After finishing the cases in PWH, I moved to Queen Elizabeth Hospital (QEH), which is the biggest hospital in Hong Kong and located in the center of Hong Kong Peninsula. I delivered a talk there during lunch time. Dr. KT Chan, Dr. Boren Cheng, Dr. Michael Lee and Dr. Duncan Ho kindly welcomed me. After the talk, I did one CTO case successfully with a retrograde approach. The case could be finished earlier than expected. I came back to the Peninsula Hotel, which is famous for “High Tea”. I enjoyed it at the cafeteria in the lobby of the hotel. There were many Japanese tourists enjoying it.

There was a symposium at night which was co-sponsored by the Hong Kong Medical Association and Hong Kong College of Cardiology titled “Advanced Cardiology”, which was held in the same hotel. There were many famous physicians invited like Dr. Alan Yang from US, Dr. Lee from Singapore, Dr. Shim from Malaysia, and so on. Actually, the meeting was provided with Chinese dinner. However, I really wanted to enjoy Hong Kong food. We moved to Hong Kong Island and took spicy crab there. It was nice experiences for me.

November 22nd Wednesday 2006
I again visited Queen Elizabeth Hospital in the morning. The patient had long chronic total occlusion in the proximal LAD. The previous attempt failed. I approached this patient by the retrograde approach. The retrograde guidewire successfully passed through the lesion through the septal connection into the ascending Aorta. However, the retrograde advancement of any balloon was not successful, so that the retrograde guidewire planed just only for the land mark. While keeping the wire in position, I tried the antegrade approach. At the end of my attempt, I could achieve incomplete antegrade dye filling. However, I classified this case as one of the failed cases.

Thus, I succeeded in 3 of 4 CTO cases with previous failed attempts. If I count the cases done in the week before, the result was 9 cases with successful guidewire passage and 7 cases with complete dilatation and stenting with complete success among 9 CTO patients with previous failed attempts. Although I am satisfied with this result, I also think that I have to polish my ability for CTO lesions up more.
After finishing the case without complete success, I went to have lunch in a nice Chinese restaurant with Dr. Ho and Dr. Lee. Then, I moved to the airport to take my returning flight to Japan of JL 732 departing at 15:20.

前回の香港訪問からすぐの香港
11月17日の夜に香港から戻ってから、二つの重要な仕事が日本でありました。一つは2006 年11月18日土曜日に開催された、「PCIにIVUSは必要か」というタイトルのディベイト・セ ッションでの、”Con”の役割で論陣を張ることでした。もちろん、結果は私たちの勝ちでした。 翌日日曜日には朝から夜まで、TRIトレーニング・コースで座長として忙しく過ごしました。
2006年11月20日月曜日に成田発21:25のJL 735便で再び香港に飛びました。有名な Peninsulaホテルにチェック・インできたのは夜の11:00 PMを回っていました。

九龍側にあるPrince Wales Hospital (PWH)を訪問しました。ここでは2例のCTOを行い ました。ここのカテ室は皆良く動き、親切でありとても快適でした。病院は80年代終わりから 90年代初めにかけて開発された新租界地区にあります。病院は香港の中でも大きい病院の一つ です。

PWHでの症例を終えて、Queen Elizabeth Hospital (QEH)に向かいました。この病院は香 港最大の病院で、香港半島の真ん中に位置します。まず、昼食を摂りながら、講義を行いました。 Dr. Chan, Dr. Cheng, Dr. LeeそしてDr. Hoが揃って迎えてくれました。この講義の後、一例 CTOをretrograde approachで行い、うまく終了しました。この症例は予想されていたよりも 早く終了したため、私は、あの有名なPeninsulaホテルのロビーにあるカフェで”High Tea”を 楽しむことができました。日本人観光客がたくさんいました。

夜には香港内科学会、香港心臓病学会主催のシンポジウムがPeninsula Hotelで開催されま した。タイトルは”Advanced Cardiology”でした。このシンポジウムには、米国からAlan Yang 先生、シンガポールからLim先生、マレーシアからShim先生など有名処が参加されました。 このシンポジウムの終わりには中華料理の夕食が出ましたが、私は香港料理を食べたかったので、 皆で香港島にある辛い蟹が美味しい店に行きました。とてもよい経験でした。

朝から再度QEHを訪問しました。患者さんはLAD近位部に長いCTOを有する患者さんで した。以前にPCIが試みられましたが不成功に終わりました。私は、逆行性にガイドワイヤー をクロスし、そのガイドワイヤーは上行大動脈にまで抜けましたが、逆行性にはどうしてもバル ーンを通過することができませんでした。そこで、このガイドワイヤーは単に目印として使うこ とにして、順行性に試みました。最終的に順行性に造影されるようになりましたが、この症例は 不成功、と認定しました。

結局、4例の失敗例CTOのうち3例で成功しました。この前の週の症例も合わせると、9 例の失敗例CTO症例に対して、9例全例でガイドワイヤーをクロスすることに成功しましたが、 7例でのみ完全成功でした。まあ、この結果には満足です。しかし、もっと精進せねばならない、 とも感じました。
この症例を不完全なまま終了してから、私は、Ho先生そしてLee先生とともに中華料理の レストランで昼食を摂りました。それから、15:20発のJL 732便に乗って帰国するため空港に 向かいました。
