After 4 hours’ flight, the IC 854 flight arrived in Delhi International Airport around 8:30 PM on July 11. We moved directly to a 5-stars hotel “The Oberoi”, where I spent 2 nights. The hotel is a beautiful venerable but modern hotel and built next to the Delhi Golf Country Club. I was very surprised to know they had such a huge golf course just in the center of Delhi. I had good dinner in the modern cafeteria within the hotel, which was multi-national food restaurant. I had good SUSHI dish there.

The Great Professor Ashok Seth left Escorts Heart Center and established a new private hospital. The formal name of the hospital is “Max Devki Devi Heart & Vascular Institute”. He and his colleagues joined the opening of this new hospital. I arrived at the hospital by 10:00 AM. Professor Ashok kindly welcomed me. After meeting with Professor Ashok, I had to sign my signature on the Certificate for Participation documents for almost 60 participants. The auditorium of the hospital was opened for the live transmission from the catheter laboratory. We had an in-hospital symposium titled as “Symposium on PTCA ? The Journey So Far: Entering New Vistas and Trans Radial Intervention”, which was sponsored by Max Heart & Vascular Institute.

After reviewing the case CDs with all of the attendants together, I went down to the catheter laboratories, where 2 newest Philips’ flat-panel single-plane machines were installed. They arranged 6 difficult cases including CTO for me. I did half of these cases through the right radial approach and remaining half through the femoral approach, and left the hospital by 6:00 PM. One of the case with a CTO lesion in the middle LCX artery developed late cardiac tamponade due to the perforation of a stiff guidewire.

Another Symposium sponsored by Max Heart & Vascular Institute was held in Marriott Hotel New Delhi from 8:30 PM. There were a lot of participants. After introduction of me by Professor Ashok, I made a lecture on PCI for CTO lesions. I heard that Max Heart & Vascular Institute hold these kinds of symposiums for general physicians almost once every 2 weeks in order to increase the patients referred from those general physicians. It is a kind of Marketing and Sales Promotion.

The day was a really memorable day for me. It was a real great experience for me to do PCI together with Professor Ashok Seth. I know him for many years since 1990. He is really one of the world-famous greatest Interventional Cardiologists working in Asia

I waked up early and visited the highest-rank Medical University in whole India, All Indian Institute of Medical Sciences (AIIMS). This Medical University is the most difficult to enter and graduate for students in India. I was invited to deliver a lecture on PCI for CTO lesions during the morning conference of Interventional Cardiology Doctors starting from 8:00 AM. After finishing the lecture by 9:00 AM, I hurried to the catheter laboratories of the hospital.

There were 4 laboratories. We used 2 laboratories among them, where Siemens machines were installed. Total number of PCI procedures in AIIMS is more than 2,000 every year. I did 7 cases very quickly there mainly through the right radial approach including the patients with CTO lesions. Everybody was not only very eager to learn how to do TRI and how to manage CTO lesions, but also very much kind to me. I could do all of the procedures very smoothly and comfortably. I really thank for their kindness.

Because this hospital is an Indian Government Hospital, many patients there are not rich and cannot pay the cost for such expensive drug-eluting stents as CYPHER or TAXUS. I placed many made-in-India drug-eluting stents. One was a Paclitaxel-eluting stent, and another Sirolimus-eluting one. Both stents are completely made in Hyderabad, India. I was very satisfied with their performance as stents. I was surprised to hear that they were planning to conduct FIM (First In Man) trial for a new Paclitaxel-Eluting Stent manufactured in the same company together with Columbia University Hospital in New York, USA. That new stent is a combination of Cobalt-Chromium stent, Cabon-Nanotube coating and Paclitaxel. I felt that India would run at the top of the front line in the field of Interventional Cardiology very soon. I left the hospital around 12:30.

After finishing lunch in an Indian Restaurant in a Hotel, I arrived at Escorts Heart Institute & Research Centre around 3:00 PM, where Professor Ashok Seth used to be the director of Interventional Cardiology by the autumn 2004. This hospital specific for Cardiology has been very famous and authoritative one not only in whole India but also in the world. The hospital building is very beautiful. Now, Professor TS Kler is directing the whole Cardiology team.

