Introduction

This visiting report is again about Moscow, Russia. This was my 3rd visit in Moscow after my 1st visit in June 2002. I love this beautiful city, Moscow.

Between Moscow and Tokyo, only 2 airline companies have direct flight. One is Russian National Airline Company, Aerofloat, and another is Japan Airline (JAL). However, JAL has only 1 flight every week on Tuesday from Tokyo, so that I took NH1209 flight (ANA) to Frankfurt on April 21 Wednesday, 2004, which is connecting with LH3188 flight (Lufthansa) from Frankfurt to Moscow. The NH1209 flight departed at 11:40am and arrived at Frankfurt airport at 16:40. The connection to LH3199 flight was very smooth. I finally arrived at Moscow airport at 22:40 on the same day. Because there are 5 hours’ time difference between Tokyo and Moscow, the total flight time was as long as 16 hours. However, I can tell you that more terrible thing is not long flight time but long waiting time for immigration control in Moscow Airport. Before my arrival, there was an arrival from Seoul by KAL (Korean Air). So many Japanese group tourists as well as Korean people took this flight that the immigration area was much crowded. Russia is no more communistic country. However, a lot of bad practice still remains everywhere especially in public Authorities. The space for immigration control is very narrow, and only several officials on duty are working irrespective of long waiting lines. I spent almost 1 hour for immigration as always as my previous twice experiences, and so I could leave Moscow Airport at 23:30.

The hotel, where I stayed this time, was Hotel Baltschug Kempinski, where I am always staying in Moscow because of the availability of high-speed Internet connection. This hotel is a German hotel and located on the other side of Moscow River for Kremlin Palace. Because I stopped at a Cafe to have late dinner on the way to the hotel, I finally checked in the hotel at 1:00am on April 22.

I came to Moscow to attend the 5th International Symposium of Cardiovascular and Interventional Radiology meeting organized by Center of Endosurgery and Lithotripsy (CELT) as one of the invited foreign faculty member. The medical terminology of “Lithotripsy” means “to break stones like renal or gallbladder stones without skin incision by using an ultrasound or shock wave. The meeting was held in the congress hall of Tretyakovsky National Art Gallery, which is located at 10 minutes’ distance from our hotel on foot. There were almost 150 attendants in the meeting room. Every seat was equipped with a piece of earphones for simultaneous bidirectional translation between English and Russian. Since I arrived at the hotel very late on the day before and my lecture role was scheduled from 13:40, I joined the meeting around 13:00. The title of my lecture that day was “Angioplasty for Chronic Total Occlusion ? Tips and Tricks in 2004 ?“, as usual for these several months.

After finishing my lecture, I hurried to CELT. CELT is actually only one private hospital in whole Russia except several small private Clinics of Ophthalmology or Plastic Surgery, and located in the east side of Moscow city. CELT was established about 10 years ago by several physicians whose specialties were Interventional Cardiology, Interventional Radiology or Endoscopic Surgery. Now, CELT has 55 beds in active use and 28 doctors working solely in the hospital as well as more than 80 visiting physicians of various specialties. For the Cardiovascular department, they have 2 vascular surgeons, 3 Interventional Cardiologists and 3 Intensive Care Unit physicians. The department accommodates about 40 patients hospitalized every week. Last year, they performed 400 cases of percutaneous coronary intervention, and surprisingly almost 90% of them were done through the radial approach. I have never known that TRI has been used in Russia. The chief Interventional Cardiologist in CELT is Dr. David P. Doundoua .

The traffic of the street to CELT is always most congested in Moscow, because it is the most important street for business transportation. The traffic gets worse sometimes due to the mechanical failure of public trolley buses. In those cases, the drivers, who are normally women, try to fix the mechanical problems by themselves.

In CELT, they prepared one 43 male patient for me to introduce the 1st case of Rotablator in whole Russia. The patient had previous CYPHER stent implantation in mid-LAD and 3 bare metal stents implantation in LCX. He presented with the 2nd instent restenosis in LCX. The strategy of this time was to do Rotablator first and then place CYPHER stent within the previous stents. Because that case was the 1st case of Rotablator in Russia, I carefully took a 1.5mm burr through a 7-French guiding catheter with the assistance by David. After Rotablator, I placed a 2.75x28mm of CYPHER stent within the previous stents. Zaza (Dr. Zaza A. Kavteladze) was watching the procedure in the cathlabo. The case was successful and transmitted to the meeting room via Internet.

