After “IPPUDO”, I visited Japanese noodle shop “Saba-Koh”. The taste was nice as always. I still believe that Soba (= Japanese noodle) is best here in the world. It was pity that I could not see the chef, Mr. Takahashi.

I came to NY to attend TRI workshop held in St. Vincent’s Catholic Medical Center Hospital, which is the oldest hospital in NY. Dr. John Coppola is the director of Cardiology and catheter laboratories. He is a good friend of mine. The hospital is famous for training young fellows. Many young physicians want to be trained in St. Vincent’s Hospital. I heard that only 1 among every 40 to 50 young physicians could pass the entrance examination for the fellowship course in St. Vincent’s Hospital. Six relatively young Interventional fellows came to the hospital from all around the US to learn how to do TRI. After finishing the dinner at “Soba-Koh”, I came back to the hotel. It was snowing outside.

It was completely white outside due to snowing. I went to the hospital before 9:00 AM. I was expecting to do TRI cases. However, Dr. Coppola arranged me a very difficult CTO case. The patient was a Chinese 60+ years old male. He was living in China and US. He could not speak any English. He received BMS stents implantation in mid RCA more than 10 years ago in China. Because of recurrent effort angina, he received coronary angiography last year in St. Vincent’s Hospital, which revealed a long CTO lesion in RCA starting from the middle of the stents and ending just at the bifurcation between 4PD and 4AV. The total length of the occlusion was more than 5 CM. There was no nipple at the both ends of the occlusion. PCI was attempted from the antegrade approach by using a 7-French TFI and double wire technique. However, it failed and created dissection in the CTO lesion. I was not preparing for CTO cases. There were no short guiding catheters. I started from bi-femoral approach by using a 7-French EBU 3.5 and a 6-French AL1.0 guiding catheters. I cut and re-connected the 7-French guiding catheter outside the body. Strikingly the patient was tall, and the guiding was too short. There was only 1 7-French EBU remaining in the laboratory. I first engaged it into the LCA, and then cut and re-connected. Starting from the combination of a Finecross and a Runthrough guidewire, I tried to search for the possible connection from LCA to RCA. The epicardial diagonal route was not connected. I exchanged the wire to a Fielder-FC. Finally, I found a connection through a septal branch. I could put the tip of the Finecross into the distal RCA. Because there was no nipple at the corner, it was really difficult to select the proximal RCA. I took a Miracle 3G and a Confienza-Pro guidewires. I took a Corsair (= Channel dilator). Finally, the wire went close to the stents. I started the antegrade approach by using a Finecross and a Miracle 3G wire. The wire easily went into the subintimal space created at the previous failed PCI. I decided to do the “Reverse CARTO” technique. I enlarged the dissected space by using a 2.5 mm balloon. After this balloon dilatation, a Fielder-FC guidewire successfully reached the proximal true lumen from the retrograde approach. Fortunately the retrograde wire entered into the antegrade guiding. By using the traction anchoring technique, I could put the Corsair into the antegrade guiding. I exchanged the wire to a 300-CM guidewire and connected between both ends of the 2 guiding catheters. After that, everything became simple. I finally placed 5 Xience DESs. The final result was so beautiful that everybody in the laboratory got happy. The procedure took less than 2 hours. I used only FDA-approved products.

Since I was sleepy and happy for the PCI result, I proposed John to come back to the hotel and see him at the dinner. After leaving the hospital, I visited another ramen shop “SETAGAYA”. I felt the taste went down a little. There were 3 young people working in the shop. All of them were not Japanese but Korean. That might be the reason why the taste went down.
After taking short sleep in the hotel, I joined the dinner with doctors in an American Japanese restaurant. After this dinner, I visited “IPPUDO” again and had “KARAKAMEN”. The taste was also nice. If you visit in NY, you have to visit “IPPUDO”.

I was supposed to take a UA 2775 flight to Washington DC. However, because of heavy snowing, I took an AMTRAK train from NY station departing at 8:00 AM.

Visiting Washington Hospital Center
It took more than 3 hours to Washington Union Station. Everything was covered by white snow on the way. After checking in the Omni Shoreham Hotel, I visited Washington Hospital Center to teach how to do transradial access. This hospital is one of the biggest hospital in US and very important to the US Government. They are doing more than 10,000 catheter laboratory cases including more than 5,000 PCIs annually. When I visited the catheterization laboratories, Dr. August Pichard and other doctors were waiting for me. Although the laboratories were busy in the preparation for live transmissions to CRT meeting next day, he kindly introduced me the laboratories. They had 11 catheter laboratories for PCI, EVT and Valvular diseases. EP team has 5 independent catheterization laboratories as well. Off course, Pediatric Cardiology has its own laboratories. After having light lunch in the office with Dr. Pichard and Dr. Ron Waksman, I was taken to a catheter laboratory. There was one male patient lying. I was asked by Dr. Pichard to teach how to do transradial diagnostic angiography. I was very impressed for having this kind of opportunities in so famous hospital, Washington Hospital Center.