Professor Kler kindly invited me into his nice office, where many honorable plates were displayed. I was very happy to realize that I could finally reach so famous Cardiology Hospital as Escorts Heart Institute. After discussing several matters, I moved to the catheter laboratories. They have 4 independent laboratories and perform more than 2,500 PCI cases every year. I used one of the 4 laboratories. The X-ray machine was a GE’s INOVA 2000 flat panel machine. I did 4 cases there. Dr. Sanjay Chugh was doing a lot of cases through TRI. He told me that TRI was getting more popular and popular also in Escorts Heart Institute, since they have too many patients for limited number of beds. They really want to shorten the average hospital stay after PCI. Thus, TRI is really demanded in those high-volume centers like Escorts Heart Institute.

Since I had to come back to the hotel by 10:00 PM in order to pack my luggage, I left the catheter laboratories after 9:00 PM, where we had still many patients. Before leaving the laboratories, we took several memorial photos together. Those photos are very valuable for me.

When I walked out from the entrance of Escorts Heart Institute, it was, of course, dark outside, so that I could not take pictures of the hospital clearly. While coming back to the hotel, I could fortunately take the photo of Indian Gate. This picture was my only symbol for sightseeing in Delhi this time.
After having dinner and packing my luggage in the hotel, I moved for Delhi International Airport, and took a Thai Airline TG 316 flight departing at 1:05 AM on July 14. After smooth transit in Bangkok Airport, I came back to Narita International Airport before 5:00 PM on July 14 Thursday, 2005 by JL 708 flight. Again, this trip to India was very short. However, it was very wonderful and fruitful for me. I felt that I could make another forward step in Interventional Cardiology and in my life.

バンコク行き成田発JL 717便に乗りました。この便は2005年7月11日月曜日午前11時 発の予定でしたが、あいにく成田空港では強風のため発着便がストップし、結局一時間遅れで出 発しました。バンコク空港で1時間の接続時間の後で、デリー行きインド航空IC 854便に乗り ました

4時間の飛行の後で、IC 854便は無事にデリー国際空港に到着しました。時刻は7月11日 午後8:30分でした。そのまま五つ星ホテルであるThe Oberoiにチック・インしましたが、こ のホテルは由緒ある美しく、かつモダンなホテルです。そして、隣にはデリー・カントリー・ク ラプというゴルフ場があります。デリー市内の真ん中にこのようなゴルフ・コースがあるなんて 驚きです。ホテルの中の多国籍料理レストランで、何と寿司を食べましたが、とてもおいしい味 でした。

偉大なAshok教授は、それまで勤めていたEscorts心臓病センターを去り、新たに民間病 院を設立しました。その病院の名前は“Max Devki Devi Heart & Vascular Institute”といいます。 Ashok先生とその部下が新しい病院に移りました。私は、朝10時前に病院に着きました。Ashok 教授が招いて下さいました。暫く色々と話し合った後、60名ぐらいの参加者に対する参加証書 にサインせねばなりませんでした。病院の講堂を用いてライブデモンストレーションが行われま した。そして、院内で「経皮的冠動脈インターベンションについてのシンポジウム ? 我々は何 処まで来たか? 新たな展望そして経橈骨動脈冠動脈インターベンション」という名のシンポジ ウムをMax心臓センター主催で開催しました。

参加者と共にCDを供覧した後、私はカテ室に降りました。カテ室には新しい2台のフィリ ップス社製のフラット・パネルがありました。CTO症例を含む6例が準備されていました。半 分の症例はTRIで治療しましたが、残りはTFIで治療しました。そして、夕方6時にカテ室を 退きました。回旋枝のmid portionのCTO症例は、ガイドワイヤー穿孔のため後で心タンポナ ーデとなってしまいました。

Max心臓病センターは、午後8時半からマリオット・ホテルでもう一つシンポジウムを開 催しました。ここにはたくさんの聴衆が集まりました。Ashok先生に紹介されて、私はCTOに 対するPCIの講演をしました。何でもMax心臓病センターはこの類のシンポジウムを2週間に 一回程度開催し、開業医よりの紹介患者さんを増やそうとしているそうです。要するに販売促進 のためのマーケティングです。

