I was invited as the operator and lecturer by the 4th annual meeting of the Hungarian Society of Interventional Cardiology, which are held in 4 university hospitals in 4 major cities in Hungary in turns. This year, it was held in the Szeged National University Hospital. Although there was a welcome reception in the National Theatre, I did not join it and went to bed.

October 5th Friday 2007

I left the hotel around 7:30 AM and visited the catheter laboratory at Szeged University Hospital. There was a GE flat panel machine. When I arrived at the laboratory, few people were there. Twenty minutes later, Professor Ungi Imre, who was the organizer of the meeting, appeared after preparing the meeting.

When the meeting was planned originally, it was dedicated to TRI. However, the concept of the meeting was totally changed meanwhile. It was completely for the CTO angioplasty. Dr. Ungi prepared 2 cases of the long-standing CTO in the LAD for me in the morning. The anatomy of both cases seemed similar. LAD was totally blocked in the proximal part for almost 3 CM. Previous attempts to recanalize were failed. Before the occlusion, where were several branches. The entry points were not clear. Although the arteries distal to the CTO lesions were clearly visualized from RCA, there were no septal connections or relatively straight epicardial collaterals. It was impossible to take the retrograde approach in both cases.

By using double guidewire techniques with fine guidewire manipulation through the antegrade approach, I succeeded in crossing both CTO lesions in 1 hour. I placed Xience DES. The chairmen in the auditorium included Dr. George Sianos in Thoraxcenter, Rotterdam. I was happy for the results. I believe both cases were really educational in teaching how to use stiff guidewires and how to do double guidewire technique. While finishing my 2nd case, George came in the catheter laboratory.

After leaving the laboratory, I moved to the beautiful auditorium of the University, where the meeting was held. I did a lecture on CTO angioplasty and chaired the live case transmission performed by George. His case was with a CTO lesion in the proximal part of RCA. Honestly speaking, it looked not so difficult through the antegrade approach. However, he started from retrograde approach. Finally, he was successful. I was satisfied to know that he had completely mastered how to do retrograde approach after I taught him during my live case transmission from Thoraxcenter during EuroPCR 2006.

After my roles, Dr. Ungi kindly arranged a short city tour in Szeged. I had a cup of chocolate moose at a pretty cafeteria at the Mikszath Kalman Street. I visited the Cathedral in the center of the city. There is the 2nd biggest pipe organ in Europe, which has 10,000 pipes.

The gala dinner was held in Reok Palace from 8:00 PM, which used to be a National Bank during the Communistic era and was changed to the art gallery. I was given the certificate of diplomate for the Hungarian Society of Interventional Cardiology. It was my great honor. The food there was typical Hungarian one. A polish cardiologist sat together with us in the same table. He just arrived the dinner from his country by car. He was too much proud of himself, although he did not indentify me. How he could be the professional for CTO angioplasty, while he did not know the world-most famous operators in CTO angioplasty like me. Since I had sever jet lag, I returned to my room by 10:00 PM.

October 6th Saturday 2007

I left the hotel by 6:30 AM and moved to the Budapest Airport by car to take the LH 3441 flight departing at 9:40 AM for Frankfurt. After transferring to the LH 4084 flight, I finally arrived at Venice Airport in Italy before 14:00. Finally, I had no chance to visit the city of Budapest.

空からだけのブタペスト訪問

2007年10月4日木曜日5:30 AMに自宅を出て、9:55 AM発のルフトハンザLH 711便で 出発しました。フランクフルト経由でLH 3446便でブタペストFerihegy国際空港に着いたの は、その日の20:30であり、日本時刻では翌日の3:30 AMでした。空港に到着する前に空から ブタペストの美しい町並みを見ることができました。空港から車で1時間半移動して、南部の セルビア国境間近のセゲドという街に移動し、NOVOTELホテルに入りました。この街はまた 香辛料のパプリカの生産地としても有名です。この街は人口17万人余り、学生がそれ以外に3 万人ぐらいの街ですので、丁度　鎌倉市と同じくらいです。セゲドというのは古いハンガリー語 で「三角」という意味だそうです。これは、ティサ河とマロス河の合流部に町が位置するからで す。1879年には大洪水のために街全体が水没し破壊されました。このため、ほとんどの建物は それ以降に建造されたものです。

私は、第4回ハンガリー・インターベンション学会から術者および演者として招聘されま した。この学会は4つのハンガリー主要都市にある4つの大学の持ち回りで開催されているそ うです。今年はセゲド国立大学病院で開催です。国立劇場で歓迎晩餐会が開催されましたが、参 加せずに部屋で寝ました。

ホテルを7:30 AMに出てセゲド大学カテ室に向かいました。ここにはGEのフラットパネ ルがありました。私がカテ室に到着した時にはほとんど誰もいませんでした。20分ぐらいして Ungi先生が学会の準備を終えて到着されました。

この会が最初に計画された時には、それはTRIをメインとして行う会でした。しかし、次 第にCTOの会に変貌しました。Ungi先生は私に午前中2例のCTO症例を準備されていました。 この2例とも似たような症例で、両者ともLAD近位部に3 CMぐらいの閉塞期間が長いCTO を有し、閉塞の手前から枝が分岐していました。末梢はRCAから良く造影されていましたが、 中隔枝は無く、心外膜を走る副血行はとっても蛇行していましたので、逆行性アプローチは不可 能でした。

ガイドワイヤーを慎重に操作し、double wireによって順行性に行い両方とも1時間でうま く成功しました。最終的にXience薬剤溶出性ステントを植え込みました。会場では ThoraxcenterのGeorge Sianos先生が座長をされていました。私の行った両症例とも、どのよ うに硬いガイドワイヤーを操作し、どのようにdouble wireを行うかを示すとても良い教育的症 例であったと思います。2例目の途中でGeorgeがカテ室に現れました。

カテ室を離れ、大学の中にある綺麗な講堂に向かいました。そして、そこでCTOの講義と、 Georgeのライブ症例の座長を務めました。Georgeの症例はRCA近位部のCTOでしたが、正 直そんなに難しそうではありませんじた。でも彼は逆行性アプローチで行い、うまくやりました。 私が、EuroPCR 2006会期中にThoraxcenterで行ったライブの時に、彼に逆行性アプローチの やり方を教え、それを彼がマスターしていることに満足しました。

これらの役目を終えた後、Ungi先生が市内観光をセットして下さいました。ミクサート･ カールマーン通りにあるカフェでチョコレート・ムースを食べました。その後、街の真ん中にあ る大聖堂を訪れました。そこにはヨーロッパ第二の大きさのパイプ・オルガンがあり、そのパイ プの数は何と10,000本ということでした。

ガラ晩餐会は8:00 PMからレーク・パレスで開かれました。この建物は共産主義政権の時 には、国立銀行でしたが、現在は画廊となっています。まず、私にハンガリー・インターベンシ ョン学会の名誉会員が授けられました。そこでの食事は典型的なハンガリー食でした。同じテー ブルに一人のポーランドの医者が座りましたが、彼は自信たっぷりで自分がCTOの専門家のつ もりのようでしたが、この世界的に有名なCTO術者の一人である僕のことも知らないなんて、 まったくどうかしています。まだ時差ぼけでしたので、10:00 PMには部屋に戻りました。

6:30 AMにはホテルを出て車でブタペスト空港に向かいました。そして、9:40 AM発のLH 3441便でフランクフルトに向かいました。そこで、LH 4084便に乗換えで、ベニス空港には 14:00に到着しました。結局、ブタベストを訪れる機会はありませんでした。
