However, I could place a DES stent. I was happy for the result. It was a difficult case. However, I could finish the case through the transradial approach by using a 6 French guiding catheter.

After the case, I returned to the hotel. I had duties to give a lecture and to chair the session for CTO from 13:00. However, my flight from the airport was departing at 16:00. I quit my duty around 13:30 and hurried to the airport. I took the IB 3502 flight. However, the departure was again delayed by 1 hour. The airline company changed our aircraft to another one because of some machine trouble. After very short transit time in Frankfurt airport, I finally changed to NH 210 flight to Narita, and arrived in Tokyo around 15:00 on September 28th Thursday 2006. I love Spain.

2006年9月21日から23日まで開催されたCCT 2006での忙しいスケジュールを終え、翌 日日曜日は休むことができました。そして、その翌日25日月曜日11:35 AM成田発フランクフ ルト行きNH 209便に搭乗してマドリッドに向いました。予定ではフランクフルト空港で4時 間の待ちの後、イベリア航空IB 3506便でマドリッドに向うことになっていましたが、マドリ ッドに向けての出発が3時間遅れました。結局、空港ラウンジで7時間近くも過ごすことにな りました。幸い、ラウンジではワイヤレス・インターネット接続が可能でした。 http://www.kamakuralive.net/のウェブサイトから無料DICOM Viewerをダウンロードするた めのcgiプログラムを書き上げることに一生懸命でした。プログラムはApache Web Serverで 走るPerlで書きました。私にとってPerlスクリプト言語でプログラムを書くのは初めてでした のでとても大変でした。しかし、その一方で、自分の知識欲をいたく刺激しました。
結局、Barajas国際空港に到着したのは、9月26日0:30 AMになっていました。空港は拡 張され、完全に立て替えられました。新しいターミナルは巨大で綺麗です。スペイン経済がとて もうまく行っているのだな、と分かりました。ホテルは何時ものようにMelia Castillaで、TEAM 学会もそこで開催されました。夜遅くに軽く食事を摂ってから眠りにつきました。

この日は仕事が無かったので、午前中は部屋でだらだらしていました。昼食はホテル近くの BARに行きました。スペインのBARというのは、ビールを飲んだりスペインおつまみを食べ るのです。スペインつまみを僕は好きです。例えば、イワシのマリネ、スペイン風オムレツ、ス ペイン風サラダなどです。勿論、生ハムを忘れてはいけません。とても疲れていたし、時差ぼけ もひどかったので、この昼食の後、また部屋に戻り眠りました。

夕食はFina先生と約束していました。先生は9:00 PMまでぎっしりスケジュールが詰ま っていましたので、Siena Verdeに行くことにしました。ここは、ガルシア地方のシーフード・ レストランで僕のお気に入りです。マドリッドに来る時は大体いつも来ます。頼んだのは、トマ ト・ペーストのパン、白アスパラガス、緑アスパラガス、生ハムイベリコ、ガルシア風タコ、そ してウナギの稚魚でした。ウナギの稚魚がいくらになっているのかは、とても興味をそそります。 2005年11月には70ユーロ、2006年1月には95ユーロ、そして今回も95ユーロでした。食 事はとてもおいしいものでした。どうやったらあんなにタコを軟らかく仕上げることができるの か分かりません。食事の間そして終わってから高くないシシリア赤ワインとリカーを飲みました。 とても楽しい夕食でした。

Gregorio Mararion病院に行きました。ここはガルシア先生のおられる大学病院です。ライ ブで私に与えられた症例は比較的若い男性でした。LADと右冠動脈に病変がありました。LAD の狭窄はとてもきつく、ほとんど完全閉塞でした。右橈骨動脈穿刺から始めました。ガイドワイ ヤーが通過してもどんなバルーンも通過せず、アンカー・バルーンなども行いましたが駄目でし た。ガルシア先生は僕の横に立ち助手をして下さいました。彼がロータブレーターをしよう、と 提案しました。ただ、問題はどうやってガイドワイヤーをロータブレーター・ワイヤーに交換す るか、です。ただ、他に手段が無かったので、マイクロカテーテルをぎりぎりまで持って行き、 そこでPCIガイドワイヤーを抜去し、ロータブレーター・ワイヤーを入れました。とても大変 でしたが、何とかガイドワイヤーの交換をすることができました。1.25mmのバーでロータブレ ーターをした後は、バルーンで拡張し、DESを植え込みました。右冠動脈近位部の病変もきつ いものでした。またもやバルーン通過が困難でしたが、何とかDESを植え込みました。結果に 満足しました。この難しい症例をTRIで6Frを使ってやれたことが幸せでした。

この症例を終わってホテルに戻りました。13:00からCTOのセッションで講演と座長があ りました。しかし、私の飛行機は16:00に出発するものでしたので、13:30に会場を発ちました。 IB 3502便に乗るために急ぎました。しかし、またしてもこのイベリア航空は1時間出発が遅れ ました。マシン・トラブルのために飛行機そのものを入れ替えたのです。フランクフルト空港で は少ししか余裕がありませんでしたが、何とかNH 210便に乗り、2006年9月28日木曜日15:00 頃に成田に到着しました。スペインっていいな。
