14 Years of Transradial Coronary Angioplasty

Annual my schedule includes my visit to Amsterdam in October or November every year to participate in the transradial coronary intervention course held by Dr. Ferdinand Kiemeneij. This year, it was held between October 13 and 14 with the title of “14 Years of Transradial Coronary Angioplasty”.

I flied directly to Amsterdam by taking JL 411 flight departing at 12:45 on October 12 Thursday 2006. As always, I directly moved to the Japanese Restaurant “Yamazato” in Hotel Okura Amsterdam after 12 hours’ flight and had Sushi dinner there from 6:30 PM on the same day. Yamazato was fully crowded mainly by Dutch people. I like Kazunoko cooked there. After having dinner, I checked in the Barbizon Palace Hotel in front of Amsterdam Central Station later than 9:00 PM.

October 13 Friday 2006

Totally differently from the past several years, it was clear outside. The sky was beautifully blue. Since there were some transmission problems, the starting of the meeting was delayed, and it began from 8:15 AM. Ferdinand did the 1st case. He renewed his department recently. First, he set up “Radial Lounge”. Second, he opened the No. 3 catheter laboratory.

The new laboratory is equipped with a Philips’ flat panel single plane machine and a Stereotaxis machine as well. Ferdinand did the 1st case from the radial puncture. The case was not a difficult one with 2 discrete in-stent restenosis in the proximal LAD. He showed the TRI conducted by the Stereotaxis. It was my first time to see Stereotaxis actually worked. It looked Ferdinand’s new toy but not a new medical modality.

Next to Ferdinand, Dr. Jean Fajadet did his case. The lesions were in a non-protected left main and proximal LAD. Then, Dr. Tejas Patel did his case. His case had a severe narrowing in the distal RCA, which could be reached through the saphenous vein graft. Following these famous operators, Dr. Tift Mann did his presentation on the situation of TRI in Wake Med Heart Center.

Dr. Slagboom came to me and told “You have to go to the hospital now, since the traffic may be heavy”. I took a taxi to the OLVG hospital with Dr. Hildick Smith from England. Ferdinand welcomed us and showed my case CD. He asked me, “Shigeru, do you want to see the Lounge?”

I answered, “No, I never want to see it. If do, I must be jealous.” He kindly introduced me to the Radial Lounge. Patients come to the Lounge in the morning, receive TRI, stay in the Lounge for several hours, and come back on their foot before the evening. If something happens, those patients are transferred to CCU next to the Lounge. There are no monitors or no physicians taking care in the Lounge. I was really jealous for it. The No. 1 laboratory was under the renewal. The mixing machines were set up in the No. 1 laboratory.

My case was a big and tall gentleman. He had 3-vessel disease. His RCA was totally occluded in the middle segment. I started from the right radial puncture. By the combination of a Runthrough-hypercoat guidewire and a Finecross microcatheter, I could cross the total occlusion and finally placed an Endeavor DES stent. His LAD and LCX had a discrete 90% narrowing respectively. I placed an Endeavor stent each in both lesions. The whole procedure was finished very quickly in 30 minutes. I was very happy with Ferdinand.

I took lunch in a Chinese restaurant near the hotel. I did my lecture on CTO 2006 in the afternoon. After my lecture, Jean came to me and told me, “If you do not mind, could you come to Toulouse during next EuroPCR meeting and do live transmission for TRI?” My answer was of course “Yes, it is my pleasure.”

Leaving the meeting around 5:00 PM, I went to the Schiphol Airport to take the return flight of JL 412 departing at 20:15. I arrived at Narita Airport around 2:30 PM on October 14 Saturday 2006. This trip to Amsterdam was again very short. I just spent 24 hours in Amsterdam.

経橈骨動脈的冠動脈インターベンション14年

毎年10月か11月にはKiemeneij先生がアムステルダムで開催されているTRIコース に参加するのが通例になっています。今年は10月13日と14日に開催され、経橈骨動脈的 冠動脈インターベンション14年と銘打たれました。

2006年10月12日木曜日12:45発のアムステルダム直行JL 411便で出発しました。 12時間あまりの飛行の後、いつものようにアムステルダム・ホテル大倉内の日本料理「山 里」に行き、6:30 PMから寿司を食べました。山里は主にオランダの人たちで満杯でした。 私はここの数の子が好きです。この後Barbizon Palaceホテルにチェック・インしたのは 9:00 PMを回っていました。

これまでと違って、外は晴れており、空は青く澄んでいました。中継トラブルがありま したので、会の開始は遅れ、8:15 AMに始まりました。第一例目の術者はKiemeneij先生 でした。彼は、最近彼のセンションを改装しました。それは、Radial Loungeを設けたこと と、カテ室No. 3を新たに作ったことです。

新しいカテ室にはPhilipsのフラット・パネルがあり、またStereotaxisが備えつけら れていました。Ferdinandはまず橈骨動脈穿刺から入りました。その症例は難しい症例で はなく、LAD近位部に二ヶ所、限局性のステント内狭窄がありました。彼はStereotaxis を用いたTRIを行いました。Stereotaxisで実際に治療するのは始めて見ました。医療器械 というよりも、Ferdinandの新しいオモチャのように見えました。

Ferdinandに続いて、Jean Fajadetが症例を行いました。彼症例は左主幹部からLAD 近位部の病変でした。続いて、Tejas Patelが行いましたが、その症例はSVG越しの右冠動 脈狭窄でした。これらの有名な術者に引き続いて、Tift MannがWake Med Heart Center でのTRIの現状について話をしました。

Slagboom先生がこちらに来られました。そして、「渋滞があるかもしれないから、も う病院に行かないと駄目だよ」と、言われました。イギリスのHildick Smith先生と共に タクシーでOLVG病院に向いました。Ferdinandが症例のCDを見せてくれました。彼は、 「滋、Loungeを見たい?」と聞きました。

私は、「いんや、決して見たくなんかない、見たら嫉妬するから。」と、答えました。そ うして彼はRadial Loungeを案内してくれました。患者さんは朝このLoungeに来られ、 TRIを受け、そして、そこでくつろぎ、それから自宅に夕方には帰宅します。何かあれば、 隣のCCUに移ります。Loungeの中にはモニターも無く、また医師もおりません。とって も羨ましく思いました。カテ室No. 1は改修中でしたが、そこで中継操作が行われていまし た。

私の症例は、大きな背の高い男性でした。3枝病変で、右冠動脈は中ほどで完全閉塞で した。橈骨動脈穿刺から開始し、Runthrough-hypercoat + Finecrossで完全閉塞を通過し、 Endeavor DESステントを植え込みました。LADとLCXにも90%の限局性狭窄がありま したが、それらにもEndeavorを植え込みました。全手技を30分以内で素早く終了しまし た。Ferdinandと共にとても嬉しく思いました。

ホテル近くの中華料理で昼食を摂りました。午後にはCTO 2006というタイトルで講演 を行いました。この講演の後、Jeanが来ました。そして、「良ければ、またEuroPCRの時 にToulouseに来て、TRIのライブをしてくれない」と、聞きました。もちろん答えは「喜 んで」でした。

会場を5:00 PMに離れ、空港に向いました。そして、20:15発のJL 412便に乗り、翌 日10月14日土曜日2:30 PM頃に成田に着きました。今回も、このアムステルダム訪問は とても短く、滞在は24時間ぐらいしかありませんでした。