この一日は私にとって記念すべき日でした。Ashok先生と一緒にPCIを行ったことは、私にとって 本当に思い出となることでしょう。1990年当時より彼のことをアジアにいながら世界的に有名な インターベンションの医師として尊敬していました。

この日は朝早く起き、インド国内で最高ランクの医学部を訪れました。それは、全インド医 学研究所(All Indian Institute of Medical Sciences: AIIMS)です。この医学部は入学および卒業 がインド国内で最も難しい大学です。朝8時から始まる循環器科の朝のカンファレンスでの CTOに対するPCIの講演を依頼されました。講演を9時に終えて、カテ室に向かいました。

ここにはカテ室が4つありました。その中の2つのカテ室を用いてPCIを行いましたが、 機械はシーメンスのものでした。AIIMSでは年間2,000例のPCIが行われています。この日は CTOを含む7例のPCIを主にTRIで行いました。みなは、TRIをどうやるか、そしてCTOに 対してどのような戦略で行うか、を学ぼうと一生懸命でしたが、とても親切でした。ですから、 とても気分良く上手にPCIをすることができました。この親切に感謝します。

この病院はインド国立病院の一つですので、患者さんの多くは裕福ではありません。ですか ら、CYPHERやTAXUSのような高いDESに対して支払いをすることができません。私は、 インド製のDESをたくさん用いました。一つは、Paclitaxel-eluting stentであり、もう一つは Sirolimus-elutingのものでした。両方ともインドのハイデラバードで作られたものです。ステ ントの出来はとても良く、その性能には満足しました。更に驚くべきことに、ニューヨークのコ ロンビア大学と共同で、コバルト合金製ステントに、カーボン・ナノチューブを用いてPaclitaxel をコーティングした新しいステントの人類初のトライアル(FIM: First In Man)を行うのだそう です。インドは間違いなくPCIの世界で研究開発の最先端を近い将来歩むことになるでしょう。 私は病院を12:30に出ました。

ホテルのインド料理レストランで昼食を終えてから、Escorts心臓病センターに午後3時頃 到着しました。ここのカテ室の長はかつてAshok教授でしたが、彼が2004年の秋に退職され た後、Kler教授が循環器科のトップで皆を引っ張っておられます。この病院はインド国内のみ ならず、世界的に有名な循環器専門病院です。建物もとても奇麗です。

Kler教授は私を部屋に招いて下さりました。室内には様々な表彰状が飾られていました。 つにい、この有名なEscorts心臓病センターにまで来ることが出来た、と思うと感慨深いものが ありました。いくつか話し合った後、カテ室に行きました。カテ室は4つあり、年間2,500例 以上のPCIが行われています。機械はGEのINOVA 2000でした。ここでは4例行いました。 Chugh先生は既にたくさんのTRIの経験があられます。彼によれば、TRIはインドでもそして Escorts心臓病センターでも急速にポピュラーになってきたということです。Escortsでは患者 さんの数が多く、ベッドが足りないので入院日数の少ないTRIが実情に即しているそうです。 Escortsのような症例数の多い病院ではTRIは本当に必須のテクニックなのです。

荷物をまとめるためには午後10時前にはホテルに戻る必要がありました。そこで、午後9 時過ぎにはカテ室を出ました。しかし、まだカテ室にはたくさんの患者さんがいました。去る前 に、皆で記念写真を撮りました。これらの写真はとても記念になります。
Escorts心臓病センターを出た時には、既に真っ暗でした。ホテルに戻る途中で、やっとの ことでインド門の写真を撮ることができました。今回のデリー訪問の唯一の観光写真でした。
ホテルで荷物を纏め、夕食を摂ってから、デリー国際空港に向かいました。そして、7月14 日午前1:05発のバンコク行きタイ航空TG 316便に乗りました。バンコクでトランジットをして、 7月14日午後5時にJL 708便で成田空港に戻りました。今回のインド訪問は非常に短い旅で した。でも、とても意義深いものでした。そして、私のPCIの歴史の中で大きな意味を持ちも したし、それだけでなく、私の人生の中で大きな意味を持つ旅となりました。