During this long trip starting from February 26th, I first visited a SUSHI restaurant in Washington DC. The name was “MAKOTO”. It was nominated as the top 3 restaurant in Washington DC by some food magazines. However, I was not satisfied with the quality. I never recommend any people to visit it.

I was busy whole day due to many roles in CRT 2009 meeting. CRT is “Cardiovascular Research Therapeutics” and run by Dr. Ron Waksman in Washington Hospital Center. The meeting was held in my hotel. I did several lectures and chairs during the CTO forum. I also received a TV interview by “Cardiotube”, which is a web cast program. You may see my interview on the website soon.

I had dinner at a Korean restaurant “Woo Lae Oak” with Dr. Ikeno from Stanford University. The restaurant used to be located at Arlington near Washington DC and a real high-grade Korean restaurant. However, it was completely burned down by a fire 2 years ago. Now, it was moved away, taking more than 30 minutes by taxi from Washington DC. Everything was changed. I was again not satisfied with the taste there. I never recommend anybody to visit it.

The 1st TRI forum during CRT meeting was held from 7:00 AM on the day. I did lectures and chairs during the meeting. I was wondering whether we could have many participants, since the meeting started very early. Confounding my expectation, many people attended the forum. I was quite sure that many US physicians wanted to start TRI.
After finishing this forum, I moved to Washington Dulles International Airport to catch the NH 0001 flight for Narita. The flight was delayed by 1 hour. I finally arrived at Narita Airport around 4:20 PM on March 5th Friday 2009. I directly moved to Yokohama to do a lecture in Kanagawa PTCA Conference.
This world round trip was busy but very fruitful for me. I learnt much. I could visit Washington Hospital Center. I found that “IPPUDO” in New York was very nice.

St Vincent病院でのTRIワークショップ
AF 006便でパリからニューヨークに飛び、2009年3月1日15:30にJFK国際空港に到 着しました。外は寒く感じました。ホテルはとても良いRitz Carltonホテルでした。ラーメ ン店の「一風堂」で夕食をとりました。この店は2008年4月に開店しました。初めて訪れ たのですが、たくさんの人たちが行列を作っているのに驚きました。バーで30分待ちまし た。店はとても広く、78テーブルもあります。驚いたことに完全なオープン・キッチンで した。たくさんの若い人たちが働いているのですが、そのほとんどが日本人でした。お客さ んは厨房で何をしているか、全て見ることができます。こんなにオープンなのはこの「一風 堂」以外では見たことありません。メニューとしては、いわゆるアメリカ風和食のつまみを たくさん取り揃えていますが、もちろん主菜は博多豚骨ラーメンです。「赤玉」を頼みまし た。とってもおいしくて、日本で食べるのと何ら変わりませんでした。

「一風堂」の後は、「蕎麦こう」に行きました。何時もながらとてもおいしい味でした。 正直、ここの蕎麦が世界で一番おいしいと思います。でも、高橋店長がおられなかったのが 少し残念です。

セントビンセント病院で開催されたTRIワークショップのためにNYに来ました。この 病院はNYで一番歴史のある病院で、その循環器科およびカテ室のトップがCoppola先生で す。彼とは良い友人です。病院は研修医に人気があり、若い医師はここで研修したがります。 何でも研修医試験の合格率は40から50人に一人しかいないそうです。今回全米から6名の 比較的若いインターベンション医師が参加し、TRIについて学びました。「蕎麦こう」で夕 食を食べてからホテルに戻りました。外は雪が降っていました。

雪のため外は真白でした。病院に9:00 AMに行きました。TRIの症例をやると思ってい ましたが、Coppola先生はとても難しいCTOの症例を準備されていました。患者さんは60 歳代の中国人男性でした。彼は半分半分中国とアメリカに住んでいますが、英語は全く喋れ ません。10年以上前に中国で右冠動脈の中ほどにBMSを植えています。狭心症が再発した ため、去年この病院で冠動脈造影を受けましたが、結果は、ステント中程から右冠動脈末梢 分岐部までの長い慢性完全閉塞でした。長さは5CM以上あります。2月にこの病院で7Fr TFI により順行性にdouble wireテクニックを用いてPCIが試みられましたが、不成功に終わり、 解離を作ってしまいました。僕はCTOのための準備をしていませんでした。もちろん、短 いガイディング・カテーテルはカテ室にありません。とにかく大腿動脈両側穿刺で開始し、 7Fr EBU3.5と6Fr AL1.0で入りました。体の外でEBUを切り、6Frシースで繋ぎました。 しかし、驚いたことに患者さんは大きな人で、何と届きませんでした。カテ室には7Fr EBU は後一本しかありませんでした。まず、左冠動脈に挿入し、その状態で切り、つなぎました。 Finecross + Runthroughで開始し、Fielder-FCに交換し、何とか逆行性ルートは無いか探し ました。対角枝のルートは繋がっていませんでした。幸い、中隔枝が繋がっていました。 Finecrossを右冠動脈末梢に持っていきました。でも、分岐部はツルツルで何のとっかかり もありません。Miracle 3やConquest-Proを用いて何とか入っていきました。ここでCorsair (= Channel Dilator)に交換しました。そして何とかステントの近くまで達しました。ここで、 Finecross + Miracle 3を用いて順行性アプローチを開始しました。もちろん、前回作成され た解離腔に簡単に入ります。そこで、”Reverse CARTO”を行うことに決めました。2.5mmバ ルーンで解離腔を拡張し、Fielder-FCを逆行性に右冠動脈近位部までクロスすることに成功 しました。幸い、このワイヤーは順行性ガイディング・カテーテルの中に入りましたの で、”Traction Anchor”をかけながら、Corsairをガイディング・カテーテル内に持ち込みま した。後は、300CMのガイドワイヤーでループを作り、その後は簡単でした。最終的にXience 薬剤溶出性ステントを5個植え込んで終了しました。とても綺麗な結果に、カテ室の皆が喜 びました。手技時間は2時間を切りました。結局、FDAに許可されているディバイスのみ で治療しました。

