They have many patients now. On my visit, they arranged 7 difficult cases, and most of them had CTO lesions with previous failed attempts. I was successful in many cases. I did the retrograde approach for one of them. However, it was unhappy. One of the patients had a short CTO lesion in the middle LCX. The previous attempts failed. After crossing the lesion by using a Conquest-Pro 12 guidewire, the lesion was successfully recanalized by Tornus device. However, the subsequent ballooning with a 2.0-mm balloon resulted in the balloon burst at less than 6.0 ATM. Because of the jet from the burst balloon, the dye extravasated. I tried to stop the bleeding by applying a long inflation. However, it did not work. We send the patient to the surgery. When I visited last time, they did not have so many patients. I was very impressed to see how much they had achieved a big growth.

I hurried to Jakarta Airport to catch the Garuda Indonesia flight for Surabaya. I could take the flight departing at 18:15. After arriving in Surabaya, I met with Dr. Rurus and had dinner at Sea Master Seafood Restaurant. The hotel for my stay was again at Shangri-La Hotel Surabaya.

I visited Mitra Keluarga Hospital again in the morning. Total of 8 or 9 cases were prepared. Everything was running very smoothly, since I was already very familiar with the catheter laboratory. After finishing all of the cases, we had pleasant dinner at the Chinese Restaurant in IW Marriott Hotel Surabaya.

I checked out the hotel early in the morning. I flew to Bangkok by Thai Airline TG 434 flight via Jakarta, and finally arrived in Bangkok New Airport at 16:40. It was too short journey for Indonesia.

ジャカルタ訪問
12月15日から17日にかけての鎌倉ライブデモンストレーションでの忙しいスケジュ ールを終えて2006年12月19日火曜日11:20発のJL 725便でジャカルタに飛び立ちまし た。ジャカルタに着いたのは17:00頃でした。そのまま市内にのインターコンチネンタル ニ・ホテルに向かったのですが、とってもひどい交通渋滞には参りました。ジャカルタそ のものを訪れるのはもう6年以上ぶりでしょうか。
ホテルにチェックインしてからHarapa Kitaの先生方と夕食を摂るためにシャングリ ラ・ホテルの香宮という中華料理レストランに向かいました。

Harapan Kitaというのはインドネシア語で「私たちの希望」という意味です。この病 院は、母子病院、癌病院そして循環器科センターの合さったインドネシア国立病院です。 NCCはインドネシア最大の循環器病センターです。以前ここにも1回か2回訪問したこと がありますが、それは10年以上前の1994年のことだったと思います。Dr. Munawarが歓 迎して下さいました。カテ室は4部屋あり、一つにはPhilipsの最新型フラット・パネルが 設置してありました。

Harapan Kitaはたくさんの症例を有しています。私が訪れる、というので7例の難し い症例を準備されていました。ほとんどの症例が以前うまくいかなかったCTO病変でした。 その多くでうまくいきました。一例ではretrograde approachを行いました。しかし、一例 ではうまくいきませんでした。その患者さんはLCXの短いCTO病変を有していました。 前回もうまくいかなかった症例です。Conquest-Pro 12でクロスすることに成功し、Tornus で再開通させた後、2.0mmのバルーンで拡張したところ、バルーンが6気圧にも満たない 内に破裂し、破裂したジェットで穿孔を起こしました。バルーンを拡張することにより止 血を試みましたがうまく行かなかったのでその患者さんは手術になりました。前回私が訪 問した時には、ここにはたくさんの症例はありませんでした。こんなにも発展したことに 印象を深めました。

スラバヤ行きのGaruda Indonesia航空に乗るためにジャカルタ空港に急ぎました。結 局、18:15発の便に乗ることができました。スラバヤに着いて、Rurus先生にお会いし、馴 染みのSea Masterというレストランに行きました。私が宿泊したホテルはシャングリラで した。

朝再びMitra病院を訪ねました。全部で8例か9例の症例が準備されていました。と てもこのカテ室に馴れているので全てがスムーズに進みました。全症例を終えてから皆で マリオット・ホテルの中華レストランに行きました。

朝早くチェックアウトしてから、ジャカルタ経由でTG 434便に乗り、バンコク新空港 に16:40に到着しました。インドネシアを訪問するには短すぎる旅行でした。
