Next day was free for me. There were many Japanese tourists and businessmen staying in the Hotel Golden Tulip. I had breakfast early. After the breakfast, I walked around the hotel. There was a big bridge overriding a big river. The bridge is Erasmus Bridge. A big cruising ship was at anchor along the riverside.

There were river taxies connecting between a small port behind my hotel and a historical hotel “Hotel New York”. Dr. Tsuchida kindly introduced a nice lunch in the restaurant within the historical beautiful Hotel New York. Many people moved to New York from Rotterdam in the past. There was a golden signboard displaying “Holland America Line” on the outside wall of the hotel. After finishing lunch and having short rest in my room, we had dinner together with Dr. Masato OTSUKA and Dr. Syuzo TANIMOTO in a Japanese Restaurant “Sapporo”. Dr. Otsuka was studying cardiac radiology under Radiology Department and Dr. Tanimoto was studying under Professor Serruys. We had pleasant Japanese dinner.
As always I suffered from severe jet lag. I waked up very early. I went to Erasmus University Medical Center by 7:00 AM. After George came in the catheter laboratory, we went to the room of the patient. George introduced me to the patient. Because of the previous twice failed attempts of PCI for the CTO lesion in his RCA from his right groin, the femoral pulsation was very weak. Because I was planning to use the right brachial artery for the retrograde approach, I checked his right brachial pulsation. However, his pulsation was again very weak probably due to his fatty arm.

The patient was transferred to No. 3 catheter laboratory around 7:30 AM. The battle of the day started. Professor Patrick Serruys started his case in the other catheter laboratory, where the latest technology machines like Stereotaxis or ultrasound ablation were installed. We started our cases almost simultaneously. That was Professor Serruys’ idea. He would show the latest technology for CTO lesions to the audience, and I was supposed to show the latest Japanese techniques to these lesions.
Patrick’s case had a short CTO lesions located in the straight middle part of LAD. It seemed not so difficult. My case looked very difficult. While scrubbing, I was not so strained. I told to myself, “I can do only what I can do. However, today I will do everything which I can do.” After several difficult attempts, George finally got the right femoral cannulation by using a 7 French introducer. I was asked to do the right brachial puncture, since he has had no experiences to do it. The pulsation was weak. However, I succeeded in putting a 7 French introducer into the brachial artery. I put a 7 French 85-CM BL3.5SH guiding catheter (TERUMO) into the left coronary artery from the right brachial artery and JR 4.0SH guiding catheter (Medtronic) into the right coronary artery from the right femoral artery. By doing the simultaneous bilateral dye injection, we found that the TAXUS stent previously placed proximal to the CTO lesion had been completely occluded. Thus, the previous failed attempt made the condition much worse. The length of the CTO lesion became longer.
As planned previously, I started to do the retrograde approach by tracking one of the septal branches. By using the combination of a Finecross microcatheter, a Ryujin-OTW 1.25-mm balloon and a Fielder hydrophilic guidewire, I could reach the distal end of the CTO lesion retrogradely. Because of the difficulty in achieving the good backup support by the guiding catheter for the right coronary artery, I first placed and inflated a 1.5-mm balloon in the conus branch to anchor the JR 4.0 guiding catheter. If there would be no occluded stent, I could finish the all of the procedures successfully and very quickly within 1 hour. However, it was very difficult to cross the occluded stent from antegradely or retrogradely. However, finally I was successful to cross the lesion. I placed 3 TAXUS stents by using a 5 French child guiding catheter through a 7 French guiding catheter. George was not able to understand what I was doing. The final result was beautiful. Dr. Antonio Colombo was chairing the live session in Paris. Dr. Mitsudo was one of the moderators for my live case session.
In another room, Patrick was not successful to cross the lesion, while I was successful. Since I was not in the auditorium, I could not feel the response from the audience. However, I heard that all of the audience cheered with a storm of applause for my great success. Not only I but everybody in the catheter laboratory was happy for this success.
Because I had to fly to Toulouse from Amsterdam in the afternoon, I left the hospital after saying a good bye to Professor Serruys. Flying by AF 8263 flight departing Amsterdam at 14:40, I reached Toulouse after 2 hours’ flight around 5:00 PM. A car arranged by the EuroPCR organizing committee was supposed to wait for my arrival. However, I could not find any car waiting. I took a taxi to the hotel. The hotel was the best hotel in Toulouse “Grand Hotel de L'Opera” in front of the city hall of Toulouse “Captolium”. I had a nice dinner with Dr. Bruno Farah in the restaurant “Les Jardins de L’Opera” within the hotel.

この招待は私にとってとても光栄なものです。しかし、その一方ではとてもストレスフルな ものでした。いわば日本を代表してのものでした。私は、EuroPCR 2004でもトラックスセン ターから慢性完全閉塞のライブデモンストレーションを行いましたので、この施設については良 く知っていました。
2006年5月15日月曜日12:45発のJL 411便に乗ってアムステルダムに飛びました。アム ステルダムのスキポール国際空港に到着したのはその日の午後6時頃でした。そこから車1.5 時間ぐらいかけてでロッテルダムのGolden Tulipホテルに向いました。土田先生がホテルで待 っていて下さいました。彼は、数年前に私の病院で研修し、2年前からSerruys教授の下で研修 しています。患者さんのCDは既に私のもとに届いていました。患者さんは60歳台の男の人で 労作性狭心症でした。右冠動脈近位部で完全閉塞し、トラックスセンターで2回PCIが試みら れましたが不成功でした。最初にこのCDを見た時の正直な印象は、「何てひどい症例だ! この 症例は世界で一番難しいものに違いない。何で私が、世界で一番大きなライブデモンストレーシ ョンEuroPCR 2006でこの症例をライブデモンストレーションで治療せねばならないんだ!」と いうものでした。
Parkホテルの中のレストランで、9:00 PMからSianos先生、主任ナース、その他のお医者 さん達と夕食を食べました。Sianos先生が私の助手をすることになっていました。Sianos先生 はギリシャ出身ですが、本当にギリシャ彫刻のようです。私たちは戦略について検討しました。 夕食を摂りながら、Sianos先生は私に、「成功の可能性はどれぐらいと見込んでいる?」と聞き ました。私の回答は簡単なものでした。「水曜日、私は自分が見せることができる全てを見せる つもりです。私ができないものは見せることができません。」 夕食は11:00 PMまでには終わ りました。ベッドに横になりましたが、いつものようにひどい時差ぼけでほとんど眠れませんで した。