眠かったし、PCIの結果も良かったので、早々にお暇することをCoppola先生に申し出 ました。病院を出てから、別のラーメン店「世田谷」に行きました。味が少し落ちた気がし ます。店には三人の若い人が働いていましたが、彼らは日本人でなく韓国人でした。これが 味の落ちた原因かも知れません。
ホテルで短い睡眠をとってから、アメリカ風和食の店でお医者さん方と夕食を摂りまし た。その後、またまた「一風堂」に行き、今度は、「カラカメン」を食べました。味はまた もや良く、NYに来たら是非とも行くことをお勧めします。

ワシントン病院センター訪問
ワシントン・ユニオン駅までは3時間余りの旅でした。窓から見える景色は完全に雪で 覆われていました。Omni Shorehamホテルにチェック・インしてから、ワシントン病院セ ンターでTRIを指導するために出かけました。この病院は全米でも随一の病院で連邦政府 にとっても重要な病院です。そこでは年間10,000例以上の冠動脈造影などが行われ、その 内の5,000例以上がPCIです。私がカテラボを訪れると、August Pichard先生たちが待って おられました。翌日からのCRTのライブに向けてカテラボ内は忙しそうでしたが、Pichard 先生は親切にカテラボを案内して下さいました。そこには冠動脈、末梢血管、そして弁治療 用に11のカテ室があり、それ以外に、EP用に5つ、そして小児用にも別にカテ室があるの です。先生のオフィスでPichard先生、Ron Waksman先生たちと軽い昼食を摂ってから、 カテ室に行きました。一人男性患者さんが既にカテ台に寝ておられました。Pichard先生に 経橈骨動脈冠動脈造影の仕方を教えるように頼まれました。こんな有名なワシントン病院セ ンターでそのような機会を持てて幸せでした。

2月26日から始まったこの長い旅の中で、寿司屋さんには一回も行きませんでしたが、 このワシントンDCで初めて行きました。その名前は「まこと」というもので、色々な雑誌 ではワシントンDC全ての飲食店の中で3位にランクされている店です。でも、僕はその質 に満足できませんでした。誰にも勧めません。

一日中CRT 2009学会での役割で忙しく過ごしました。CRTは”Cardiovascular Research Therapeutics”の略であり、ワシントン病院センターのRon Waksman先生が主催されていま す。学会は私の泊っているホテルで行われました。私はCTO Forumでいくつかの講演と、 座長をしました。それから、CardiotubeというTVインタビューを受けました。これはWeb 上のテレビです。その内、そのwebsiteで見ることができます。

“Woo Lae Oak”という韓国料理店でStanford大学の池野先生と夕食を摂りました。この レストランは以前ワシントンDC近くのアーリントンにあり、とても質の高い韓国料理店で した。でも、2年前に火事で焼けてしまいました。今は、DCからタクシーで30分以上かか る郊外にあります。全てが変わってしまいました。ここでも味に満足できませんでした。誰 にも勧めません。

この日の7:00 AMからCRTでは初めてのTRI Forumが開催されました。私は、講師お よび座長でした。朝早いので参加者がいるかどうか心配でした。でも思いがけずも、たくさ んの人が参加されました。このアメリカでもTRIが確実に増加するでしょう。
このForumを終えて、成田行きNH 0001便に乗るため、ワシントン・ダレス空港に向 かいました。飛行機は1時間ぐらい遅れ、2009年3月5日金曜日4:20 PMに成田空港に到着 しました。そこから、横浜に神奈川PTCAカンファランスでの講演のため、横浜に直接向か いました。
この世界一周の旅はとても忙しいものでしたが、自分にとってとても有意義でした。た くさんのことを学びました。ワシントン病院センターも訪問することができました。ニュー ヨークの「一風堂」がとても良い、ということも知りました。