While we were discussing about the case after PCI, David suddenly told me that they were not Russian but Georgian. He told he was born in Georgia and came to Moscow. Not only him but Zaza was also Georgian. There are many Georgian doctors working in Moscow. When I heard the name of Georgia, I could not understand where it was. After coming back to the hotel, I tried to find where Georgia is by using Internet. Then, I found that Georgia was called Griziya in Japan, that’s name is famous in many Japanese people as a beautiful country. Georgia was an independent country and has the oldest alphabetical character sets in the world. Georgia was named after Saint Georgius, who is believed to protect the country. Soviet Union for many years, now Georgia is an independent country. The name of Griziya is actually the Russian perversion of Arabic name of Georgia ? Gurjistan. Mr. Edwald Schwarnaze, the former famous Minister of Foreign Affaire in Soviet Union, also came from Georgia. David told me, “I love my beautiful mother country, Georgia. I am very proud that I am a Georgian. However, I am very sorry that Georgia is now politically unstable.”

That night, we booked the Ballet in Bolshoi Theater from 7:00pm. While spending time before the Ballet starts, we looked around the center of Moscow on foot. There are many overbearing buildings, which were constructed during the communistic Government. The temperature in Moscow was 8 degree Celsius, and the sky was bright even at 8:20pm under the near midnight sun. Moscow is famous also for well-developed subway system, although the traffic jam in Moscow is very bad. The number of automobile cars running on the streets has markedly increased compared to even last year. The economic growth reached more than 10% every year in Moscow. The average price in Moscow Stock Market grew up by 60% during the last 6 months. Many people are now investing their own money into the stock market through the Internet. The cars in the street used to be old Russian-made cars spewing their dirty exhaust fumes in the air, but many of them changed to new German or Japanese cars in these 2 years.

The atmosphere inside the Bolshoi Theater was very attractive. However, the Ballet performed that night was not classical but modern and a little bit lazy for me. Thus, we left the Theater during the Intermission.

April 23 Friday, 2004

I presented my lecture entitled “Cypher Stent ? New Era in Coronary Stenting” from 9:00am on April 23 in the meeting. My 1st experience to use Cypher stent was 2 years ago in Moscow. After that time, I have used both Cypher and TAXUS in many patients in various foreign countries but not in Japan. During my presentation, I talked about these experiences of using Cypher stent in the foreign countries. The departure time of my returning flight from Moscow to Japan via Frankfurt was 16:55. Because the day was Friday and many people go out by car from Moscow to the suburb to spend the weekend, people in Moscow recommended us to leave for the airport at last 3 hours before the departure time. Although we left Cafe Pushikin around 1:30pm after having lunch, I arrived at the airport at 16:15 due to the unbelievable traffic jam.

Cafe Pushikin is one of the historical and famous Restaurants in Moscow. The building is very old (historical). It is difficult to indicate any differences between Cafe and Restaurant in Russia. We can have nice and very formal dishes in both of them. The price is very similar. However, I guess that Cafe is open to later midnight compared to Restaurant. For example, Cafe Pushikin opens 24 hours a day. Thus, I recommend you to visit this attractive Cafe, if you arrive in Moscow very late and to have late dinner. You will never regret it. Another suggestion to you is you should order the menu of “Beef Storganov”. I think it is one of the best dishes in Russia, and quite different from those we can have in Japan. But, I have to tell you that the prices in Moscow are very expensive for everything, and those in nice Restaurants are displayed not in Russian Rouble but in Euro. If you order one dish of black caviar, you have to pay almost 4,000 Japanese Yen (30 Euros).

One hint in Russia

Russian alphabetical characters are totally different from English characters. For example, “P” in Russian is “R” in English, “H” for “N”, and so on. Thus, if you find the signboard of “PECTOPAH” in Russia, you know that is “Restaurant”.

St Petersburg

The stature of limitation
　After closing my mouth for 1 year, I tell you my experience in St. Petersburg last year. I went to St. Petersburg, the most beautiful historical city in Russia, after visiting Moscow in June last year. That trip was just for relaxing. I was very much impressed by the beautiful scenery and a lot of historical wisdom in that historical city. In the last night of my stay in St. Petersburg, I booked a Restaurant, which is famous for traditional Russian foods and Gipsy music. I confirmed that Gipsy music would be played just around the table.

