SCAAR

SCAAR means “Swedish Coronary Angiography and Angioplasty Registry”. It is a very famous registry and covering all of the cases of coronary angiography and angioplasty performed in 27 hospitals in Sweden. It was my great honor to do PCI in one of the core hospitals in SCAAR.

I departed NARITA on November 25th Sunday 2007 and arrived in Stockholm, Capital of Sweden, later than 9:00 PM on the same day via Frankfurt. It was 5 degree outside, which was warmer than I expected. I was invited from Dr. Mikael Aasa of the South Hospital as single operator and lecturer in the CTO workshop.

November 26th Monday 2007

While skipping breakfast, I visited the South Hospital, which is a big university hospital. After having lunch, I started 1 case with a long CTO lesion in the RCA during the CTO workshop. I did this case through the retrograde approach. It was a really difficult case but successful. Most of Swedish Interventional Cardiologists joined the workshop. They were watching my procedure in an auditorium. After successfully crossing a guidewire through the CTO lesion, I placed 3 Endeavor DES stents. SCAAR reported a very sensational result during European Society of Cardiology (ESC) meeting in 2006, that the mortality after DES was worse than after BMS implantation. After the report, the use of DES in Sweden dramatically dropped from 60 to 20%. It was so interesting that Endeavor was not a real DES but a good BMS for SCAAR physicians. Anyway, the previous result was rejected during the ESC meeting in 2007.

Dinner of the day was held in a Swedish restaurant in the South Island. Stockholm is called “Venice in North Europe” as well and composed of islands can canals. The South Island is the center of Stockholm. My hotel as well as the hospital was in the South Island. The food in the restaurant was nice. However, I had a big problem during the dinner. I do not take chicken or duck. However, the main dish was duck. I tried my best to take that dish. Finally, I took at most one quarter of the dish.

After finishing the dinner, it started to snow outside. It was the first snowing of the year in Stockholm, and temperature was getting lower and reached below -5 degree.

November 27th Tuesday 2007

I did 3 patients in the day. All of the cases had difficult CTO lesion. As a conclusion, I was successful in opening the CTO lesions in 3 of 4 patients in total. We did a lot of discussion. It was a really nice workshop. I believe that every participant could learn much how to treat CTO lesions.

It was a nice memory for me that I could perform PCI for difficult CTO cases in the center of SCAAR, and that those cases were also registered in SCAAR. I could contribute something to this famous registry of PCI “SCAAR”.

November 28th Wednesday 2007

It was very cold outside. The temperature was -10. I went to the Palace around 11:30 AM, since there is the exchange of palace guards from 11:30 AM on Wednesday. All of the soldiers from Swedish Navy and Army for Palace Guard looked shaking with cold. The ceremony was nice to see.

After leaving the Palace, I went to the International Airport to fly to Barcelona.

SCAAR

SCAARとはスウェーデン冠動脈造影・冠動脈インターベンション登録研究のことです。こ の登録研究はとても有名な研究であり、スウェーデン国内で冠動脈造影や冠動脈インターベンシ ョンを行っている全病院、といっても27しか無いのですが、そこでの全症例が登録されていま す。今回このSCAARの中枢にある病院で経皮的冠動脈インターベンションを行う機会を得たの はとても光栄なことでした。

2007年11月25日日曜日に成田を飛び立ち、フランクフルト経由でスウェーデンの首都ス トックホルムに着いたのはもう9:00 PM過ぎでした。外は気温5度と私の予想よりも暖かいも のでした。南病院のMikael Aasa先生に同病院で開催されるCTOワークショップでの唯一の術 者として招聘されたのです。

朝食は食べずに大きな大学病院である南病院に直行しました。そこで昼食を食べてから右冠 動脈のCTO症例をCTOワークショップの一環として一例しました。この症例に対しては逆行 性アプローチで行いました。とても難しい症例でしたがうまく行きました。会にはスウェーデン で積極的にPCIを行っているほとんどの医師が集まりました。彼らは講堂で私の手技を見てい ました。CTO病変をガイドワイヤーでクロスした後、Endeavor薬剤溶出性ステントを三つ植 え込みました。SCAARは2006年の欧州心臓病学会でとても有名なセンセーショナルな発表を しました。その発表では、薬剤溶出性ステントを植え込んだ方が、通常ステント植え込み後より も長期死亡率が悪かったというものでした。この発表の後、スウェーデンでは薬剤溶出性ステン ト使用率が60%から20%に急落したのです。SCAARのお医者さんにとってはEndeavor薬剤 溶出性ステントはまるで「良い」通常ステントとして考えられているのはとても面白いことでし た。どちらにしても、先の結果は2007年欧州心臓病学会で否定されました。

夕食会は南島のスウェーデンレストランで行われました。ストックホルムは「北欧のベニス」 とも呼ばれ、たくさんの島と運河があります。私のホテルも病院も南島にありました。レストラ ンでの食事はとてもおいしかったのですが、一つ大きな問題がありました。メインの料理は何と あひる料理だったのです。僕は鳥がとても苦手なのですが、苦労して1/4だけ食べました。

夕食を終える頃には雪が降り出しました。その雪が2007年最初の雪だったそうです。気温 は下がり、マイナス5度以下でした。

この日は3例行いました。何れもとても難しいCTO症例でした。結果的に4例の内3例で 成功しました。多くの議論を行いました。とても良いワークショップだったと思います。どのよ うにCTO症例に対してPCIを行うかを参加者は学ぶことができたと思います。

SCAARの中枢で困難なCTO症例に対してPCIを行うことができたのはとても良い記憶で す。しかも、僕の症例もSCAARに登録されたのですよ!　SCAARという有名な登録研究にい くばくか寄与することが出来たのです。

外はとても寒かったです。気温はマイナス10度でした。11:30 AMに王宮に行きました。そ こでは水曜日の11:30 AMから衛兵の交代式が行われるのです。衛兵はスウェーデン海軍と陸軍 から選ばれるのですが、皆寒さに震えていました。交代式は素敵でした。

王宮から直接国際空港に向かい、そこからバルセロナに飛びました
