I left the hotel around 7:00 AM and took a taxi. However, there was a
miss-communication. Taxi driver took me to a wrong direction. Since he could understand
only Spanish, it took time to correct the mistake. Finally, I reached the hospital later than
8:00 AM. The 1st case was a 50 +/- years old male patient with a CTO lesion in the RCA. By
the bi-femoral approach, I took a retrograde approach for this CTO lesion. Fina was
assisting me. Because Fina was my assistant operator, I asked Fina to do bi-radial punctures.
She made skillful radial punctures. We started with bi-radial approach. It was a difficult case.
However, I was successful to open the lesion during the
live transmission to the auditorium in Madrid. It was the actually 1st case for a new balloon
“Apex” from Boston-Scientific Company. I was really shocked for its performance. Before
using that, I tried several nice 1.00, 1.25 or 1.50 mm balloon. However, I could not cross the
lesion. However, by using the Apex balloon, I could not feel any difficulty in crossing the
same lesion. Finally, I placed Endeavor Risolute new DES stents. All of the commentators in
the auditorium were surprised to see what I was doing. We were very happy for the result.

The 2nd case had a critical left main bifurcation lesion. I treated this patient with through the radial approach. It was very easy compared to the 1st case. The case was also transmitted to the auditorium through the satellite. After finishing the live transmission, both Tino and Fina left to Madrid. I took nice lunch in a seafood restaurant

Hospital del Mar is a new hospital. It is located just in front of beautiful beach in the new area of Barcelona. There are many restaurants in the beach. I visited one of them. The food was nice. It was really relaxing time for me.
I left Barcelona by AF 1949 flight departing at 18:20 on November 29 Thursday 2007. The flight was delayed for 1 hour. We have to be very careful, when we take Air France flights in Europe. They are always delayed. Last time when I took one of them from Athena to Paris, it was delayed for 3 hours, and I could not catch the connecting flight to Japan. This time, I could fortunately take the connecting flight of JL 5056 to NARITA. I arrived in NARITA at 19:00 on November 30 Friday 2007.

ストックホルムからバルセロナに飛びました
スペインのマドリッドで開催されたTEAM 2007学会に術者および講師として参加しまし た。でも、私のスケジュールがとても厳しかったので、マドリッドでの講師の役目はキャンセル し、バルセロナでの術者のみ受けました。
SPANAIR JK 048便に乗り、2007年11月28日水曜日15:00にストックホルムを飛び立 ち、バルセロナに到着したのは同日18:30でした。最初の直接「海の病院」に向かい、Tino先 生とFina先生と落ち合い、翌日ライブデモンストレーションで行う二例について検討しました。 この後、Finaととても良いシーフード・レストランに行きました。

ホテルを7:00 AMに出でタクシーを捕まえました。でも、連絡ミスで、タクシーは別の方 向に向かってしまいました。タクシー運転手はスペイン語しか話さないので間違いを正すのに時 間がかかりました。最終的に病院に到着したのは8:00 AMを回っていました。最初の症例は50 歳台の男性患者さんで右冠動脈のCTOでした。両側経大腿動脈的冠動脈インターベンションに より逆行性アプローチで行いました。Finaが僕の助手をしました。 Finaが僕の助手でしたので、彼女に両側橈骨動脈穿刺を頼みました。彼女はとても上手に穿刺しました。そして、両側橈骨動脈アプローチで開始しました。難しい症例でしたがうまい ことCTOをマドリッドへのライブ中継の最中に開けることができました。この症例ではスペイ ンで初めてボストンの新しいバルーン「Apex」を用いました。その優れた性能に驚きました。 このバルーンを使う前に、1.00, 1.25, 1.50-mmバルーンを試みたのですが、全く病変を通過し ませんでした。しかし、その病変に対してApexを使うと、何の抵抗も無く通過しました。最後 にEndeavorリゾルテという新しいDESを植え込んで終了しました。会場のコメンテーターは 私のやっている手技を見て驚いていました。我々は結果に満足でした。

二例目は左主幹部分岐部のきつい病変でした。この患者さんに対してはTRIにより行いま したが、一例目と比べると簡単でした。この症例も衛星通信でマドリッドに中継されました。ラ イブデモンストレーション中継を終えてから、TinoとFinaはマドリッドに移動しましたが、私 は素晴らしいシーフード・レストランに行きました。

「海の病院」は新しい病院です。バルセロナ新市街地の美しい砂浜の前に建ちます。砂浜 には良いレストランがたくさんあり、その内の一つを訪れました。食事はおいしく、とてもリラ ックスできました。
バルセロナを2007年11月29日18:20発のAF 1949便で発ちました。しかし、便は1時 間遅れました。ヨーロッパでAir Franceの飛行機に乗る時には気をつけねばなりません。何時 も遅れるのです。前回アテネからパリに向かう便に乗った時には3時間遅れ、このため、成田 に向かう連絡便に乗れませんでした。今度は幸運にも成田行きのJL 5056便の乗り継ぐことが できました。到着は2007年11月30日金曜日19:00でした。