We arrived at the Restaurant a few minutes before they opened the door. I was kept waiting for the Gipsy music team coming around my table. After 2 hours, they came to us, and started the live music. Next to our table, there were 6 to 7 people sitting around a big table. They were talking about some Russian business. One big gentleman with clear and loud voice like a tenor singer was frequently speaking the word of “Siberian business”. People except him were just hearing his speech. The Gipsy music team finally came to our table and started the music after 2 hours’ waiting. However, the guy with loud voice called the musicians, and the music was not continued any more around our table. After I waited for another hour to have live music, I was finally disappointed to know that we would not have any live music any more. Then, I got angry. I got out from the Restaurant without any payment. Of course, the manager of the Restaurant came to me and asked me “Why you do not pay for the food?” I told him “I confirmed that the live Gipsy music would be definitely played beside our table at my reservation. However, after the order of somebody, all of the live music was stopped. Thus, I have no duty to pay anything for you.” Then, the manager told me “Please, could you pay at least for the food you ate?” I answered “No, because I reserved the table for food and music, I cannot pay if I cannot have music.”

Finally the manager told me “Please, do not blame me. He, the guy who ordered to stop the music, is the Minister of Trade in Russia. If he orders to close this Restaurant, how I can stand against that? So, please pay for the food.”
I answered to him, “Ok, that is a good reason for you but not for me. I am totally independent from your situation. You promised me to show live music in front of my table. But, you did not do it. Thus, I would not pay anything.”
After my strong claim, he was almost crying. Then, I changed my policy to make negotiation with him. Finally, he arranged another table for us, and asked Gipsy music team to play live music in front of us with other customers.

From this experience, I learned that Russia was still a very bureaucratic country as similar as the past Communistic country.

はじめに

今回の訪問記は再びモスクワです。初めて訪れたのは2002年の6月のことであり、今回は3回目の訪問ということになります。私はこの美しいモスクワのことをとても好きです。

モスクワ-東京間に直行便を飛ばしているのはロシア国立航空会社エアロフロートと日本航空(JAL)だけですが、 JALは週一便火曜日にしか飛んでいません。従って今回は全日空NH1209フランクフルト行きの便で2004年4月21日水曜日に出発しました。 この便はフランクフルトでルフトハンザ航空のLH3199モスクワ行きの便に接続しています。NH1209は11:40amに出発し、フランクフルトには16:40に到着しました。 そこでの乗り継ぎはスムーズでモスクワ空港には同日22:40に到着しました。日本との時差は5時間あり、全旅程に16時間もかかったことになります。 でもモスクワ空港での入国審査場での行列に比べればこの長い飛行時間なんて楽なものです。丁度、フランクフルトからの便の前にソウルからの KAL(大韓航空)便が到着しました。この便には日本からのツアー客も多く、入国審査場は日本人と韓国の人たちで込み合っていました。ロシアは もはや共産主義国家ではありません。しかし、特に公共施設ではその時代の悪習がまだまだたくさん残っています。入国審査場はとても狭く、 どんなに長い行列ができていても入国審査官を増やすこともありません。結局のところ、ここで1時間ぐらい費やして23:30になってようやく空港を 出ることができました。

今回私が宿泊したホテルはいつものようにBaltschug Kempinskiホテルでした。このホテルはドイツ系のホテルで 高速インターネット回線が整備されています。ホテルはモスクワ河を挟んでクレムリン宮殿の向かいにあります。空港からの途中にカフェで遅い 夕食を摂ったため、結局ホテルにチェック・インしたのは翌日の午前1時になっていました。

今回私がモスクワに来たのは”管腔内治療及び結石破砕センター” (Center of Endosurgery and Lithotripsy: CELT)主催による第五回 国際心臓血管放射線インターベンションシンポジウムに外国からの招聘講師の一人として参加するためでした。この結石破砕という医学用語は 皮膚切開することなく、超音波や衝撃波を用いて腎結石や胆石を体外から細かく砕いてしまう、という意味です。学会は国立トレチャコフスキー美術館の 会議場で開催されました。ホテルからその美術館までは歩いても10分ぐらいの距離です。学会の参加者は150名ぐらいでした。会場の各席には ロシア語と英語の双方向同時通訳のためのイヤホンが備え付けられていました。前日遅くにホテルに入ったので、私は自分の発表予定時刻が13:40 であることをいいことに、会場には13:00ぐらいに入りました。今回の講演題目はここ数ヶ月のお得意題目である「慢性完全閉塞に対するPCI ? 2004年における工夫」という題でした。

