The 2nd CTO Live in Seoul

Again after last year, I visited Seoul in the beginning of New Year. Flying by JL 5235 flight departing at 13:05 from Haneda Airport on January 4th Friday 2008, I arrived at Kimpo Airport around 15:30. I stayed at Grand Intercontinental Hotel in GangNam district. I skipped the faculty dinner and had blow fish.

PCI for CTO lesions recently has been collecting attentions by Korean doctors. Professor Seung-Jun Park in Asan Medical Center started CTO Live in the early January 2007. The operators last year included me, Dr. Takahiko Suzuki, Dr. Yasushi Asakura and Dr. Etsuo Tsuchikane. I did 2 CTO cases last year.

Dr. Park held the same meeting called “CTO Live 2008” this year. The operators included Dr. Osamu Katoh as well.

January 5th Saturday 2008

I went the conference room beside catheter laboratories in Asan Medical Center around 7:00 AM. Recently the hospital was expanded and has more than 4,000 beds. It is a really huge hospital. However, it is not only huge but also best in quality of medical service.

Japanese 5 operators including me, Dr. Hee-Yeol Kim from the Catholic Medical University of Korea, St. Mary’s Hospital, who was only 1 operator from Korea, Dr. Park and his colleagues sat around the table. We reviewed 9 cases in total in 1 hour. All of the cases seemed very difficult. Dr. Park and Dr. Suzuki assigned the cases to the operators. Only 2 cases seemed feasible for the retrograde approach.

Two RCA-CTO cases were assigned to me. It looked impossible to take the retrograde approach. I sighed why I could not take the cases, for which the retrograde approach was possible. My condition was so bad due to the heavy drinking and incomplete sleep last night.

The 1st cases had very tortuous RCA, which was occluded from the proximal to middle part. I attempted the retrograde approach but failed as I expected. My assistant doctor was a beautiful Korean physician, So-Yeon Choi from Ajou University Hospital, Suwon. For this case, I spent 5 and half hours with 700 ml of contrast dye. It was my longest procedure in my life. I did IVUS-guided, double guidewire or triple guidewire techniques. Finally, a Conquest-Pro 12 passed into the true lumen. Since I created the big dissection from the proximal to distal RCA, I covered it by using one 3.0 x 24 mm Endeavor and three 4.0 mm Driver stents. The final result was fine. While doing the procedures, Dr. Nozaki from Hokko Memorial Hospital visited my laboratory. He did a great job in giving me a lot of helpful information.

The 2nd case also had a CTO lesion with calcification in the proximal RCA. I again attempted the retrograde approach but failed. However, the antegrade approach using a Conquest-Pro 12 guidewire was successful. I placed CYPHER stents. The result was fine. I finished this case in 1 hour. Dr. Jae-Hwan Lee from Chungnam National University Hospital in Daejeon assisted me.

After finishing these 2 case, I was really tired. I spent more than 7 hours in the catheter laboratories. I had no time to visit the auditorium at the 7th floor. I came back to my hotel and skipped faculty dinner.

January 6th Sunday 2008

Flying by JL 5234 from Kimpo, I reached Haneda Airport around 11:00 AM. My body smelled garlic. I took a lot of garlic in Seoul.

第二回CTO Live 2008 in Seoul

昨年に引き続き、新年早々ソウルを訪問しました。2008年1月4日金曜日13:05発の JL 5235便で羽田から金浦に飛びました。宿は江南区(カンナム)のGrand Intercontinental Hotelでした。Faculty Dinnerはキャンセルしてフグ屋さんに行きました。

韓国でもここ数年CTOに対する関心が高まり、AsanのPark先生は昨年第一回CTO Live 2007を開催しました。その時の術者は私、鈴木孝彦先生、朝倉先生そして土金先生で した。私はその年　2例のCTOを行いました。

Park先生は今年もCTO Live 2008と称する同様のライブデモンストレーションを企画 されました。今年は加藤　修先生も術者でした。

Asan Medical Centerカテ室横のカンファレンス・ルームに7:00 AMに集合しました。 この病院は最近拡張し、何と4,000ベッド以上あります。でも、巨大なだけでなく、その レベルも非常に高いのです。

日本からの5名の術者、韓国からの唯一のカトリック大学の金先生、Park先生、そし てその部下達がテーブルの周りに座りました。1時間ぐらいかけてその日行う予定であった 9例の症例を見ました。Park先生と鈴木先生が、「はい　これは誰々先生ね」っていう感じ で症例を割り振っていきました。逆行性に行けそうなのは2例ぐらいしかありませんでし た。

右冠動脈近位部のCTO2例が私に割り振られましたが、逆行性は無理そうでした。何 で逆行性アプローチできそうな症例が自分のところに来ないのか?　と嘆きました。前夜の 深酒と、睡眠不足のため僕のコンディションは最悪でした。

一例目は蛇行した右冠動脈の近位部閉塞でした。最初に逆行性を試みましたが、案の定 不可能でした。助手は美人の韓国のお医者さんであるChoi先生でした。彼女は水原市 (Suwon)の亜州(Ajou)大学病院の先生です。この症例には5時間半と700mlの造影剤を使 ってしまいしまた。僕の人生の中で最長記録です。色々やりました。IVUS-guide、double wire, triple wireを行いました。最終的にはConquest-Pro 12で真腔を捉えました。既に右 冠動脈近位部から遠位まで大きな解離ができていましたので、3.0mmのEndeavor一個と、 4.0mmのDriverステント3個を使いました。最後には綺麗に仕上がりました。手技の最中 に北光記念病院の野崎先生がカテ室に来られました。とっても有意義なコメントを頂きま した。

2例目も右冠動脈近位部に石灰化を伴うCTO病変を有していました。この症例も最初 逆行性を試みましたがうまく行きませんでした。でも、順行性にConquest-Pro 12を用い たら、うまくいきました。CYPHERステントを入れて終りました。この症例は1時間以内 に終了しました。大田市にある忠南国立大学のLee先生が助手をして下さいました。

2例を終えたらとても疲れていました。カテ室に7時間以上もいたのです。7階の会場 に行く時間は全くありませんでした。ホテルに戻り、会食はキャンセルしました。

JL 5234便で金浦から戻り、羽田には11:00 AM頃には到着しました。ソウルでたくさ んにんにくを食べましたので、私の体からはにんにくの匂いがしました。
