CTO Summit

Flying by JL 006 flight departing at 11:35 AM from Narita Airport on February 20 Wednesday 2008, I left for New York, USA to attend CTO Summit. The airplane arrived at JFK Airport around 11:00 AM. After checking in New York Marriott Marquis Hotel, I had lunch at Oyster Bar within the New York Grand Central Station, and then I had short sleep in my room. The dinner party of the day was held in Sushi Restaurant “15 East” with Dr. John Coppola, Dr. Kwok Tak, and their colleague from St Vincent’s Catholic Medical Center. Because we were not a small party, we had dinner at the table but not at the counter, where we can see very skillful cooking procedures by a famous Sushi chef, Mr. Masato SHIMIZU. After finishing the nice and pleasant dinner, I had tasty Japanese noodle at the restaurant “Soba Koh”.

I discussed about the coming St. Vincent’s TRI course with John. John passed me a CD of one patient for possible live demonstration candidate. John also proposed me a study protocol, which could be done together between Japan and US. It will be very much interesting. I will discuss with my staffs after coming back to Japan.

I waked up at 3:00 AM due to severe jet lag. My eyes were red. Since I was assigned to a live operator in the 1st live session, I left for the Presbyterian Hospital of Columbia University at 7:00 AM. The case assigned to me was 45-years old male patient after CABG. The target lesion was a CTO lesion of > 5 years in his native RCA. My assistant doctors were a lady doctor Dr. Roxana Mehran and Dr. Kotaro Obunai. I started with bi-femoral approaches. The case looked not so difficult. However, I had to wait for > 1 hour before live transmission. Roxana showed a new technology, which was combining CT angiogram with standard angiogram. I crossed the lesion by using a Miracle 3g guidewire and placed 5 CYPHER drug-eluting stents in total. While doing the procedure, Dr. Antonio Colombo came in the room frequently to check my procedure. The final result was complete. I am happy for the result.

Before coming back to the meeting venue, which was Marriott Marquis, I had lunch at a Japanese Ramen Restaurant “SETAGAYA”, which is the most famous Ramen restaurant in New York. I arrived there before noon. I waited for the opening in front. I watched at a bicycle there. It was striking to see a seal with “Vote OBAMA” attaching to the bicycle. The taste of “Shio” Ramen was acceptable in Japanese standard. I did not satisfy with “Tsukemen”.

Recently, many famous Japanese Ramen restaurants are coming to New York. Before last year, the taste of Ramen in New York was terrible. However, the situation was totally changed now. It is worth having Ramen in New York now.

When I came back to the hotel, a naked gentleman with pants alone was playing guitar at the middle of the street in front of Times Square. It was minus 2 degree outside. The afternoon session started from 1:30 PM. I had a busy schedule in the afternoon. I was one of the moderators for live case transmission between 1:30 PM and 4:00 PM. I had one lecture after that. My roles were finished around 5:00 PM.

I had dinner with Dr. Ikeno at “15 East” again. I sat in front of Mr. SHIMIZU. The taste is always nice. I think the level of the taste is at high standard even in Ginza, Tokyo. Octopus and sea urchin were especially nice. After “15 East”, we had SOBA at “Soba Koh” again. The taste of SOBA in this restaurant seems much better than in any SOBA restaurants in Japan. The lady in “Soba Koh” was very friendly.

I waked up at 5:00 AM. It was heavily snowing outside. I left the hotel relatively early at 8:30 AM for JFK Airport. Fortunately, the Airport was not closed. My returning flight JL 005 took off the Airport around 14:00 PM, which was almost 1 and half hours after the schedule.

2008年2月20日水曜日11:35 AM成田発のJL 006便でCTO Summitに出席するために New Yorkに向かいました。JFK空港には11:00 AM過ぎに到着しました。New York Marriott Marquisホテルにチェック・インしてからGrand Central StationにあるOyster Barで昼食を 摂り、それからホテルで少し休みました。夕食はSt. Vincent病院のJohn Coppola先生、Kwok Tak先生達とおなじみの「15 East」に行きました。ここの板前さんであるKIMURA Masato さんはとても腕が良いのです。何時もはカウンターで食べるのですが、この日は人数が多かった のでテーブルで食べました。「15 East」の後はこれも行きつけの蕎麦屋さん、「Soba Koh」でお いしい蕎麦を食べました。

Johnとは今度開催するSt. Vincent TRI courseについて色々話しました。Johnは私にその 時の候補の症例のCDを渡しました。また、Johnから日米共同臨床研究の提案がありました。 とても面白そうなので帰国後検討するつもりです。

ひどい時差ぼけのため3:00 AMに起きてしまいました。目が真っ赤でした。僕は最初のラ イブ・セッションの術者になっていましたので、7:00 AMにコロンビア大学プレスビテリアン 病院に出かけました。僕の症例は45歳の男性で冠動脈バイパス手術後の症例でした。標的は右 冠動脈の慢性完全閉塞病変で5年以上の閉塞歴がありました。僕の助手にはRoxana Mehran 先生と、Obunai先生がついてくれました。両側大腿動脈アプローチで行いました。症例はそん なに難しくありませんでした。しかし、ライブ中継まで1時間以上も待たねばなりませんでし た。Roxanaは新しいテクノロジーを示しました。それはCTアンジオと冠動脈造影を重ねて表 示するものでした。病変をMiracle 3でクロスし、CYPHERを5個植え込んで終了しました。 自分が手技を行っている間、コロンボ先生が何をしているかを度々チェックに入ってきました。 最終結果は完璧でした。とても満足です。

学会場のMarriott Marquisホテルに戻る前に、ラーメン「せたが屋」に行きました。今 New Yorkで一番有名なラーメン店です。正午よりも前に着いたのでラーメン店の前で待ちまし た。店の前に停めてあった自転車を見たら驚くべきことに「オバマに投票を」というシールが貼 ってありました。塩ラーメンの味は日本の立場から見ても満足すべきものでしたが、つけ麺は良 くありませんでした。

New Yorkには最近日本の有名なラーメン店が進出してきています。昨年前まではNew York のラーメン事情は最悪でした。しかし、今やNew Yorkのラーメンはもっともホットです。是非 お試しあれ。

ホテルの近くではパンツ一丁の裸の男性がTimes Square前の通りの真ん中でギターをひ いていました。外気温はマイナス2度でした。午後のセッションは1:30 PMから始まりました。 午後は色々な役目で忙しかったのです。1:30 PMから4:00 PMまでライブのモデレーターを行 い、その後講演などが5:00 PMまで続きました。

夕食は池野先生と「15 East」で食べました。板前さんの清水さんの前に座りました。味は いつものようにとても良く、銀座のレベルでもハイクラスと言えるでしょう。中でも蛸とウニは 絶品でした。「15 East」の後は、「Soba Koh」で蕎麦を食べました。ここの蕎麦は日本のどんな 蕎麦屋さんよりも良いです。「Soba Koh」の店の女性はとても親切でした。

5:00 AMに起きました。外は激しい雪でした。このため、ホテルを予定よりも1時間早く 8:30 AMに出てJFKに向かいました。幸にも空港は閉鎖されていませんでした。帰国便のJL 005 は空港を14:00頃になって飛び立ちました。予定よりも1.5時間ぐらいの遅れでした。