講演終了してすぐにCELTに急ぎました。これまでも眼科や美容形成外科の小さなクリニックでは私立のものが ロシアにもあったそうですが、有る程度の規模のものとしてはこのモスクワ市の東に位置するCELTが全ロシアで唯一の私立病院なのだそうです。10年前に PCIを行う循環器内科医、放射線科専門医そして内視鏡外科医達が集まってこのCELTを開院したそうです。現在CELTでは常時55ベッドぐらいを動かし、 28名の常勤医そして80名以上の色々な分野の非常勤医師が勤務しているそうです。循環器科としては血管外科医2名、Interventional Cardiologists 3名そしてCCUを担当する医師3名の計8名を有し、これで毎週40名の入院患者さんに対して治療しているそうです。昨年のPCI症例数は400であり、 驚くべきことにその90%がTRIにより治療されているそうです。これまでロシア国内でTRIが行われているとは思ってもいませんでした。主任医師は Dr. David P. Doundouaです。

CELTへの道路は主要な幹線道路なのでモスクワ市内でも一番渋滞がひどい道路です。しかも、時には公共 トロリー・バスが故障のため立ち往生しています。そうなると、ドライバー(たいていは女性ドライバーです)が自分でその修理をすることになります。

CELTでは私のために43歳の男性患者さんを用意しておられました。この患者さんは以前にLADにCypherステントを 植えられ、またLCXには3個のbare metal stentsが入っている患者さんです。LCXの二回目のステント内再狭窄に対してロシア初のロータブレーターを 披露するために私に術者としての白羽の矢が立った訳です。今回の治療戦略は、まずロータブレーターを行い、その後Cypherをステント内に植え込むという ものでした。何しろロシア初のロータブレーター症例なので慎重を期して7Frのガイディング・カテーテルを用いて1.5mmという小さめのバーでロータブレーターを David先生に助手をして頂きながら行い、その後2.75 x 28mmのCypherステントをステント内に植え込みました。Zaza先生はこの手技の様子をカテ室内で じっくりと見ておられました。

手技終了後、色々なことを話している時に、突然Davidが「自分たちはロシア人ではない、ジョージア人だ。」と、 言いました。そして、ジョージアで生まれ、その後モスクワに来たのだそうです。更に、DavidのみならずZazaもジョージア人だというのです。正直、それを聞いても ジョージアが何処なのかさっぱり分かりませんでした。ホテルに帰ってから早速インターネットで検索しました。何と、ジョージアとはいわゆるグルジアのことだったのです。 この名前であれば多くの日本人が美しい国として知っていると思います。ジョージアはれっきとした独立国であり、世界最古のアルファベットを有する国です。 そもそも守護神聖ジョルジウスに因んでこの国の名前がつけられたそうです。

長い間ロシアあるいはソビエト連邦の占領下にありましたが、今や独立した国家です。 そもそもグルジアという呼び名はジョージアのアラビア語、グルジスタンがロシア語の中でゆがめられて名づけられた名前なのだそうです。以前のソビエト連邦の 有名な外務大臣シュワルナゼ氏もこのジョージア出身です。David先生は私に、「美しい母国ジョージアのことをとても好きだし、自分がジョージア人で あることを誇りに思っている。でも、現在は政治的に不安定なので残念だ。」と、仰っていました。

その夜はボリショイ劇場でのバレー観劇を7:00pmから予約しました。それまでの間、街をぶらぶらと散策しました。 市内には旧共産国家時代に建造された大げさなビルがいくつもあります。モスクワの気温は摂氏8度であり、白夜とまではいかないまでも午後8:20になっても 外はまだ白々していました。モスクワはまた発達した地下鉄網でも有名です。でもモスクワの交通渋滞はひどいですよ。自動車の数は昨年と比べても 明らかに増加しています。何でもモスクワの年経済成長率は10%に達し、モスクワ株式市場の平均株価はこの半年間で何と60%も上昇したのだそうです。 皆、インターネットで株式投資をしているそうです。昔は街を走っていた自動車は古いロシア製のもので、汚い排気ガスを撒き散らしていましたが、 この二年間でその多くは新しいドイツ製あるいは日本製の自動車に替わりました。

ボリショイ劇場内部の雰囲気はとてもすばらしいものです。でも、この日のバレーはクラシックではなくモダンだったので 僕には退屈で途中休憩の時間帯に出てきてしまいました。

