The 1st ACT Training Course

I left for Seoul from Haneda Airport by JL 8833 flight departing at 15:35 on January 13th Tuesday 2009. I was invited by Dr. SJ Park at Asan Medical Center in Seoul as an operator, a lecturer and the 1st Visiting Professor for the 1st ACT Training Course. ACT is “Asan Medical Center Interventional Cardiology Training Course”. Dr. Park is inviting 10 to 15 relatively young interventional cardiologists from Asian nations to Asan Medical Center for 1 week. They are sitting in the conference room next to the catheter laboratories from every early morning to the evening. In the morning, they have live transmission from the catheter laboratories mainly by the doctors in Asan Medical Center. In the afternoon, they have lectures. They can visit at the laboratories anytime to see how the procedures are being done. Dr. Park invented this program. The 1st course was held from January 12th to 16th. I was invited as only one foreign operator and lecturer for this program. Dr. Park was planning to hold these one week course in every month except April in 2009. After arriving at Kimpo Airport, I had dinner in a seafood restaurant near the hotel.

January 14th Wednesday 2009

I arrived at the catheter laboratory in Asan Medical Center around 7:30 AM. I did not have any information about how many patients and what kinds of patients I would treat. A young doctor took me to the viewing station and introduced 2 cases for me. The 1st lady patient had a CTO lesion in the proximal LAD, which was tried to open without a success previously. The 2nd patient had one CTO lesion in RCA and another CTO lesion in LAD. They seemed not so easy.

I started the procedure from 8:15 AM. By using a typical double guidewire technique using Miracle 3 and Conquest-Pro guidewires, I crossed the CTO lesion in the 1st patient. I immediately moved to the 2nd patient. By using a double wire technique again, I first crossed the CTO lesion in RCA. After fixing this lesion, I moved to the CTO lesion in LAD. Again, I took a double wire technique for this lesion. I used Miracle 3, Conquest-Pro and Conquest-Pro 12 guidewires. Finally, I finished these 2 cases by 10:15 AM. I thought that I could finish all of the cases. I thought that I was released from the duties, and got relaxed.

However, everything is not so easy in my life. A young doctor came and told to me, “It is so early. We prepared another difficult CTO case for you. You can quickly come to the next laboratory.” I was so surprised and moved to the next room. They showed the case. They just finished a CYPHER stent implantation in the middle LAD. The patient had a long CTO lesion from the ostium of his RCA. I started the case from 10:30 AM. The antegrade approach looked impossible. I took the retrograde approach from his LAD. It was a really difficult case. Also, the possible selection for the guiding catheters was not enough. Finally, I could open the difficult CTO lesion and placed several CYPHER stents. The final result was fantastic. It was almost 13:00. Thus, I could open 4 real CTO lesions in 3 patients using 3 antegrade double wire techniques and 1 retrograde approach within 5 hours in total. After finishing my cases, I had lunch in the conference room by the laboratory. I delivered a lecture on CTO angioplasty. At the end, Dr. Park presented me the crystal certificate that I was the 1st Visiting Professor in Asan Medical Center Cardiology Center. It is my great honor.

I quickly moved to Kimpo Airport and caught a returning flight to Haneda. Thus, I stayed in Seoul only for 20 hours, did complete angioplasty for 4 CTO lesions in 3 patients and delivered a lecture. I think I have done a good job.

第一回ACTトレーニング・コース

2009年1月13日火曜日の15:35羽田空港発のJL 8833便でソウル・金浦空港に飛び ました。私はAsan Medical CenterのSJ Park先生から第一回ACTトレーニング・コース での術者、講師および最初の招聘教授として招かれました。ACTというのは”Asan Medical Center Interventional Cardiology Training Course”のことです。Park先生はアジアの国々 から比較的若手のインターベンション医師10から15名を1週間Asan Medical Centerに 招いているのです。彼らは毎日朝から晩までカテ室の隣のカンファレンス・ルームに座りま す。午前中は、主としてAsanの先生方によるカテ室での手技をライブ中継します。午後に は講義があります。また、何時でもどのように手技が行われているのか見るためにカテ室を 訪れることができます。Park先生がこのプログラムを考案されたのです。第一回目のコー スはこの1月12日から16日開催されました。私は、このプログラムの唯一の外国人招聘 者として招かれました。Park先生の考えでは、そのようなコースを2009年中は4月以外 毎月開催されよう、とのことです。金浦空港に到着してからホテル近くの海鮮レストランで 夕食をしました。

朝7:30 AMにAsan Medical Centerカテ室に到着しました。どのような患者さんを治 療するのか、何の事前情報もありませんでした。若い先生が私をシネ端末の前に連れて行っ て下さり、私に2例見せてくれました。最初の患者さんはLAD近位部のCTOでした。以 前にPCIが試みられましたが不成功だったそうです。二例目は右冠動脈にCTOがあり、そ して、左冠動脈前下行枝にもCTOがありました。二例とも易しい症例には見えませんでし た。

8:15 AMから手技を開始しました。Miracle 3とConquest-Proによるdouble wire techniqueを用いて、最初の患者さんのLADの慢性完全閉塞病変をクロスしました。その まま連続して二例目に移りました。右冠動脈の慢性完全閉塞に対してはまたもやdouble wire techniqueにより開通させました。この病変を直してから、LADのCTOに移りました。 またもや、Miracle 3, Conquest-Pro, Conquest-Pro 12によるdouble wire techniqueを用 いてうまく開通させました。結局、この2例を10:15 AMまでに終了させました。これでも う解放されたと、張りつめた緊張から自分を解きました。

しかし、人生ってそんなに簡単ではありません。若い先生が僕の所に来ました。そして、 「まだ早いし、だから先生にもう一例CTOを準備しました。だから、早く隣のカテ室に移 って下さい。」　私はとても驚きました。そして、隣の部屋に移動しました。症例を見せて くれました。彼らはまず、左冠動脈前下行枝の狭窄病変にCYPHERステントを植え込んだ ところでした。患者さんは右冠動脈入口部から長い慢性完全閉塞病変がありました。私は、 結局10:30 AMからこの症例を開始しました。順行性アプローチは不可能に思えました。そ こで、左冠動脈前下行枝からの逆行性アプローチを開始しました。とっても難しい症例でし た。さらには、十分な形状のガイディング・カテーテルがありませんでした。最終的にこの 難しい慢性完全閉塞も開けることができ、何本かのCYPHERステントを植え込みました。 最終結果はとってもすばらしいものでした。時刻は13:00になっていました。かくして、私 は3例の症例の4か所の本当の慢性完全閉塞病変に対して、順行性アプローチ3回、逆行性 アプローチ1回を用いて5時間以内に全部を開通させることができました。症例を終え、カ テ室横の部屋で昼食を摂りました。その後、慢性完全閉塞に対する冠動脈インターベンショ ンの講義を行いました。最後にPark先生が、Asan Medical Center心臓センターの第一番 目の招聘教授を証明するクリスタルの記念碑を僕に授与して下さいました。とっても光栄な ことです。

私はすぐに金浦空港に向い、そのまま羽田に飛んで戻りました。結局今回のソウルは 20時間しか滞在しませんでしたが、3症例の4つの慢性完全閉塞病変に対して完璧な治療 を行い、講義も行いました。我ながら良い仕事をしたと思います。