翌日は自由な日でした。我々のホテルにはたくさんの日本人ビジネスマンや旅行客が宿泊 していました。さっさと朝食を食べてからホテルの周囲を歩き回りました。大きな河にかかる大 きな橋がありましたが、これはエラスムス大橋でした。大きな旅客船が川端に停泊していました

ホテル裏の小さな港と、歴史的ホテル「ホテル・ニューヨーク」を結んでいる河タクシー がありました。土田先生が、そのホテルの中のレストランでの昼食に連れて行ってくれました。 かつてたくさんの人々がこのホテルからニューヨークに移住しました。ホテルの壁には「オラン ダ-アメリカ航路」という文字が残っていました。昼食を終えてから、部屋で休み、それから日 本料理屋「札幌」で、大塚 雅人先生、谷本 周三先生達と共に夕食を楽しみました。彼らもこ のエラスムス大学病院に研修に来られているのです。
何時もの如くひどい時差ボケでした。とても早く目が醒めました。7:00 AMにはエラスム ス大学に行きました。Sianos先生が現れてから患者さんの部屋に連れていって下さいました。 過去に行われた2回の不成功PCIのために、大腿動脈拍動は弱くなっていました。右上腕動脈 からのアプローチも考えていたので、右上腕動脈拍動も確認しましたが、腕が脂肪で太いため、 これも拍動がとても弱く感じました。

患者さんは7:30 AM頃に第三カテ室に搬送されました。この日の戦いの火蓋が切られまし た。Serruys教授は別のカテ室で自分の患者さんの治療を開始しました。その部屋には最新医療 設備、ステレオタクシスや、超音波アブレーションなどが設置されています。私たちはほぼ同時 にスタートしました。それがSerruys教授の考えでした。彼は、最新医療機器による慢性完全 閉塞治療をデモし、私が日本のテクニックについてデモする、というものでした。
Serruys教授の症例は比較的真っ直ぐな左冠動脈前下行枝中位にある短い慢性完全閉塞病 変の症例でした。その症例はあまり難しそうではありませんでした。私は術衣に着替える間もあ まり緊張していませんでした。自分自身に言い聞かせていました。「私は自分ができることしか できない。でも、今日は私ができること全てを見せよう。」 少し苦労していましたが、Sianos 先生は右大腿動脈に7Fr シースを挿入しました。俺は私に右上腕動脈にシースを入れるように 頼んできました。彼はその経験が無いのだそうです。拍動は弱かったのですが何とか7Fr シー スを挿入しました。7 Fr 85-CMのBL3.5SH(TERUMO)を右上腕動脈から左冠動脈に挿入しま した。そして、7Fr JR4.0SH(Medtronic)を右大腿動脈から右冠動脈に挿入しました。左右同時 造影を行いましたが、前回右冠動脈の慢性完全閉塞近位部に入れられたTAXUSステント部が完 全閉塞になっていました。事態はこれまでよりも深刻でした。閉塞長がもっと長くなっていたの です。
最初のプランの通り、私は中隔枝経由の逆行性アプローチを行いました。Finecross、 Ryujin-OTW 1.25mmバルーンそしてFielderを用いて慢性完全閉塞遠位端まで到達しました。 右冠動脈のガイディング・カテーテルはバック・アップが取れませんので、1.5-mmバルーンを Conus枝に入れて拡張してアンカーとしました。もしも、前回ステントを入れておられなけれ ば、1時間以内に成功裏に終了していたことでしょう。しかし、ステントが入れられていたため に、順行性にも逆行性にも病変をクロスするのはとても大変でしたが、最終的にクロスすること でできました。それから5 Frの子カテを用いて3つのTAXUSステントを植え込みました。 Sianos先生は私が何をしているか理解できませんでした。最終造影はとても綺麗でした。 Antonio Colombo先生がパリでこのライブ・セッションで座長をしていました。 光藤和明先生はモデレーターの一人でした。
別の部屋ではSerruys教授が格闘していましたが、病変を通過することができませんでした。 私は会場にいませんでしたので、聴衆の反応は分かりませんでした。でも、後で聞いた話によれ ば、私の大成功に聴衆の皆が万雷の大喝采をしたということです。私だけでなく、カテ室内の皆 がこの大成功に幸せでした。
この日の午後にアムステルダムからトゥールースに飛ばねばなりませんでしたので、 Serruys教授に挨拶してから病院を離れました。アムステルダム発14:40分のAF 8263便に乗 って2時間の飛行の後でトゥールースに到着したのはもう5:00 PMになっていました。学会事 務局が車を用意している筈でしたが、待っている車はありませんでしたので、タクシーで市庁舎 Captoliumの前にあるGrand Hotel de L'Operaに向いました。夜はその中のLes Jardins de L’OperaというレストランでBruno先生と夕食を摂りました。