この日は朝9:00から「Cypherステント-冠動脈ステント植え込みの新しい時代」と題する講演を行いました。私自身Cypherを初めて用いたのは2年前 このモスクワにおいてでした。それ以来、CypherあるいはTAXUSを日本以外の色々な国々で多くの患者さんに対して用いてきました。私の講演の中では、 この外国での経験を交えて話をしました。モスクワから日本にフランクフルト経由が帰る飛行機の出発予定時刻は16:55でした。当日は金曜日で、 モスクワ在住の人々が週末を郊外で過ごすために車でモスクワが脱出する日でした。ですから、皆は少なくとも出発予定時刻の3時間前に出発することを 薦めました。私たちはプーシキン・カフェで昼食を摂り、午後1時半には空港に向けて出発したのですが、途中の交通渋滞は想像を絶するものであり、 結局空港に着いたのはぎりぎりの16:15になっていました。

プーシキン・カフェはモスクワ市内でも伝統のある有名なレストランの一つです。建物もすごく古いというか 格式があります。正直、カフェとレストランの違いが分かりません。どちらでもおいしい料理を食べることができますし、値段だって同じようなものです。 でも、カフェの方が遅くまでやっているような気がします。このプーシキン・カフェにしても24時間休み無くやっています。とにかくもしもモスクワに夜遅く着き、 しかも夕食を食べたいな、と思ったならばこのプーシキン・カフェを訪れることをお薦めします。絶対に後悔しないと思います。それと、行ったならば ビーフ・ストロガノフを注文するように。これはロシア料理最高峰のものですし、日本で食べるものとは全然違います。でも、気をつけて下さい。 良いレストランの料金表示はルーブルではなく、ユーロです。ですから、例えばキャビア一皿注文したら、きっと日本円にして4,000円(30ユーロ)ぐらいは覚悟して下さい。

ロシアで用いられているアルファベットは英語のアルファベットとは全然違います。例えばロシア語の”P”は英語の”R”ですし、”H”は英語の”N”です。 従って、街中で”PECTOPAH”という看板を見たならば、それは”Restaurant”のことだと思って下さい。

もう時効の話
この話は一年間内緒にしていましたが、昨年サンクト・ペテルスブルグで経験したことを今話しましょう。昨年モスクワを6月に訪れた後、サンクト・ペテルスブルグに仕事でなく 二日間行きました。サンクト・ペテルスブルグは歴史あふれるとても美しい街でした。サンクト・ペテルスブルグ最後の夜、私はジプシー音楽のライブを行うあるロシア料理レストランを 予約しました。そして予約の時に、絶対にジプシー音楽の実演をテーブルのそばで行うことを確認しました。

私たちは、レストランに一番に乗り込みました。そして、ジプシー音楽の演奏家達がテーブルに来るのを じっと待ちました。私たちのテーブルの隣には6,7名のグループが大きなテーブルの周りに座っていました。そして、どうやらロシアのビジネスについて 話しているようでした。中でも、テノール歌手のようにひときわ声の通る大きな男が盛んに「シベリア・ビジネス」とか演説しているのが聞こえました。 かれこれ2時間ぐらいしてやっとジプシー達が私達のテーブルに来て音楽を始めました。しかし、その大声の男が彼らを呼び音楽を止めました。 その後、一時間も待ったのですが、二度と私達の周りでは音楽を始めませんでした。それで私は怒って、レストランから支払いをせずに出ていこうとしました。 勿論、支配人が来て「どうしてお金払わないのか?」と問いただしました。私は、「予約の時に、ジプシー音楽を行うことを確認したのに、約束を破ったからだ。 誰かが音楽止めたじゃないか。だから、お金払う必要なんて無い。」と、答えました。彼は、「お願いだからせめて食事の代金だけでも払って欲しい。」と、 言いました。でも私は、「駄目だ、私はこのテーブルを食事のためだけでなく音楽のために予約した、だから音楽が無ければ支払いできない。」と、答えました。

最後に支配人は私に、「どうか私のこと責めないで欲しい。あの大声の人はロシアの通産大臣なんだから、 もしも彼がこの店を閉めろ、と命令すれば逆らうことできない。だからお願いだから支払って欲しい。」と言いました。私は、「ああそうなの。それはご愁傷様でした。 でもそんなことこっちには関係無いよ。あなたは私たちのテーブルの前で音楽をするって約束したでしょ。だから絶対に払わない。」と突っ返しました。

これだけ強く拒絶したので、支配人はもうほとんど泣き出しそうになりました。そこで、私は方針を変更して、 何とか彼を説得しました。最後にやっと、彼は他のテーブルを用意し、そしてジプシー音楽をそこで行うように演奏家達に頼みました。

この経験は、ロシアがまだ過去の共産主義国家の時代の官僚主義を未だに引きずっているのだということを 本当に思い知らせるものでした。
