November 16th Thursday 2006
I visited Ruttonjee Hospital in Hong Kong Island in the morning (Dr. Lau Yuk Kong and Dr. Keung Kin Kwan). They prepared 2 CTO cases for me. One of them had old very long chronic total occlusion in both RCA and LAD, and his distal LCX showed CTO as well. His LV function was very poor and EF was 25% with severe mitral regurgitation. I cancelled this patient and recommended Dr. Lau to send the patient to surgery. Another patient had CTO in RCA. Previous attempt was failed. I did this case from the right femoral approach using an antegrade approach. I could finish this case successfully within 1 hour. From 11:30 AM, they arranged a lunch-time lecture session in the hospital for me. I talked about my approach for CTO lesions.

In the afternoon, I moved to Pamela Youde Eastern Hospital (Dr. Li Shu Kin, Dr. Tse Tak Sun and Dr. Tsui Kin Lam). They arranged 2 CTO cases for me. Previous one or twice attempts to open the lesions failed for both cases. I took the bilateral femoral approach by putting 7-Fr guiding catheter. For both of them, I started with retrograde approach. For the 1st case, I crossed a guidewire and a balloon retrogradely from LAD to distal LCX, and could open the CTO lesion in mid LCX successfully. The 2nd case had long in-stent occlusion at least for 3 years after previous TAXUS implantation in the proximal RCA. I approached from LAD through septal connection, and crossed guidewires into Aorta retrogradely. However, either balloons or Tornus devices could not pass through the lesion. Finally, I gave up this case.

After finishing the cases, we moved to Kowloon side for dinner. We visited LeiYueMun fish market, where we could eat very fresh fishes. We enjoyed the dinner.
November 17th Friday 2006
I visited Princess Margaret Hospital in the Kowloon side (Dr. Wu Chee Wo and Dr. Tsui Ping Tim). It was my 2nd visit to this beautiful hospital. There were 2 CTO cases with previous failed attempts. For these 2 cases, I took one retrograde approach and one antegrade approach. Both cases were successfully done. After the procedures, we discussed about the tips and ticks in PCI for CTO lesions with Hong Kong physicians each other.

After finishing lunch with catheter laboratory staffs near the laboratory, I hurried to the airport to take the returning flight to Japan. In conclusion, I stayed in catheter laboratories in Hong Kong for 1 and half days, did PCI for 5 CTO patients with previous failed attempts, took 3 retrograde and 2 antegrade approaches, passed guidewires through the lesions in 5 patients, and opened the lesions completely in 4 patients. I was satisfied with these results. Hong Kong is a nice place.

4年振りの香港訪問
私が最後に香港をPCIで訪れたのは2002年11月のことでした。従いまして、今回の香港 訪問は4年ぶり、ということになります。2006年11月15日水曜日朝の便で香港に飛びました。 今回は、Island Shangri-Laホテルに宿泊しましたが、窓からは香港海峡を見ることができます。

朝から香港島にあるRuttonjee病院を訪問しました。ここでは2例のCTO症例が準備され ていました。その中の1例は、RCAにもLADにも長いしかも古いCTO病変があり、LCX末 梢も閉塞し、左心機能は落ち、EFは25%ぐらいで、ひどい僧帽弁閉鎖不全がありました。私は この症例はキャンセルし、Lau先生に、患者さんをCABGに回した方が良い、と薦めました。 もう一人の患者さんは以前にPCIを試みたがうまく行かなかったRCAのCTO患者さんでした。 右のTFIで行い、順行性に通かし、1時間もかからずに成功裏に終了しました。11:30 AMから、 ランチョン講義が予定されていました。そこではCTOに対するPCIの話をしました。

午後にはPamela Youde Eastern病院に移動しました。ここでは2例の以前失敗したCTO 症例が用意されていました。両者に対して、両側大腿動脈7 Frアプローチを行いました。了承 例とも、逆行性アプローチで開始しました。最初の症例は、LADから逆行性にLCX末梢にガイ ドワイヤーとバルーンを通し、LCXのCTO病変を開けることができました。次の症例は、3年 以上前からあるTAXUSステントの中の長いCTOでした。LADから中隔枝経由で逆行性にガ イドワイヤーを大動脈まで通過することができました。しかし、バルーンもTornusも通過する ことができず、この症例は諦めました。

これらの症例を終えてから九龍側に夕食に行きました。場所は”レイユーメン: 鯉魚門”です。 ここには生きた魚がたくさん並んでいて、とても美味しい魚を食べることができます。おいしか ったです。

九龍側にあるマーガレット王女病院を訪ねました。この綺麗な病院を訪れたのは2回目で した。ここでも以前に失敗したCTO症例2例が用意されていました。1例は順行性に1例は逆 行性にアプローチしました。両方の症例ともうまくいきました。手技を終えて、香港のお医者さ ん達とCTOの話をしました。

カテ室の皆とカテ室の近くで昼食を摂ってから、帰国のために空港に急ぎました。結局今回 の短い香港訪問では、香港のカテ室に1日半おり、以前の失敗例CTO症例を5例行い、3例で 逆行性に、2例で順行性に行い、全例でガイドワイヤーをクロスすることができましたが、1例 では開通することに不成功でした。私はこの結果に満足です。
